1 Dear Scholarship Applicant, Thank You for participating in 2015 Scholarship Program. We are excited for the opportunity to reward our outstanding students and provide financial assistance in furthering your education. This year will award over $50,000 in scholarships to deserving students for their outstanding academic achievement, leadership, community service, and perseverance. Please carefully review all rules, deadlines, procedures listed below. Any applications that are late or incomplete will not be considered. General Rules and Requirements -Completed 12 Units as of fall Minimum 2.0 GPA -Application and letters of reference must be typed -You may apply for multiple scholarships on a single application. -Unless otherwise noted, scholarships are for educational expenses for the academic year and will be paid upon proof of enrollment for the Fall 2015 semester. *Please note that certain scholarships may have additional and/or different requirements All applications must be submitted by March 27, 4:00pm at the Student Life office, Student Center (Building 2300), room 2355 Thank you again for applying and we wish you the best of luck. 2 ELIGIBILITY CRITERIA: Applicants must be attending or entering a Four-Year College/University for the academic year. Applicants must have completed a minimum of 12 semester units (as of Fall 2014) at and be currently enrolled in a minimum of 6 units (as of Spring 2015). Please note that some scholarships may require additional information and/or a different personal statement from the applicant. Students may apply for more than one scholarship. AWARDS: Multiple scholarships may be awarded in the amounts of $300 - $1,000. SELECTION PROCESS: The scholarship committee will carefully review all applications and select the recipients. 5 Endowed and Community Based Scholarships OSHER SCHOLARSHIPS - $1,000 EACH Must be a student, with 24 degree applicable units by the beginning of the Fall 2015 semester. Must be eligible to receive the Board of Governors (BOG) Fee Waiver for the Must have applied for a FAFSA or the CA Dream Act application for the school year and demonstrate financial need. Please visit to apply for financial aid if you have not already done so. Student must meet Financial Aid Satisfactory Academic Progress (SAP) standards, with a minimum 2.00 cumulative GPA. Must be enrolled in at least 6 units during all terms in which the award remains active. Disbursement of award will take place at the beginning of the semester when enrollment status and SAP are reviewed. Award is non-transferable (can only be used for enrollment while at ). Awards will be adjusted based on enrolled status: Full-time (12 or more units per term) = up to $500/semester. Three-quarter-time (9-199 units per term) = up to $375/ semester. Half-time ( units per term) = up to $250/semester FRESH & NATURAL $500 This scholarship is to recognize four students who will be transferring to a 4-year college, who have exhibited leadership and a commitment to volunteerism in college or community activities. Applicants must be a student in good standing, have a cumulative grade point average (GPA) of 3.0 or higher, and completed all the steps under. BOOK STORE SCHOLARSHIP: - $500 & $500 STORE CREDIT This scholarship is to recognize 20 continuing students who have exhibited leadership and a commitment to volunteerism in college or community activities. Applicants must be a student in good standing, and have a cumulative grade point average (GPA) of 3.0 or higher at. MARYLOU CISNEROS MEMORIAL VETERANS SCHOLARSHIP $500 This scholarship was created to honor the memory of MaryLou Cisneros, Veterans Specialist. The purpose of the fund is to help a veteran or active member from a branch of the United States Armed Forces continue their educational pursuits. This scholarship application will be distributed with the ASCC Scholarship packet in the Spring and will adhere to their deadline dates and criteria for submission. The $500 scholarship will be awarded at the Annual scholarship Awards Ceremony in May. Honorably discharged veteran or active member of the U.S. Armed Forces Minimum GPA of 2.50 Currently enrolled in at least 6 units at at the time of the application Have never received this award in the past Items to include with scholarship packet: Two letters of recommendation (typed or word processed) There are no additional essay questions required for the scholarships listed above. 6 ZERMEÑO FAMILY SCHOLARSHIP- $500 The purpose of this scholarship is to recognize a Latina student who is committed to the community, to education, and to family. This scholarship remembers Maria del Rocío Zermeño Cárdenas, who went from a ten-year old poor Mexican immigrant in 1964, to one of the Women of the Year in Salinas, California in She was the sister of J. Francisco Zermeño C., profesor de español en. The Zermeño family believes in and maintains a true commitment to community, education, and family. For transferring students, a $500 check will be sent to the institution the student is transferring to, to be credited to the student s financial account. Continuing Chabot Students will receive a $250 Bookstore gift card for textbook purchases at the Bookstore and $250 credit to your Chabot account. (If you have a BOG fee waiver you will receive a check instead of a credit to your account.) Applicants must be a PUENTE participant in good standing, have a cumulative grade point average (GPA) of 3.0 or better, and completed all steps under Procedures for Application. 8 CHABOT COLLEGE CLASSIFIED SENATE SCHOLARSHIP AWARD $500 This award, in this amount of $500, is to acknowledge the work and contribution made by Student Assistants at. Only currently working Student Assistants may apply. Criteria requirements: Minimum of 2.5 GPA Currently working as a Student Assistant on campus Two reference letters; one from the person for whom you work, one from a person of your choice. 9 Melinda Matsuda Sponsored Scholarships EOPS AWARD(S) - $ EACH The purpose of this scholarship is to recognize two EOPS students who are first generation college students and have overcome life s obstacles to accomplish their educational goals. Applicants must be current EOPS students in good standing, have a cumulative grade point average (GPA) of 2.5 or higher, and completed all steps in the application process. One scholarship will be given to a student who is graduating and/or transferring, and another to a continuing student. 10 Melinda Matsuda Sponsored Scholarships STRIVING BLACK BROTHERS COALITION AWARD(S) $ The purpose of this scholarship is to recognize two Striving Black Brothers who have overcome the odds to accomplish their educational goals. Applicants must be current SBBC students in good standing, have a Fall semester grade point average (GPA) of 2.5 or higher and completed all steps in the application process. One scholarship will be given to a student who is graduating and/or transferring, and one to a continuing student. 11 The Friends of Foundation Sponsored Scholarships BERTY NEBENZAHL-RUTH TALLEY SCHOLARSHIP - $750 After retiring from successful careers as public school teachers, Berty Nebenzahl and Ruth Talley decided to attend classes at Chabot. They greatly enjoyed their courses, and as a result came to love the college. In particular, they thought the Quest Program was important because of its emphasis on older adult learning. Both Berty and Ruth were educators at heart, and Ruth came to teach in the Quest Program. She passed away in 2011, but Ruth s legacy lives on thanks to a generous gift made by Berty in both their names. 12 The Friends of Foundation Sponsored Scholarships MACHINE TOOL TECHNOLOGY $1,000 Mr. Woody Phillips, the donor, is a 2007 alumnus of the program, who wants to assist students in completing their studies. Must be a student enrolled in 12 units or more. Must substantiate financial need as documented by Office of Financial Aid Must be enrolled in Machine Tool Technology course of study Must complete application including essay and provide transcripts (unofficial accepted) Award may be used for books, school supplies, registration, fees, and associated expenses such as shelter, food, transportation, medical care, childcare Machine Tool Techonology Scholarship Additional Essay Question: Describe how this scholarship will assist you in completing your studies in machine tool technology and what you hope to accomplish when you graduate? 13 The Friends of Foundation Sponsored Scholarships PETER SATEGNA VETERNS SCHOLARSHIP $5,000 The purpose of this scholarship is to support student veterans who served in combat zones. Peter Sategna Educational Foundation has contributed funds in 2015 to a scholarship for student combat veterans. Sategna for whom the foundation is named served in World War II and used the GI bill to purchase a home in the East Bay upon his return from duty. He understood firsthand the challenges faced by returning veterans. Although Sategna passed away in 2007, his benefaction continues through his foundation and its numerous scholarships for students. The Sategna Educational Foundation believes in the practice of agency and expects recipients of their scholarships to interact with the Board of Directors. Scholarship recipients may be asked to join the Board of Directors or to serve as an advisor to the foundation in the future. Must be a student enrolled in 12 units or more Must substantiate financial need as documented by Office of Financial Aid Must have a GPA of 2.0 or higher Must be a veteran with preference for student veterans who served in active combat Must provide transcripts (unofficial accepted) Must remain in communication with the Peter Sategna Educational Foundation to report on outcomes as a result of receiving the scholarship Award may be used for Award may be used for books, school supplies, registration, fees Peter Sategna Vetern Scholarhip Essay Question: Describe the way that your military service has influenced your studies at Chabot and your view of the world? 14 The Friends of Foundation Sponsored Scholarships EDWARD E. AND DONNA L. MARTINS SCHOLARSHIP - $500 The purpose of this scholarship is to assist students in need. In 1963, distinguished local attorney Edward Ed E. Martins was elected to the Board of Trustees for the South County Community College District, as it was then called. After service of one term, he relinquished his position because he believed that elective service ought to be circulated among the citizenry. However his commitment to and the District has never ceased. Must be a student enrolled in 12 units or more Must substantiate financial need as documented by Office of Financial Aid Must have a GPA of 2.0 or higher Must complete application including essay and provide transcripts (unofficial accepted) Award may be used for books, school supplies, registration, fees, and associated expenses such as shelter, food, transportation, medical care, childcare Edward E. and Donna L. 15 The Friends of Foundation Sponsored Scholarships MICHAEL SWEENY - WORKING ZAPATOS SCHOLARSHIP $500 The purpose of this scholarship is to support graduates of any one of the Hayward high schools (Brenkwitz Continuation HS, or Hayward HS, or Mt. Eden HS, or Tennyson HS) who are attending and who actively participate in civic engagement activities. Michael Sweeney, the donor, is a former elected official whose service as the Mayor of Hayward, California State Assembly Member, and Deputy Secretary for Resources was distinguished by innovative community oriented public service. He hopes that this scholarship inspires students to lead a life of service. Must be a student enrolled in 12 units or more Must substantiate financial need as documented by Office of Financial Aid Must have a GPA of 3.0 or higher Must demonstrate active involvement in civic engagement activities such as tutoring in Hayward Unified School District schools Must have graduated from a Hayward high school and be a resident of Hayward Must be employed at least part time Award may be used for books, school supplies, registration, fees Working Zapatos Scholarhip Essay Question: Please list the name of the Hayward high school you attended. 2.
