1 There are now three simple ways to pay for school travel on routes 1, &. Customers with access to online home banking should download the online registration form from our website at In order to use online banking we will need to issue each pupil with a unique reference number. This number can then be used to set up your initial order and pay for your new ticket & any subsequent renewals. The unique number will also be printed on your ticket. On the form you will be asked to enter your child s name, date of birth and year group. As the bill payer, we will need your details. Please select the ticket type and enter you child s boarding point. From this form we will be able to establish which Zone your child boards. A summary can be found on page 4. E mail the form to us and we will issue you with a customer number from which you can make on line payments. You should not make any payment until you have received a unique number and confirmation from us. If you supply us with your e mail address, we will keep you up to date with service changes as and when they take place. We will also e mail you with service updates in the event of adverse weather. We will not share your e mail details or phone details with any other party. Sullivan Buses cannot/will not make any direct deductions from your bank account. All payments will need to be initialised by you. We will not complete your order until payment is made. Your pass/es will then be sent out by post. For security reasons we regret that passes cannot be distributed on the bus. Or: Alternatively tickets can be ordered by post. Please download the Season Ticket Leaflet from our website or collect one from our driver. Complete the form and send the required payment to our offices at South Mimms (full address below). Please make cheques payable to Sullivan Bus & Coach Ltd The process is similar to above, please complete the form giving all the details necessary. If you are unsure of the pickup/set down places, please consult the map attached or call us on Online Banking By Post We will print the ticket and post it to your home address. Please allow at least ten working days for orders by post.

Please call us on before making a personal visit as it can take some time to print off individual passes. Our location map is shown on page. The office is open during normal traffic hours (06:00 1:00 M F and 06:00 16:00 Saturday and at certain times on Sundays & Bank Holidays). However ticket printing can only take place between 09:00 & 16:00hrs M F. Therefore if you need to collect your ticket outside these times please give us a call first so we can pre print the pass to avoid a wasted journey. Counter Please note we do not need passport sized photographs. Pupils aged 11 and above should have a HCC Savercard. This will act as sufficient ID should we need to identify the holder. 3 Terms and condition for School season tickets 1. The ticket is not transferable and is only valid for use by the person named.. The ticket can be used on St Michael Schooldays only on services 1, &.. The ticket must be shown to the driver on every journey and must be produced on demand to an official of Sullivan Buses or St Michaels School. 4. If the ticket is not produced on any journey then the appropriate single fare is payable. No refunds will be made. 5. A ticket is not valid if it has been altered or defaced in any way. 6. The loss of a season ticket must be reported immediately in writing or by to Sullivan Buses. Only one duplicate ticket will be issued each term. A fee of 10 is payable for a replacement ticket. 7. Refunds will be considered for any unused or partially used season tickets. The amount of refund will be calculated from the date the ticket is received at Sullivan Buses office.. Season tickets are issued subject to Sullivan Buses Conditions of carriage. 9. Sullivan Buses will not accept liability for the consequences arising from (i)journeys that do not operate in accordance with the published timetable, (ii) the shortage of accommodation in any vehicle, (iii)detention; delay or non operation of a journey for whatever reason; (iv) failure to provide a relief vehicle 10. Sullivan Buses will consider any complaints about its services in a fair and objective manner and will respond to all complainants. 11. Sullivan Buses will work closely with St Michael s School in the operation of these services and will ensue the school is informed as soon as possible of any operational difficulties and told in advance of any proposed changes. 1. Parents and guardians will ensure the good behaviour of their child travelling to and from school and accept that travel facilities may be withdrawn (permanently in severe cases)for bad behaviour. 1. A season ticket may be forfeited and renewal refused if there is any breach of these conditions. 14. Sullivan Buses will not pass any information as a result of these transactions to a rd Party. 15) Pupils 11 and over must hold a Hertfordshire Saver Card to be eligible for half fares.
