2 Application 2 American Case Management Association Social Work Fellowship 2011 Fellowship Application About the Fellowship The ACMA Social Work Case Management Fellowship offers a premiere educational experience for social work professionals desiring to expand their understanding of hospital social work and case management practice. The fellowship is a nine-month hospital-based program that includes hospital case management training, project work, research, and formal presentation. Ultimately, fellowship recipients gain enhanced credibility and recognition in the practice of hospital social work case management. The Fellowship is governed and bestowed by the American Case Management Association (ACMA) in partnership with approved CSWE accredited Schools of Social Work. Governance is provided by an ACMA Fellowship Board comprised of Social Work/Case Management professionals and representative(s) from the partnering school of Social Work. This board will review and substantiate satisfactory completion of the fellowship. One individual will be selected for the 2011 Fellowship through an application process. The fellow will work under the supervision of a fellowship preceptor who is minimally a Masters in Social Work (MSW) and Director/Manager/Supervisor of Case Management in their organization. Upon successful completion of the fellowship, ACMA will bestow the designation of Social Work Case Management Fellow of the American Case Management Association. Partnering Schools of Social Work & Fellowship Site The New York University Silver School of Social Work has partnered with ACMA to oversee the evaluation and completion of the Fellowship. The 2011 Fellowship will be conducted at: New York University Langone Medical Center in New York, NY Preceptor: Brenda Ohta, PhD, MSW, MSG, CPHQ, Senior Director of Care Management Fellowship Preceptor will provide day-to-day supervision of the Fellow. All Fellowship sites are required to be a hospital/health system with a collaborative practice of case management service, and must comply with the ACMA definition of Case Management

3 Application 3 Application / Eligibility Requirements All applicants must meet the following criteria to be considered for the Fellowship: Applicant has earned a Masters in Social Work (MSW) degree from an accredited CSWE school of social work Applicant has completed a minimum 60 credit /2 year program Applicant has hospital / health system experience of: o Last year s field practicum or secondyear placement must have been in a hospital / health system, or o Candidate must have at least one year of prior hospital / health system experience (post-masters) Application Components 1. Completed application and essays 2. Three (3) letters of recommendation 3. Curriculum vitae 4. Copy of graduate degree or letter of attestation from the applicant s university indicating pending degree and expected date of completion 5. Phone interview (Required for the top two Fellowship finalists) Application Deadline Application deadline: March 25, 2011 Fellowship winner will be announced after April 9, 2011

4 Application 4 Application Evaluation Applications will be reviewed by the Fellowship Board (in blinded form). The Board will score applications on the following four equally weighed categories: 1. Application organized and complete, and applicant s attention to detail (CV, letters of reference, overall application) 2. Essay Question 1 3. Essay Question 2 4. Essay Question 3 Board scores will be used to determine the top five applicants, who will then be discussed by the Fellowship Board. Based on this discussion, the two finalists will be determined. These finalists will receive phone interviews with the Fellowship Board. Fellowship Curriculum & Requirements for Completion The Fellowship curriculum includes the following key elements: General Case Management orientation Observation of clinical core processes and key roles in the hospital / health system setting o Admissions o Health information o Patient care o Revenue cycle o Quality improvement o Medical staff o Ancillary services (i.e. Lab, Pharmacy, Respiratory, Dietetics, etc.) o Hospital administration o Medical Director for Case Management

5 Application 5 Fellowship Curriculum & Requirements for Completion (Continued) Case assessment training Acuity criteria training Clinical / medical terminology basic primer Communication skills, including crucial conversations, managing confrontation, and interpersonal communication Post-acute care resources Presentation skill development Regulatory requirements and processes Revenue cycle / reimbursement / denial management The Fellowship will also include the following elements/requirements: Completion of a project identified and assigned by Fellowship Preceptor (assigned during Month 4) Exposure to the Case Management service at two additional hospitals through one-week onsite visits (Month 6) Perform a SWOT analysis of the Case Management services at the Fellowship Site and the two additional hospitals visited. The written SWOT analysis will be submitted to the Preceptor, and subsequently presented to the Fellowship Board (Month 7) At the completion of the Fellowship, the Fellow will present their Fellowship experience and lessons learned at the ACMA Annual Meeting. Fellow will submit the abstract, learning objectives, and presentation slides for prior approval. Application Fees & Fellowship Stipend There is no fee to apply for the Fellowship. The Fellow will receive a stipend of $50,000 as their compensation during the fellowship. In addition, travel expenses related to site visits and presentation required by the Fellowship will be paid by ACMA. Fellow will also receive a complimentary one-year membership to ACMA.
