1 C ONFERENCE 2016 OSA Digital Workflow and Future Sleep Trends What does every patient, sleep doc, and dentist want to know? Who will be the best patient for an oral appliance and where do we have to position the jaw to keep the airway open? This talk will present early results from a clinical trial of a device to do just that. In the past year, our field has become even more important as a means of managing Obstructive Sleep Apnea - research is supporting our cause, and you ll hear how you can use this new information to support your practice. Steve Carstensen, DDS After graduating from Baylor College of Dentistry in 1983, Steve s been in private practice of general dentistry in Texas and home near Seattle, Washington. He treated his first sleep breathing patient in the first year of practice, but it was in 1998 that it became a passion to help people breathe easier throughout the night. In 1996 he was named a Fellow in the Academy of General Dentists, and is currently the Sleep Education Director for both The Pankey Institute and Spear Education. For the American Academy of Dental Sleep Medicine he s been a Board Member, Secretary Treasurer, and President-Elect. In 2006 he achieved Certification by the American Board of Dental Sleep Medicine. His most recent educational passion is as Editor-in-Chief of Dental Sleep Practice Magazine. Oral Appliances for OSA: Titration Protocol to TMD Concerns The relationship between oral appliances for sleep, the effects of TMD, and some of the predispositions and preventions of creating pain and problems with oral appliances will be discussed. Imaging ideas, as well as diagnostic protocols will be demonstrated, and specifically how to predict potential TMD problems. Learn about the relationship between oral appliances and TMJ. Medical Billing for Dental Sleep Medicine - Dentist Focused Rose will give an overview of what s really wanted and needed by medical insurance for OAT reimbursement. She will demonstrate specific processes and protocols for medical insurance reimbursement for oral appliances and tips to institute medical billing in a dental office. Identify medical billing processes for dental sleep medicine. Establish specific protocols for oral appliance therapy reimbursement. Determine what s wanted and needed for successful and accurate medical billing in a dental office. Rose Nierman, RDH Rose Nierman has published a manual with ICD 10 Diagnostic Codes for dentistry and online software, CrossCode for cross-coding from dental to medical. Rose has over 30 years of practical experience in both the clinical setting and practice management. Rose is the CEO of Nierman Practice Management and the author of numerous articles and manuals. Nierman Practice Management s flagship product, DentalWriter Software, a Narrative Report Writing and Medical Billing application also assists with medical insurance reimbursement. A nationally known speaker and recognized expert in the industry, Rose is a featured speaker for many dental organizations and currently sponsors seminars on medical billing 2

Her specialty in communications with insurance carriers, patients and other health care providers has helped thousands of dental practices with reimbursement and practice building for oral appliance therapy for Obstructive Sleep Apnea. OSA: Orthodontic Treatment Considerations and Case Studies Lecture will focus on post orthodontic patients that are to be retained with the MicrO 2 device at nighttime for treatment of Sleep Apnea, Snoring, and restricted airway. Dr. Warshawsky will present an evidence based argument that indicates that oral sleep devices that are built out of methyl methacrylate have a tendency to introduce minor tooth movement. He will present several clinical cases where Cone Beam Imagery will be presented prior to the MicrO 2 device and a minimum of 3-4 months after its initial introduction. Hypothesis is to show that the position of the teeth remained unchanged following the application of the MicrO 2 device. Identify clinical traits that are representative of individuals who are suspect to suffer from Sleep Apnea. Describe the logic to the favor of utilization of the MicrO 2 device for nighttime retention on Class II cases, and those who may suffer from Sleep Apnea. Clinically demonstrate the lack of change in dental position/movement as a result of wearing the MicrO 2 device nightly. Describe the process of prescribing the MicrO 2 device and how we titrate the individual to their own desired needs. Dr. Neil Warshawsky, DDS, MS Dr. Warshawsky is a born and bred true Chicago native. He attained all of his college training from the University of Illinois where he earned a total of five college degrees, culminating in his Orthodontic Certificate. Today he divides his time among the three pillars that support his practice. Dr. Neil leads a world-class staff at Get It Straight Orthodontics. With four locations spread across the city and suburbs, they are a very high energy, dynamic practice that concentrates on delivering customized orthodontic solutions in comfortable, inviting atmospheres. High-end cosmetic efficiency is the focus of this patient-driven practice. However, as you walk around the offices you will quickly notice that there is more to the practice than just a pretty space. When he is not in his private practice, Dr. Warshawsky is an Associate Professor of Orthodontics at the Craniofacial Center at the University of Illinois Health Center. With more than 18 years of cleft palate team experience and a vast knowledge of 3D imaging, he manages a large majority of the craniofacial cleft palate orthodontic cases. Dr. Warshawsky feels very fortunate to have the opportunity to practice orthodontics, and enjoys the ability to give back routinely at the University in a place that Makes a Difference where it matters most. Lastly, to stay current, Dr. Warshawsky may be found on the road some 40+ times per year lecturing to audiences literally around the world on a number of subjects including building clear aligners, lingual orthodontics, and the use of fixed skeletal anchorage for more efficient mechanics. 3

Outside of work, he is an Assistant Scoutmaster for Boy Scouts Troop 156 in Glenview, Illinois. When he is not working he may be found at home with his high school sweetheart or one of their three kids, or fishing at their family cabin in the Northern woods of Wisconsin. We re All Broken; Why Fatigue In Sleep Apnea Means More Than Excessive Daytime Sleepiness and What You Can Do About It Physiology is like gravity. Its presence is constant and vigilant yet it is rarely top of mind in dental treatment. Homeostasis dictates physiologic economy meaning that the body will not do anything it does not need to do yet everything it does need to do, to survive. Corruption of the human phenotype through processes developing from birth such as mouth breathing, poor oral posture and the increasing trend towards obesity have led to the epidemic of Sleep Disordered Breathing we encounter today. Unfortunately, these postural adaptations continue both day and night, causing muscle fatigue, cervical compensations and symptoms of TMD. Just as you would not put a broken arm in a cast without first correcting its alignment, so too the compensations occurring in the head, neck and airway deserve correction prior to putting the patient in a cast. The oral appliance is a cast and patients wear it when they are most vulnerable - lying down and asleep. OSA develops from birth. Postural compensations. Muscle physiology and fatigue. The role of muscle fatigue in OSA. Tools and techniques to optimize the head, neck, and jaw prior to taking the bite. How the physiologic bite position and the MicrO 2 can treat TMD rather than cause it. Anne-Maree Cole, B.D.Sc MScMed {Sleep Medicine} LVIM, MICCMO Anne-Maree Cole graduated from the University of Queensland, Australia with Honors in She has completed in excess of 27 separate programs at LVI including the full Core Curriculum, and has reviewed many of the programs multiple times for a total of 53 in all. She is a Master of the Institute, having received her LVIM in 2006 after submission of cases and examination. In 2007, she was awarded Alumni of the the Year. She is an instructor at LVI and has been appointed the Director of LVI Global Australia to teach the Core Curriculum within Australia. She has been awarded LVI s Most Educated Instructor and the International Leadership Award. Anne-Maree is currently Vice-President of the IACA and will be President for the year leading up to the Boston meeting in Over 20 years with her husband Andrew she grew a squat practice into a large multi-doctor facility that was recognised with 2 national small business awards. In addition, Anne- Maree has been a national finalist in the Telstra Business Women s awards. In 2006 she built the stunning Aesthetic Excellence facility with a view to practicing and teaching neuromuscular dentistry in a truly unique environment.
