2 INDO AMERICAN EYE CARE ORGANIZATION Indo American Eye care Organization (IAEO) is a charitable non-profit organization established to prevent curable blindness in India. It is registered in the state of Michigan, USA, and certified as a 501(c)(3) organization by IRS. IAEO is the major supporter of Pushpagiri Vitreo Retina Institute (PVRI), an exclusive super specialty eye care centre, located in Secunderabad, India. It is the first of its kind in the state of Andhra Pradesh, India, established in 2001, with a Vision 20/20 goal in providing services to the poor completed several tens of thousands of lasers and retinal surgeries. In close association with PVRI, IAEO is committed to providing total eye care to the poor and needy, irrespective of race, religion, and color in India and around the world. 4 FOREWARD Pushpagiri Vitreo Retinal Institute is making significant contributions in handling the various emerging challenges in eye care in clinics and as well as communities at national and international levels with the help of major supporter specially Indo American Eye Care Organization (IAEO) of USA and local philanthropists. IAEO, a core partner of PVRI and working towards reduction and prevention of avoidable blindness in India and a portion of amount received from IAEO is being spent in each charitable case treated by PVRI in the form of education, awareness, training and surgical interventions etc. Today, PVRI stands for quality medical treatment for challenging eye conditions and also offers comprehensive preventive measures besides creating awareness among common people. Today s Eye Care Hospitals are involved in research and development activities thus controlling the spread of the curse called Blindness in society. Recently published data Indicates that eight out of every hundred Indians are suffering with various levels of visual impairments includes blindness. It has been estimated that two out of every hundred Indians has been suspected with blinding condition and mostly located in rural and tribal areas. Globally 37 million people are blind. Keeping in view of the Burden of Blindness on the individual, family and society, PVRI started department of community ophthalmology consists of planned comprehensive community screening programs in addition to focused on school eye health programs. Serving fellow human beings and offering them the best possible eye care is our motto, and it is our vision to be one of the world s best eye care service provider with the state-of-the-art technology at affordable cost to people from all walks of life. Our pursuit towards excellence does not end here. With rapidly increasing service volumes and with a greater outreach and impact by the month, we are presently in the verge of a rapid expansion phase, with a vision to cover more than 70% of the population of Andhra Pradesh. We envisage this expansion to facilitate our goal to expand throughout South India. PVRI is also in the process of creating its academic wing in the area of Optometry, Post-graduation in Ophthalmology and Ophthalmic subspecialty fellowship programmes etc., with an array of experienced professionals and doctors engaged in training young ophthalmologists, optometrists and nurses in order to generate qualified and excellent personnel in every spear of eye care activities to help us carry on our crusade against blindness. We foster the aforementioned goals with great zeal and enthusiasm. Our leaps towards these goals bolstered with support and care from benefactors, gives us a feeling of satisfaction and the courage to carry on our activities. Dr. K. 5 OUR VISION & MISSION Our vision is to create cost-effective and quality services to all those in need. The mission is to be a center of quality in eye care services exclusively in retina related disease. Clinical research focus in eye ailments and different vision conditions, education and training with an aim on equitable and effective eye health services to non-reachable areas and under-privileged population in India and developing countries. Keeping in view of the changing organizational and environmental situations, we realize the need to assess our progress and make necessary changes in our system that is able to address the existing and new challenges in the field of eye care. Accordingly, the key issues which need further growth are as follows. Patient Care Services Hospital Management Development Community Eye Health Clinical Research Education Training In detail, substantial qualitative improvements in service delivery among all sectors that lead to patient satisfaction. Improvements in the quality of management in all levels that obtain better contribution from the employees and supporters. We will apply our expertise in primary eye health that directs stronger service delivery network across the country. We will focus to realize and address the key eye diseases and genetic conditions by applying new knowledge and research techniques. We will develop this institute as a premier center for fellows and mid level medical persons in clinical service deliveries. SERVICE APPROACHABILITY LEARNING PROFESSIONALISM After many brain storming sessions, we decided on some key goals for the next five years as follows: Cataloging as best Institute Premium Quality services Acceptable community eye care programs Publications in clinical and public health research Focus on Consultancy and Advocacy across the globe PVRI will extend its services to Rural Andhra Pradesh, as Kadapa facility representing the southern A.P., Vijayanagaram facility representing the eastern A.P., and Nizamabad facility representing the northern A.P... The secondary level eye care facility that is going to be established in the town of Kadapa will serve approximately 1 million people of Kadapa and peripheral districts. The alienation of land by the Government to the Institute is in process. A tertiary level eye care facility that is going to established at Vijayanagaram will serve approximately 5 million people of Vijayanagaram and peripheral districts. Similarly another eye care facility will be established in Nizamabad to serve the people in around that region. In each area, specific goals have been identified with a clear track to achieve them. 4

8 RESEARCH Diabetic Retinopathy and Role of Micronutrients: II. B-Vitamins and Minerals Diabetic retinopathy (DR) is one of the most common vascular complications of diabetes and leading cause of blindness. In the study we examined association between micronutrients (B vitamins and minerals) and type-2 diabetic retinopathy. One hundred and twenty subjects of type 2 diabetic mellitus patients with (65) were included in this case control study. Clinical evaluation of fundus was done by slit lamp and indirect ophthalmoscopy and fundus fluoresceine angiogram was recorded. Serum levels of minerals (Ca, Mg, Fe, Cu, Co, Zn, Mn and Se) were estimated by atomic absorption spectroscopy (flame and graphite furnace). Vitamins B1, B2, B6 and B12 were analyzed in erythrocyte lysates or in plasma by enzyme activation coefficient or by radioimmunoassay. In the present study, we observed no significant difference between normal controls and diabetic with and without retinopathy with respect to the levels of most of the micronutrients which have an important role in insulin resistance and sensitivity (Zn, Mg, Mn, and Cr), Among the B-vitamins, levels of B-12 and B-6 appear to be deficient in DR patients compared to normal and diabetic subjects. These results indicate an association between decreased levels of plasma vitamin B6 & B12 and diabetic retinopathy, which needs further evaluation in terms of exploring their role in diabetic retinopathy. These studies are also expected to provide the possibility for the nutritional therapy in diabetic retinopathy. Morphological Evaluation of Central Serous Chorioretinopathy Using Fourier Domain Optical Coherence Tomography To evaluate the retinal morphological alterations associated with fluorescein angiographic leakage in central serous chorioretinopathy (CSCR) using Fourier domain optical coherence tomography (FD-OCT) Consecutive patients with clinical findings suggestive of CSCR underwent baseline fundus fluorescein angiography (FFA) and FD-OCT (raster and B-scan) of the fovea and FFA leakage sites. FD-OCT was repeated at monthly follow-up visits. Twenty six eyes of 22 patients (four with bilateral involvement) were included in the study. FD-OCT showed a retinal pigment epithelial whereas (PED) corresponding to the FFA leak site in 18 (69%) eyes, whereas retinal pigment epithelial (RPE) irregularities and protrusions were noted in 8 (31%) eyes. Of these, four eyes with ill-defined angiographic leakage revealed discrete PEDs. Six eyes showed tethering of the outer retina at the site of a leaking PED. Granulated appearance of the outer retina was seen in 19 (73%) eyes of which 11 (58%) had subretinal precipitates and 8 (42%) had fibrin. Presence of either subretinal precipitates could not be correlated with the duration of detachment. Board area of RPE involvement adjacent to the leak site was seen in 11 (42%) eyes whereas RPE irregularities away from the leak site were noted in 14 (54%) eyes. Twelve (46%) eyes had single (n = 7) or multiple non- leaking PEDs, either in attached (n = 6) or detached retina (n = 7). In nine eyes having follow-up ranging upto 2 months, resolution of SRF was associated with collapse of PED (n = 3). Two eyes had persistence of PED and residual RPE irregularities (n = 4) and enlargement of PED (n = 3). Two eyes had persistence of PED with residual SRF. Fourier domain OCT helps in precisely delineating the retinal pigment epithelial abnormalities and outer retinal alterations associated with CSCR. A larger area of involvement detected in eyes with focal angiographic leakage supports the view of widespread RPE pathology in CSCR. 7
9 Training & Fellowship programs: ACCOMPLISHMENTS Two Fellowships programmes, one in comprehensive Ophthalmology and another in Vitreo Retinal Services, were started from 1st August This Fellowship programme is for the Post M.S./DNBE/D.O./FRCS (ophthal.) degree candidates. The fellowship programme will be for 15 months and aims at training young ophthalmologists to become super specialists in their respective fields. Surgical Training in Small Incision Cataract Surgery & Phacoemulsification in Liaison with the M/s. Appasamy Associates, Chennai, was initiated on September 2008 to update the surgical skills of practicing ophthalmologists in Andhra Pradesh and neighboring States. The training period will be for four weeks and aims exclusively at imparting surgical skills in Small Incision Cataract Surgery. We are in the process of starting a Paramedical Ophthalmic Assistant course in Ophthalmic Techniques to meet the demand of the same. Presentations and awards: The Pushpagiri Vitreo Retina Institute-NIN-ICMR Research study on Diabetic Retinopathy and Role of Micronutrient The initial data presented by Mr.N.Satyanarayana, Senior Research Fellow in the 17th Annual Meet of Indian Eye Research Group-2008 at Arvind Eye Hospital, Madhuri, on 26th and 27th July Dr.K.Viswanath, Chairman-(Medical) of our Institute, took over as President of Andhra Pradesh Ophthalmic Society, for the year on 27th September 2008 at the State Conference held at Vijayawada. The Consultants of Pushpagiri Vitreo Retina Institute are the resource persons for the lectures and demonstration in the Continuing Medical Education programme conducted by the Regional Institute of Ophthalmology-Sarojini Devi Eye Hospital, Hyderabad, Andhra Pradesh, from 18th to 31st August This programme is a popular educational programme for the resident postgraduate students in ophthalmology of all the Medical Colleges of Andhra Pradesh and a few from neighboring States like Karnataka, Maharastra and Madhya Pradesh. 8

The said 35 lacks was incurred for conducting regular community eye screening programs including developing, printing and disseminating IEC material and other related logistics. Subsidized expenditure A/c of Arogyasri: Arogyasri program is a public and private partnership initiative to eradicate avoidable blindness in the state of Andhra Pradesh. In this program Government insurance company will reimburse the expenditure for selected eye diseases. This reimbursement comes up to 40% of the total expenditure. Rest of the expenditure will be borne exclusively by IAEO/PVRI. For example cataracts, refractive errors and detailed camps expenditures like transport, human resources will not be coved under Arogyasri. 9
