10 Under the Protection of the Environment Operations (POEO) Act, notifications to the relevant authorities are to include the following information where known: - Time, date, nature, duration and location of the incident; - Location of the place where pollution is occurring or is likely to occur; - Nature, the estimated quantity or volume and the concentration of any pollutants involved, if known, the circumstances in which the incident occurred (including the cause of the incident, if known); and - The action taken or proposed to be taken to deal with the incident and any resulting pollution or threatened pollution, if known. If any of the information above is not known at the time of the initial notification but becomes known afterwards, that information must be notified immediately after it becomes known. Neighbours should be notified where there is a possibility that their health or property may be at risk from the incident. Notification should be made as soon as possible, and regular updates should be provided as additional information becomes available, and should include: - details of the nature of the incident; - actions being taken to address the incident; - whether there is any risk to health; and, if so, - how they might be exposed such that recommendations can be made as to how exposure can be minimised. In determining the extent of community notification for potential air emissions, the PEO should consider aspects such as the type of pollutant, prevailing winds, height and magnitude of an emission, as well as the location of any on-site fallout or off-site impacts, the likelihood of the pollutant reaching ground level, and possible impacts on sensitive receptors. 13 1.0 INTRODUCTION The purpose of this document is to form an agreement between Hydro Aluminium and Emergency Service providers, who may be called on to assist in times that may be less than ideal. With the information supplied in this document it is intended that a safe and efficient working relationship can be formalised. It has been prepared in accordance with the publication Hazardous Industry Planning Advisory Paper No. 1 - Industry Emergency Planning Guidelines, Department of Environment and Planning, Sydney, 1988, and the Environmental Guidelines: Preparation of Pollution Incident Response Management Plans, Environment Protection Authority, The Agreement sets out the onsite structure to apply in the event of an emergency and summarises the actions to be taken by people with specific emergency responsibilities A fully trained Plant Emergency Organisation (PEO) team is on site at all times. All initial emergency action is the responsibility of the PEO Team, with remaining Hydro staff, contractors and visitors being generally required only to evacuate when instructed to do so (after raising the alarm if necessary). Because of the size and complexity of the Kurri Kurri operations, specific emergency procedures have been laid down for each area of the Plant and are set out in the relevant Current Best Practices for use by the onsite PEO Team. The philosophy is for Hydro to have an emergency response capability for all emergencies that could conceivably happen onsite. It is not the function of this Agreement to indicate the likelihood of particular events occurring. The assistance of the external emergency services should be requested even if there is only a slight chance they will be required. Prompt mobilisation could save a life. This plan was amended in April 2012 to incorporate the requirements of the Protection of the Environment Legislation Amendment Act 2011 No 63 (POEO Amendment Act). This Plan was prepared in accordance with Section 153C of the Amendment Act. Under the POEO Amendment Act, holders of Environment Protection Licences are required to prepare Pollution Incident Response Management Plans (PIRMPs) for their licenced activities in accordance with requirements set out in Part 5.7A of the POEO Act for implementation by 29 August The plan is required have the following content: - the procedures to be followed by the licence holder or occupier of the premises in notifying a pollution incident to certain persons; - a detailed description of the action to be taken, immediately after a pollution incident, by the licence holder or occupier of the premises, to reduce or control any pollution; Page 13 June 2012 Revision 9

14 - the procedures to be followed for co-ordinating, with the authorities or persons that have been notified, any action taken in combating the pollution caused by the incident and the persons through whom all communications are to be made; and - any other matter required by the regulations, including: details regarding identified hazards at the site, and the likelihood and risk of the hazards; an inventory of pollutants on site; staff responsibilities; mechanisms for notifying residents in the vicinity of the premises; site maps; harm minimisation/mitigation measures; and details regarding how the plan will be made available to the public and authorised officers, as well as testing and review procedures. 15 2.0 DETAILS OF THE AGREEMENT This section provides a general description of the works and a framework for dealing with emergencies and incidents from alarm initiation to official enquires after an incident. 2.1 Scope Definition of Situations Covered An emergency is a situation: - which may not be contained immediately by the people on duty using the available resources - where injuries have been or could be incurred - where damage has occurred or property is placed in jeopardy, or - where the impact has the potential to result in serious environmental consequences. An emergency can be described as an abnormal or dangerous situation needing prompt action to control, correct and return to a safe condition. If there is any doubt, an event should be treated as an emergency. For example, all fires must be treated as emergencies. A pollution incident includes a leak, spill or escape of a substance, or circumstances in which this is likely to occur. The Aims of the Preparation of this Agreement To decrease the level of risk to life, property and the environment; and To control any incident and minimise its effects. 2.2 Purpose of Agreement To provide an overview of actions considered necessary to control and limit the effect of any emergency that may happen on the Kurri Kurri site or on neighbouring areas. To facilitate emergency response and to provide such assistance on the site as is appropriate to the occasion. To ensure communication of all vital information as soon as possible. To facilitate the reorganisation, clean up and reconstruction after an emergency so that normal operations can be resumed. To provide a basis for periodic review and update of emergency procedures for the Kurri Kurri Works. Page 15 June 2012 Revision 9

16 2.3 Recognition of Emergency Service Providers, which may be tasked to an emergency at our works site. NSW Fire Brigades NSW Police NSW Ambulance Service - Hunter Area Health Service State Emergency Service NSW Rural Fire service Volunteer Rescue association Fire Brigade: Hydro employees are only trained in first-aid fire fighting of small fires. Police: The control of external roadways, pedestrians and vehicle control as well as serious accident investigations on behalf of the coroner is the responsibility of the NSW Police Department. Ambulance: If any injuries occur (other than minor injuries that can be treated on site). Note: If several people have been injured or killed, immediate steps are to be taken by Senior Ambulance Personnel to implement the NSW Disaster Medical Plan. The PEO Leader should liaise with Ambulance Personnel to ensure the appropriate use of this facility. Police Emergency Operations Centre (EOC): May be utilised in the event of a major disaster. Local Hospitals are to be contacted if people are seriously injured and need hospital treatment. 2.4 The appropriate regulated emergency service provider will be contacted normally through a 000 call 2.5 Dangerous Goods - Products Stored or Used at Hydro Kurri Kurri Plant Drawing 0A (refer Section 5) shows the location of dangerous goods that are stored or used on the site in quantities that could conceivably be a subject of concern in an emergency. A Material Safety Data Sheet for each of the dangerous goods can be found in Appendix B of this Emergency Agreement. Material Safety Data Sheets for all materials on site are stored on the site computer system with access at any terminal on site. Page 16 June 2012 Revision 9

17 2.6 Emergency Services Assistance On discovering an emergency situation/pollution incident or a situation which is likely to give rise to an emergency/pollution incident, the PEO Leader or a nominated person acting on his behalf will AUTHORISE OR CONFIRM the need for external assistance. The call for emergency assistance may be via the Hydro communications centre (Security Gatehouse) or made direct from any onsite (PABX) telephone. NOTE: The following types of emergency are always to be treated as EXTERNAL ALERTS. If the Hydro PEO Leader cannot be contacted within two minutes of the first alarm, all incidents are to be treated as EXTERNAL ALERTS. Fire - after assessment by the onsite PEO. Any significant leakage of chlorine, LPG, aluminium fluoride or molten pitch. Any accident requiring transport or involving more than one person. Any instance of a person or persons being overcome by fumes or lack of air. Any pollution incident that causes or threatens material harm to the environment. (Material harm, as defined under Section 147 of the POEO Act 1997, includes actual or potential harm to the health or safety of human beings or to ecosystems that is not trivial or that results in actual or potential loss or property damage of an amount over $10,000.) 2.7 Emergency Response and Control Plant Emergency Organisation (PEO) Team The works operates as a continuous process 24 hours per day, 365 days per year. Hydro has set up its emergency response organisation as four identical PEO Teams, one for each shift. Because there will always be a full PEO team on site with members allocated duties by the shift PEO Leader, only the names of the eight (8) shift PEO Leaders will be shown in this Agreement. A list of personnel nominated to take charge in an emergency is contained in Table 2.1. This table must be updated whenever any change occurs to the emergency response team leaders due to transfer, long-term absence (more than one month), resignation or a change of duties, which may seriously inhibit the person's availability. Major duties to be carried out by individual PEO Team members are shown in Table 2.2. Where a duty has not been allocated to a specific person (e.g. Shift Ambulance/Security Officer), a PEO Team member from each shift must be nominated. The nature of duties required to be fulfilled will change depending on the nature and extent of an emergency. The PEO Leader will reallocate personnel as required by the circumstances. 19 2.7.3 Hydro Emergency Commander The PEO Leader has overall responsibility for emergency preparedness and emergency action. He must be free to command, and therefore it is not appropriate for him to be involved in detailed actions. We have nominated two (2) PEO leaders per shift each of equal standing; one leader will take a tactical role while the other will take an overall safety role. Given the complexity and size of our plant, it will be dependent on the area or type incident involved which leader may take which role. Persons designated, as PEO Leaders must have: Sufficient authority to take charge in any situation Site knowledge Current knowledge of materials on site Knowledge of processes used on site Adequate personal protection for all possible emergencies Knowledge of equipment and methods used in fire fighting, spill clean up and rescue. All PEO Team members wear red safety helmets whilst in the works area. The PEO Leader's helmet displays the words: "PEO LEADER". In the event of an Emergency 'PEO' Team Members wear red structural fire helmets and the PEO Leaders wear yellow structural fire helmets. 20 2.7.4 Emergency Response Command Structures during an External Alert If the emergency situation develops into an "EXTERNAL ALERT", hand over of emergency Co-ordination to the PUBLIC EMERGENCY SERVICE COMMANDER (Senior Fire Brigade, Ambulance or Police officer attending - see later notes) will occur. However, the Hydro PEO Leader or his delegate must act as an adviser/liaison officer to the PUBLIC EMERGENCY COMMANDER, especially with regard to site plant hazards and how best to minimise these during the emergency Communications Our site is covered by a 2-way radio network, with channel 3 being a priority channel for onsite emergency personnel. Our site also has reasonable GSM mobile phone coverage. Dependant on the emergency being reported, a nominated person will, escort the incoming service to the incident scene or give suitable directions to a communications command centre. Site 2 way radios are available pre tuned to channel 3 at the main security gate; these can be handed to the senior person of the first arriving external emergency service Other Members of the Hydro Plant Emergency Organisation Emergency Controller (PEO Leader)* - is to supervise site activities aimed at minimising the effects of an incident. First aid fire fighting, spill control and clean-up are examples of activities, which will require detailed supervision and co-ordination. He/she will nominate trained staff members who are to be directly involved in site emergency activities, and pass a list of their names to the Evacuation controller so that their whereabouts are known when the staff roll is called after evacuation. First Aid Controller (Ambulance/Security Officer)* - responsible for the care of injured persons until medical help or an ambulance arrives. Communications Controller (Ambulance/Security Officer /PEO member)* - to follow the direction of the PEO Leader in relaying messages to outside emergency organisations, neighbours and other members of the emergency response team. Must be familiar with all communications equipment likely to be used and the whereabouts of emergency contact telephone numbers. Evacuation Controller (PEO Leader) - to supervise orderly evacuation; conduct a roll call, and Co-ordinate search activity if any person is missing from the roll-call. Page 20 June 2012 Revision 9

21 Traffic Controller (Ambulance/Security Officer/PEO Team Member) - to take charge of the main entrance during an emergency and ensure that only identified emergency services personnel are allowed access to the site. The Traffic controller will also supervise removal of vehicles from the site if necessary to ensure that access by emergency vehicles is not hindered. Section Controllers (Site Emergency Wardens) - will supervise clearing of staff from their particular areas and safe evacuation to their evacuation assembly point or other area as required. Each Section Controller is to have a list of people in the Section at the time of the emergency for use in the roll call. *These team members plus the PEO Leader, Shift Electrician and Production Supervisors are issued with 2-way radios at the start of their shifts. NOTE: 2-way radios shall not be used within 30m of a suspected bomb Emergency Command Centre Dependent on the emergency location and type, conference room facilities in the front office, can be utilised as an emergency command centre, with external phone facilities. A command centre in this location could also be utilised as media centre Emergency Off Site Transport of Dangerous Goods All dangerous goods transported off site are accompanied by an M.S.D.S. in the case of emergencies these should be referred to. A copy of the M.S.D.S. is included in this agreement. On site contact is the PEO Leader and the Occupational Hygienist/Site Safety Coordinator Responding to Emergencies The main thrust of this Agreement is the protection of people from injury. Works staff shall only attempt protection of property where no risk of personal injury is involved. The Fire Brigade is expected to arrive on site within 8 minutes of the first alarm. Action to be taken by Hydro staff prior to arrival of the Fire Brigade is set out in detailed instructions and procedures for each section of the Plant. Page 21 June 2012 Revision 9

22 Control of Contractors on Site Work permit procedures are applied to all contractors. Trained Hydro staff supervises their activities whilst on site. Short-term visitors are accompanied all the time. All contractors are signed into the Works on arrival and out on departure by the Guardhouse. 2.8 Interaction with Emergency Services and Relationships to Existing Plans and Procedures As noted in Section 2.5.4, control over an incident is to be handed to the senior Public Emergency Service officer immediately upon his arrival. A site 2 way radio with channel 3 is to be handed to the first External Service Commander on arrival to site. This Hydro Aluminium Kurri Kurri Emergency Agreement is to supersede all previous Agreements for the works. It is to be read in conjunction with the following documents:. Hydro Aluminium Kurri Kurri Pty Ltd - Plant Emergency Organisation Training Manual.. Hydro Aluminium Kurri Kurri Pty Ltd - General Safety Rules, (Kurri Kurri Works).. Hydro Aluminium Kurri Kurri Pty Ltd - Safety and Emergency Signs by Works Area (see Section 3 for an example).. Hydro Aluminium Kurri Kurri Pty Ltd - Procedures for Emergencies on Plant. 2.9 Notification of Authorities and Neighbours The PEO Leader will instruct the Security Officer to contact the Public Emergency Services (Police, Fire Brigade and Ambulance) and neighbours as appropriate. For the purposes of section 149 of the POEO Act, a pollution incident that is required to be notified: - is to be notified verbally to each relevant authority, and - is to be followed by notification in writing within 7 days of the date on which the incident occurred. The written notification is to be undertaken by or under the direction of the Site Environmental Officer. In determining the extent of community notification for potential air emissions, the PEO Leader should consider aspects such as the type of pollutant, prevailing winds, height and magnitude of an emission, as well as the location of any on-site fallout or off-site impacts, the likelihood of the pollutant reaching ground level, and possible impacts on sensitive receptors. 23 Early warnings should be provided as soon as a potential threat to neighbouring properties is identified. This should be in the form of a telephone call, where possible, to provide sufficient timely detail to potentially affected persons. In cases where an actual immediate threat to neighbouring properties is identified, all means available should be used to ensure the affected parties are notified. This would be undertaken in a hierarchy of contact mechanisms, starting with telephone calls then door-knocking, followed by a letterbox drop about the threat. For more general or less acute threats, letterbox drops may be sufficient. Follow-up contact should be made as appropriate, providing information regarding on-going actions being undertaken, the nature of any on-going threat, and notification of resolution of the issue. In all instances, relevant details should also be posted on the Hydro website. Information provided to neighbours should include details of the nature of the incident; potential effects of the incident on their health or property; and instructions specific to the nature of the incident to minimise the risk of harm from the incident (e.g. closing windows/doors and staying inside in the event of an air release; avoiding the use of water sources potentially affected by a pollution discharge etc.). Communication mechanisms must be coordinated with emergency services personnel managing the incident response. Further information is provided in the External Alert Procedure and Contact Telephone Numbers (Page 8). Page 23 June 2012 Revision 9

24 If the Hydro PEO Leader cannot be contacted within two minutes of the first alarm, all incidents are to be treated as EXTERNAL ALERTS Internal Emergency Resources The following emergency resources are available at Hydro's Kurri Kurri Works. (a) Fire Fighting Water Supplies A 200mm branch main off the Hunter District Water board supply main to Kurri Kurri brings water to the site, where it is lifted by pumps (one duty and one standby) into a 491,000 litre vertical storage tank which supplies water for both plant and fire fighting purposes. In an emergency, all non-essential functions are closed down to conserve water for fire fighting purposes. The site fire truck (see below) can be used to boost water pressure in the event of a fire. (b) Fire Protection Systems and First Aid Fire Fighting Equipment Areas of the plant where significant fire risk occurs are provided with fixed systems, being either: Water deluge, sprinklers, CO2, Inergen or NAFS111, hydrants, hose reels, extinguishers or any combination of these as required. Summaries of protection provided in each area are contained in: "Hydro Aluminium Kurri Kurri Pty Ltd - Plant Emergency Organisation Training Manual". (c) Fire Teams The PEO Teams (one for each shift) are trained in all aspects of first aid fire fighting, location and use of equipment and emergency rescue and evacuation procedures. (d) Emergency Vehicles The following vehicles are available for use by the PEO Teams: - A fully equipped fire engine (ex NSW Fire Brigade Service). - A PEO 4WD Command First Response Vehicle. - A fully equipped ambulance. (e) First Aid Centre (Extension 263) with two First Aid rooms operates 24 hours a day, 7 days a week. Page 24 June 2012 Revision 9

25 (f) Trained First Aiders and Medical Staff Ambulance/Security Officer can be contacted on Extension 263 or 200. There are first aid officers on every shift. The Works Doctor is on site two days during the week. Emergency medical assistance is permanently available via the Kurri Kurri Hospital. Emergency resource staff are regularly trained; equipment is periodically tested in accordance with the relevant Australian Standards Emergency Communications Alarms There are twelve detection and alarm panels located around the works. A master panel located in the gatehouse has facilities for manual initiation of alarms in other areas. If a fire is detected, a pulsating alarm sounds in the affected area; a constant alarm sounds in all other areas of the Works. The PEO fire team for the affected area goes to their panel to determine the exact location of the fire. The Alarm System incorporates break-glass manual alarm stations. Upon automatic detection of a fire, or manual detection of fire, or other emergency situation and use of a break-glass alarm, or advice by telephone or 2-way radio to the gatehouse, audible and visual alarms in all locations will be activated; general location of the emergency (i.e. the section of the Plant) will be shown on all panels. Testing of the alarm system is carried out monthly by Fire Systems contractor Telephones All plant areas are linked by internal telephone. Critical Telephone stations have their own separate telephone link back to the gatehouse. Gatehouse emergency number is direct, uninterrupted emergency line. Telephone numbers of neighbouring properties which have telephone facilities are kept at the gatehouse and provided on page Way Radios The PEO Team members and other persons nominated in Section are issued with 2-way radios at the start of each shift. NOTE: Two-way radios shall not be used within 30m of a suspected bomb. 2-way radio channels are allocated as follows: Page 25 June 2012 Revision 9

26 Channel 3 - Channel 2 - Channel 1 - Channel 4 - Emergency Channel: Gatehouse Ambulance/Security Officer monitor this channel. All PEO members switch to this channel once an emergency has been established. Maintenance Personnel - includes PEO Leader Production Personnel, Certain Production area machines. Construction and/or commissioning In the case of Plant power loss, the radio system will function off battery backup on channels 1, 2 and 3 only. In addition to this, all Kenwood radios have a talk around function, which bypasses the Base Station in the event of failure and allows radio-to-radio communication. This is accessed by pressing the TA or button (the button above the 3 button). To activate, select the channel you require as talkaround, e.g. 3 for emergency button and then press the talkaround button. To deactivate press the talkaround button again. Gatehouse scanner must be switched from scan to Channel Public Address System 2.12 Evacuation The Carbon Plant and the three Potrooms have separate PA Systems. The order to evacuate all or part of the plant will be given by the PEO Leader. Upon the order to evacuate, all staff will assemble at their designated assembly points except members of the PEO Team and those given specific instructions otherwise by the PEO Leader. Evacuation of each section will be under the control of the Emergency Warden for that section, who will be advised by the PEO Leader of any change e.g. alternative assembly point to be used if chlorine is released and wind is blowing the gas towards the normal assembly point. Drawing 0A lists Emergency Wardens by area and shows location of designated evacuation assembly points Procedure for Terminating an Emergency In an emergency involving external emergency services, when the Public Emergency Service Commander's role is complete, he will hand back control to the PEO Leader. The PEO Leader will assess the situation and decide on any additional actions to be completed before declaring the termination of the emergency. Page 26 June 2012 Revision 9

27 The PEO Leader will arrange for clean up of any spill and safe disposal of any contaminated material resulting from the emergency Emergency Counselling The PEO Leader will offer counselling and if required contact HR department who will select the appropriate level of assistance Public Relations and Debriefing It is important that care be taken in the manner in which the public generally, and the press in particular, are dealt with. The Smelter Division Manager (or his nominee), is the nominated spokesman to handle the matter, other staff should not offer any comments on the incident. It is important that members of the PEO Team and any other staff keep the spokesman informed of any change in the situation. Name or names of persons involved in an accident are not to be released to the media until the next of kin have been informed. Special care must be taken to verify that the next of kin have, in fact, been informed before the press are advised. Almost invariably, the press will desire to gain access to the works area. This is usually undesirable and should be refused in a firm but tactful manner Statutory Investigation Following any emergency, a statutory investigation may be held. A coronial inquiry may also be held in the case of fire and will be held in the case of fatalities. The PEO Leader must make sure there will be no interference with physical evidence except that which is necessary to bring the emergency under control. Following the emergency, a clean up must not start before approval is received from investigating officers. A senior police officer will be appointed to take charge of all aspects of the follow-up of an emergency that may later result in a coronial inquiry. One of the main concerns for the investigation team will be preservation of evidence, which may be used in the inquiry. There must be co-operation from Hydro on this issue. 28 2.16 Training and Evaluation The PEO Leader and other members of the PEO Team will be trained in all aspects of the Emergency Services Cooperation Agreement. PEO members receive two (2) hours training per month. Subjects reviewed will include but not restricted to: 1. Fire Fighting hose drill 2. Compressed Air Breathing Apparatus 3. Hazardous Materials identification and control 4. Rescue from heights 5. Confined space emergency actions 5. Site Emergency vehicle equipment and familiarisation 6. Fire extinguishing mediums delivery and use 7. First Aid (basic) and Ambulance familiarisation 8. C.P.R. (basic) 9. Hose drill, or any plant emergency debrief 10. Fixed fire suppression systems Some employees are trained in first-aid and hold current first aid certificates. They will periodically be given refresher courses. A booklet "Plant Emergency Procedures" containing the following topics is to be kept near every phone on site: o Emergency Telephone Numbers o Fire o Evacuation o Bomb Threat o Emergency patient care 2.16 Review and Revision of Agreement The Agreement will be updated after any emergency occurs to include experience gained during that emergency. It will also be revised when there are significant changes to nominated staff, equipment or materials. The Plant Emergency Co-ordinator is responsible for initiating reviews of the Agreement and authorising any changes. Page 28 June 2012 Revision 9

29 INTERNAL PROCEDURES 3.0 EMERGENCY RESPONSE PROCEDURES PEO Leader The essential functions of the PEO Leader are set out below. Assume absolute responsibility for all activities on site from when an emergency alarm is initiated to when the state of emergency no longer exists or when control of the emergency is handed over to the appropriate Public Emergency Service Commander. Issue and follow up instructions to designated people to contact the emergency services, as appropriate.. Contact neighbours.. Direct emergency control and clean-up operations as appropriate.. Ensure that responsibility for site operations is formally handed back to operations staff at the appropriate point Emergency Procedures and Information for the Traffic Controller (Ambulance/Security Officer) On hearing the fire alarm: Assume traffic control duties. Prevent vehicles, other than those authorised, from entering the Works. Direct emergency response vehicles to the internal emergency resources and/or the emergency location. Ensure driveway and entry and exit gates are clear at all times. Obey the PEO Leader. Stop any person re-entering the Works who is not part of an emergency response unit Emergency Procedures and Information - Site Emergency Wardens On Hearing the Fire Alarm in their building, or if directed by PEO Leader or his delegate:- Evacuate all personnel - check all toilets. Exit only when the Section has been cleared. Conduct roll call in conjunction with the Evacuation Controller. Prevent re-entry to the Section (unless authorised by the PEO Leader). Obey the PEO Leader or Public Emergency Service Commander. Page 29 June 2012 Revision 9

30 3.1.5 Emergency Preparedness Site Instructions No vehicles are to be parked on site other than in designated parking areas. All internal and external access roads must be kept clear. A complete set of Material Safety Data Sheets for hazardous materials handled in bulk on the site must be kept at all times in the gatehouse. There must always be two clear paths to the site of any potential emergency for access by emergency vehicles Onsite PEO Procedures The response to the particular emergencies listed below, shall be as set out in the relevant PEO Current Best Practices Current best practices are internal documents for the use by Hydro employees acting as first Responders. The PEO team have Current Best Practices (CBP) covering the following topics Hazardous Material Incident Responding to Injury Chlorine Leak Responding to Alarms Responding to Fire Bomb Threat Responding to Major Earthquake Crisis Information The assistance of the NSW Ambulance Service, Fire Brigade, Police or Volunteer Rescue Association, should be requested even if there is only a slight chance they will be required. 36 5.0 MANAGEMENT 5.1 Management Structure and Responsibility The Environmental Manager is responsible for the implementation and maintenance of this plan. Hydro Aluminium Kurri Kurri staff general responsibilities are defined in Position Description documents, which are maintained by the human resources department. Specific environmental responsibilities are defined in Environmental Performance Standards, which are maintained by the Health, Safety and Environment Department. Performance Standards establish the specific information (who does what, when) for activities within Hydro management systems. All site personnel are responsible for environmental performance. All staff are made aware of their general environmental responsibilities through the site induction process, and are required to comply with responsibilities as defined in relevant Position Description and Performance Standards documents. 5.2 Keeping and Publication of this Plan A copy of this Plan is to be kept at the Hydro facility at Hart Road, Loxford. This Plan is to be published on the Hydro website within 14 days of finalisation (details of neighbours may be removed from the publically accessible version to protect privacy). A free copy must be made available to any person who requests it. The Plan is also to be provided to an authorised officer if requested. 5.3 Document Control Hydro Aluminium has a document control system which was implemented in conjunction with the Quality Management System. Standards are set out for: - Document layout and format; - Document labelling; - Issue numbers and dates / revision control; - Authority to approve; and - Distribution lists. All controlled documents issued to staff and contractors are recorded via the document transmittal system. The name and date that the document was issued is recorded for future reference. Documents located on the site intranet are generally considered controlled copies. Printed copies are uncontrolled and identified as such via the footer or header block. 5.4 Testing of this Plan This Plan is to be tested through assessment and review, followed by amendments to the Plan where necessary. Testing is to be conducted: - at least once every 12 months, and Page 36 June 2012 Revision 9

37 - if a pollution incident occurs at or from the premises within 1 month of the date on which the pollution incident commenced. The testing and assessment process is to include the following activities: - Auditing of the Plan to determine whether it addresses all the requirements of the POEO Amendment Act, including any revisions or amendments to that Act that may occur in the future; - Auditing of the Plan to determine that the information contained within it is correct and accurately reflects the relevant systems and procedures on site; - Testing of the Plan, which is to include desktop simulations and an incident drill to assess how staff respond to an incident and determine whether the Plan actions are followed and are effective. The testing also includes assessing the effectiveness of training. 5.5 Implementation of this Plan If a pollution incident occurs in the course of an activity so that material harm to the environment (within the meaning of section 147) is caused or threatened, this Plan must be immediately implemented in accordance with the procedures contained within this document. Material harm as defined under Section 147 of the POEO Act includes actual or potential harm to the health or safety of human beings or to ecosystems that is not trivial or that results in actual or potential loss or property damage of an amount over $10, Training All employees must undergo training about their responsibilities under this Emergency Services Cooperation Agreement (PIRM), including contractors and subcontractors. Such training should be incorporated into the site induction process. The training requirements for employees are summarised in the following sections. Details of this document should be included in all training areas where applicable Inductions Before being allowed to undertake work on-site, all staff and contractors are required to undergo a site induction. The induction requires all contractors to have read the General Site Safety Rules and pass a written examination. Included in the General Site Safety Rules is a section on the environment. This section includes environmental incident response. In addition to the site induction, all staff are required to complete all relevant Department inductions and training requirements as described in each Department Environmental Management Plan. These plans provide further detail regarding specific hazards and actions to be taken in the event of an incident, and should be updated to reflect the relevant details of this PIRMP Current Best Practices Current Best Practice (CBP) documents contain procedures for carrying out a particular task. CBPs that have environmental aspects contain an environmental section and loss control measures for instructing staff on how to manage potential environmental impacts. Page 37 June 2012 Revision 9

38 CBPs can be found in the CBP index on the Hydro intranet. These CBPs should be updated to include reference to this PIRMP, particularly details regarding incident notification procedures Operator Training The training module Environment, Occupational Health and Safety - Code: L2/L2a is administered to operators, and is required to achieve level 2 operator status. Other operator training modules also include environmental training elements. Elements of this PIRMP should be included in this training wherever applicable Contractors The OHS and environmental background of contractor companies is researched before they are allowed on site, and smelter Contract Supervisors are required to ensure contractors meet all appropriate site environmental requirements. Before being allowed to undertake work on-site, all contractors are required to complete the site induction, and all relevant Department inductions and training requirements as described in each Department Environmental Management Plan. All contractors are required to work under Hydro environmental controls and comply with Hydro incident reporting systems, including this PIRMP Training Records The Employee Information System (EIS) is an electronic filing system containing records of all types of training completed by individual employees, including records of CBP training, courses undertaken and other applicable training modules. The EIS is updated by Team Coordinators for all staff within their team, and initiates prompts for refresher training as required. The training records should include details of: - Who was trained; - Date training occurred; - The name of trainer; and - A general description of training content The EIS should be updated to include reference to this PIRMP where applicable. 5.8 Review This PIRMP is a continually revised document, which, under legislation, must be reviewed at least once every 12 months and within 1 month of any pollution incident. This document is also updated in response to changes in smelter operations. Key triggers for updates include: - Major plant or equipment changes; - Changes to legislation; - Changes to management staff, systems or processes; - Implementation of audit recommendations; and - Implementation of actions with environmental outcomes. The master copy is maintained by the Site Environmental Officer, who ensures that the current version is uploaded onto the site intranet. Any printed copies are uncontrolled.
