4 Group Products Building a great program starts with solid benefits. AlwaysCare is committed to providing our clients with the most sought-after ancillary benefits through flexible and affordable plans made simple. Dental Insurance All of our dental plans offer access to our national PPO network of more than 270,000 access points and our standard plan enhancements: Additional cleaning for pregnant women in their 2nd or 3rd trimesters Adjunctive, pre-diagnostic test for members at risk for oral cancer or suspicious lesions Carryover Benefit Members are rewarded with extra benefits for taking care of their teeth. Vision Insurance Vision plans include flexible allowances and co-pays with access to an extensive national provider network of independent practitioners and large national and regional retail chains. Vision care and materials may be received at the same or separate locations No restrictions on frames or labs Special discounts on optical materials and laser vision procedures (where permitted by state laws) Critical Illness Insurance Our Critical Illness plans provide lump-sum cash benefits with multiple benefit payouts and support services on diagnosis of some of the most common critical illnesses. Plus, we offer optional cancer and wellness benefits to enhance plans. 4
5 Accident Insurance AlwaysCare s Accident plans pay cash benefits directly to the member for more than 60 covered accidents. Our benefits are guaranteed without coordination of other benefits and include a standard Accidental Death benefit in all plans. Optional wellness benefits are also available. Life Insurance Our group life insurance includes Basic and Voluntary Life plans that offer flexible coverage options, guaranteed issue, and optional Accidental Death and Dismemberment (AD&D) benefits, all to help protect families and loved ones from financial distress after a loss. Disability Insurance AlwaysCare has excellent group/client service representation, always very responsive and highly professional. Insurance Agent/Broker AlwaysCare goes above and beyond. You show once again the true meaning of AlwaysCare. Group Administrator AlwaysCare s Short Term and Long Term Disability plans provide income protection for employees recovering from injury or sickness. Our plans include seamless claim administration when both STD and LTD are offered, return-to-work incentives, and a rehabilitation program for LTD that gives employees additional support for recovery and helps employers maintain productivity. 5
6 Individual & Worksite Products AlwaysCare Benefits and Starmount Life Insurance offer a comprehensive suite of individual and worksite insurance products to fit the needs and budgets of our clients. Individual products are available online or with enrollment support for worksite sales. We also offer combined billing when group and individual products are offered together. All individual and worksite products include our hearing savings plan and prescription discount card at no additional cost to policyholders. Individual Dental & Vision Plans Our ONEplus SM Individual Dental plans offer guaranteed coverage and a range of benefit options to fit any budget or lifestyle need. Individuals may also add a fully insured vision plan to any dental plan. Our All-Star Dental plans are federally Qualified Dental Plans, and are available in select states for both individuals and small groups. Individual Life Insurance Receive affordable lifetime protection with up to $75,000 in modified whole life insurance up to age 120 or term life to age 95. Our plans offer a living benefit and accumulate cash value. It s simple to apply with just a few health questions and typically no medical exams required! Jumbo Accidental Death Insurance Plans offer guaranteed coverage for two different levels of protection for fatal accidents, with no exam required. Benefits paid are based on the type of accident with a minimum of $100,000 for covered accidents, $200,000 for automobile accidents, and up to $1,000,000 for common carrier accidents. Plus, coverage is available for the entire family for only $2 more per month. 6
7 Accidental Death & Dismemberment AD&D Insurance is guaranteed issue with no health questions and no medical exams. Individual or family coverage is available up to $250,000 in benefits, and every plan includes dismemberment, commercial airline, seat belt, and anti-inflation benefits. Youth Life SM Insurance Parents and grandparents can guarantee their children are covered for life with our Modified Whole Life Insurance available for ages 2 months to 26 years. Plans start as low as $3.34 per month and convert to permanent whole life at age 26, providing coverage that is guaranteed for life as long as premium is paid.
