However, it is important you look on the back page as well and take care to fill in all the compartments. Practice Supervisor Because this course requires you to be assessed in practice, you will need to identify an appropriate Practice Supervisor. Please read the 'Information for Practice Supervisors' so that you can see what is required and then discuss this with someone in your clinical area that meets these requirements. You will need to ask them to complete, and sign, the 'Practice Supervisor Details' form and include it with your application. Reference With regard to a reference you will see that we provide a manager s statement of support form. This form is designed to save time in your clinical area and replaces the traditional written reference. You do not need to name an additional referee on the application form. Funding You need to fill in the form and tick one of the options regarding funding. At you will find further information about studying at our Faculty. 2 Graduate Certificate in Palliative Care Nursing (Application Code CPU) Key details This is a 60 CATS credit level 6 Graduate Certificate composed of three modules. It is for registered nurses who care for care for patients with palliative symptoms and at the end of their life.. The course is run over two semesters (two single modules in Semester 1 and one double module in Semester 2). It would be preferable to start in September for students who wish to take this course one module per semester over an 18 month period. The course uses a blended approach to facilitate learning, which includes some online learning in modules U47973 and U This course comprises of two single modules and one double module: Module U47973 End of Life Care Module U45572 Advanced Communication and Supportive Relationships Module U47971 Assessment and Symptom Management Successful completion of this course allows you to exit with a Graduate Certificate in Palliative Care Nursing which is 60 CATS Credits at Level 6. END OF LIFE CARE MODULE DESCRIPTION Module U CATS Credits at Level 6 Semester 1 This course includes 4 study days taught on a Friday. Teaching is delivered in Oxford.. This module explores the physical, social, psychological and spiritual support of individuals who are approaching the end of life. The principles involved in the assessment and management of symptoms are explored within the context of current policy for end of life care. The module also explores the practitioners role in supporting families and carers in preparation for, and after, death. The ethical issues underpinning decision-making in end of life are also debated. The module assessment comprises of: Completion of a reflective assignment and achievement of practice learning outcomes.

3 ADVANCED COMMUNICATION AND SUPPORTIVE RELATIONSHIPS Module U CATS Credits at Level 6 Semester 1 This course uses a blended learning approach to teaching and learning, which consists of 4 taught half days on a Wednesday and a one day workshop. Teaching is delivered in Oxford.. This single module explores the development of communication skills and supportive relationships, within the context of health and social care. It aims to build on existing skills and develop them within the student's area of practice. It explores the following themes: advanced communication within the context of health and social care, supportive relationships with clients and their families, counselling and other therapeutic approaches, self-awareness, recognising own need for support, and using supervision. The module assessment comprises of: A3000-word reflective essay and achievement of practice learning outcomes. ASSESSMENT AND SYMPTOM MANAGEMENT Module U CATS Credits at Level 6 Semester 2 This course includes 6 taught study days on Thursdays, 12 hours of online learning and a conference assessment day. Teaching is delivered in Oxford. This module provides students with the opportunity to develop skills in assessing and managing a range of symptoms experienced by patients with lifethreatening illness. The module aims to develop students ability to elicit patient concerns, utilise a range of approaches to assessment, negotiate a plan of care, and evaluate the effectiveness of interventions. Core symptoms covered in the module content are pain; fatigue; gastrointestinal symptoms; respiratory symptoms; psychological symptoms; agitation and terminal delirium; and managing symptoms at the end of life. The module assessment comprises of: A written assignment, online learning activities, achievement of practice learning outcomes and a student conference day presentation. assignment, online learning activities and achievement of practice learning outcomes.

4 Your Next Step Applicants who meet the following criteria are eligible to take this course: Be registered nurse practitioner with the Nursing Midwifery Council Have completed and signed Manager s Support Form to evidence support of the manager for your practice area Have a completed and signed practice supervisor form to evidence an agreed nominated supervisor who can directly supervise and assess your practice Be able to produce evidence of ability to study at the required level Be employed or have access to a health care setting that will enable achievement of the learning outcomes. 9 INFORMATION FOR PRACTICE SUPERVISORS Each student undertaking any programme, short course or module is required to have a practice supervisor to support their learning in practice and professional development. Supervisors to students undertaking the cancer and palliative care practice-based modules are central to the process of practice supervision. PRACTICE SUPERVISORS Practice supervisors are crucial to students' progress since most will be undertaking the programmes and courses from a distance. A student will be offered a place on Cancer Care and Palliative Care programmes subject to fulfilling this requirement - providing the name, work address and qualifications of their practice supervisor before entry (this should include information about any study days or courses the supervisor has undertaken on teaching/ practice supervision and facilitating). Ideally a student should select a person whom they trust since the issues that are likely to arise as a result of practice exploration may sometimes be painful and difficult to discuss. Skills and qualifications of a Practice Supervisor Practice Supervisors will usually be expected to demonstrate the following: Competence in the speciality in which the student is practising and particular interests in cancer and palliative care specialities. Academic ability appropriate to support students at the level at which they are studying. Practice supervisors will normally have experience and expertise in supporting students and be conversant with the requirements for the student's level of study. This will normally mean that practice supervisors will have studied at level 3 (degree level) themselves. Understanding of the learning, teaching and assessment processes which are used in the programmes. Supervisors are prepared for this role by undertaking a pre-course practice supervisor/supervisor day and will be supported throughout the programmes by the teaching team. Ideally supervisors will at minimum have attended yearly practice supervision updates and/or undertaken modules or courses about practice supervision/teaching in preparation for this role. Undertaking these days attracts a certificate of attendance and advice can be obtained about undertaking further facilitator, practice supervision and teaching qualifications. Ability to meet with you regularly and to be involved in the assessment of professional practice skills. You will need to be able to meet with your student on at least 3 or 4 occasions over the course of each semester. Your will need to understand your students role and be able to observe their practice on at least one occasion per semester. Support for supervisors Practice supervisors can expect to be supported by the course team through a preparation day which practice supervisors are strongly recommended to attend. These days are set up to give supervisors information about general practice supervision issues and specific information about the programme/course assessment strategies. These preparation days provide a focus in the semester and a supportive network for practice supervisors. Supervisors are given handbooks for all of the practice modules and encouraged to have on-going dialogue with lecturers where this would be useful. Supervisors will also be offered a chance to have an annual meeting with both the student and professional tutor to review the year and make plans for future study. Support for students This commences with a preparation for study day at the beginning of each student's programme or short course. During these preparation days the roles, responsibilities and expectations of students, practice supervisors and managers in relation to the course are explored. In addition, modules to be taught during that term are introduced and opportunities given to students to discuss issues associated with the learning approach adopted.

10 There is no doubt that being a practice supervisor to a student on this course is timeconsuming. On average practice supervisors spend about 6-8 hours a term supporting students. The majority of this support is focused through the assignments. We see this support as an interaction between the student and practice Supervisor in which critical questions raise debate about practice. Despite the time-consuming nature of these activities most students and practice supervisors find the experience rewarding and stimulating and the quality of the student's work is enhanced by this interaction. Supervisors are responsible for supervising the student's practice and verifying that their practice is of a sufficiently high standard commensurate with their level of study and practice.
