Most Public Aid and insurance plans pay for part or all of the cost of services Interpreter Policy: We can communicate in English, ASL, Signed English and Spanish Services Available: Individual, Family, Group and Couples Therapy, Clinical and Psychiatric Evaluations, Medication Monitoring, 24 hour TTY Crisis Line ( ), Consultation to schools and social service agencies, Community/Educational Programs, Intensive Case Management, Liaison Case Management (helping to integrate Chicago-Read patients into the community) -1-

Age Range Served: Children, adolescents and adults Eligibility: Individuals who are deaf, hard of hearing or have special communication needs and who are experiencing emotional or behavioral problems, mental illness, ADHD, autism, chemical dependency or developmental delays Intake Procedure: Contact Director of Admissions Fee Structure: Insurance, Medicaid, Medicare and self pay Interpreter Policy: All staff are fluent in sign language Services Available: Evaluation and individualized treatment plans which include individual, group and family therapy; behavior programs; activity, art and dance therapy; parent and educator/caregiver training; educational and day programming; chemical dependence evaluation and education Additional Information: This is a collaborative program between Alexian Brothers Behavioral Health Hospital and Mental Health and Deafness Resources, Inc. -2-
