6 University College of the North Immunization Requirements Students are expected to provide documentation of immunization or demonstrate immunity to the diseases identified in the Practice Requirements Record in order to ensure the protection of themselves and their patients. This guideline applies to all students who access patients in practice settings within the Regional Health Authorities accessed by UCN. All immunizations must be in accordance with the National Advisory Committee on Immunizations (NACI), Canadian Immunization Guide, Mantoux testing in accordance with mycobacterium tuberculosis pre-placement skin testing adapted from the Canadian Tuberculosis Standards, 7th edition (2013). If a person has a documented 2 step skin test, any subsequent testing only requires a one step. o o A chest x-ray does not replace the need for two-step Mantoux testing and will not be accepted. A chest x-ray is required only for those with a documented history of positive Mantoux (every two years) At least four weeks after completing the Hepatitis B or A/B series, students are required to have serology for Hepatitis B Surface Antigen through their physician or Primary Health Care in order to determine antibody response. If required, a copy of the laboratory results must be submitted to the appropriate RHA. The Practice Requirement-Immunizations Record must be signed by a physician or nurse. It is the responsibility of the student to ensure this record is current and up-to-date, and to inform the appropriate RHA of any updated immunizations/mantoux testing information. RHAs may require written documentation by the immunization provider of any immunization update. It is strongly recommended that applicants make an appointment with their physician or primary health care nurse as soon as possible after receiving the Practice Requirement Record. Immunization histories may be available from parent s records, hospital/physician visits, public health records or school records. Physicians or Primary Health may charge a fee for completing the immunization form and providing the required and recommended immunizations. Immunizations listed in these guidelines have been categorized as either required or recommended based on the effectiveness of the vaccine, the probability of exposure to/transmission of the disease, and the consequences of the disease in vulnerable patient groups. Immunization information mailed or delivered in person to the Clinical Placement Facilitator must indicate program of study and student number. The University College of the North is not responsible for contacting physician offices, labs, etc., in order to obtain immunization information. It is the sole responsibility of the student to submit all required immunization information to the Clinical Placement Facilitator on or before June 1st, along with any updated immunization information during the course of study. Questions about immunizations requirements can be directed to the Clinical Placement Facilitator (The Pas Campus)- (204) or toll free (866) ext P age
