2 History of Health Connect In 2005, The Children s Trust recognized that better lives for children and families requires better health for children, and better health for children requires improved access to health and mental health care, family and community supports, coordination of services and an emphasis on prevention. The Trust convened recommendations from state and county reports, best-practice models, national and local experts and the input from dozens of public meetings to create Health Connect. This bold new initiative consists of three components: Health Connect in the Community Health Connect in our Schools Health Connect in the Early Years 2

3 Overview of Health Connect in our Schools (HCioS) This model calls for public schools to have a health team of professionals including a nurse/nurse practitioner, a social worker and two health aides. Each team will serves two schools and offers basic health services. This is a strategic partnership between The Children s Trust, Miami-Dade Public Schools and the Miami-Dade County Health Department, to bring quality health services to every school in Miami- Dade County by

4 HCioS Goals & Benefits: Provide a coordinated level of school-based healthcare Provide primary health services for uninsured students Immediately respond to emergencies with first aid/cpr Expand health screenings & assessments with access to followup care Provide behavioral health services to identify and solve student issues Offer Nutrition and health education Increase access to primary-care physician (medical home) 4

5 HCN s HCioS Network The following subcontractors serve 72 schools and reach more than 80,000 students: 1. Citrus: 16 Schools 2. CHI: 16 Schools 3. HBB: 10 Schools 4. EOFHC: 18 Schools 5. ICFH (formerly CPC): 12 Schools 5

6 HCN s HCioS Status School started on August 14 th, 2006 with 9 sites operational By end of academic year 70 sites were operational! This summer more than 30 schools will have health teams on site! 6

9 Success Story An 8-year-old Hispanic male student was referred to the Health Suite by the classroom teacher, with the chief complaint of diffused abdominal pain, fever, malaise and nausea. The child had refused breakfast in the morning because of poor appetite. After carefully assessing the child, the Health Team RN found out that the pain was localized in the umbilical region. The Health Team RN suspected an acute appendicitis attack. The child s mother was contacted and upon her arrival, the Health Team RN strongly recommended the child be taken to the emergency room immediately given the suspected diagnosis. The Health Team RN s diagnosis was confirmed and the child was admitted and scheduled for surgery. The child successfully recovered. His mother was thankful and stated I would have given him Mylanta and could have made him feel worse. Citrus Health Team Charles Hadley Elementary 9

10 Lessons Learned Staff recruiting takes time, but when done right, makes all the difference! Relationships with Principals and school staff is key to success! Implementation takes time! Flexibility is essential! Clear lines of communication ensure program effectiveness and efficiency! Each school, community, students and parents are unique. 10
