The Reasons Slow and complex billing process Very low payments No fair share assurance for physicians The Result Hospital emergency rooms became the family doctor of the poor.

4 WHO WE ARE FROM PARTNERSHIP COMES PROGRESS Founded in 1988 to improve the health and quality of life of Solano communities by providing leadership, focusing resources and developing partnerships. Partners include: + Kaiser Permanente + United Way + NorthBay Healthcare + Solano Medical Society + Partnership Health Plan + Solano Office of Education + Sutter Health + Touro University + Solano County Health + Solano Community College and Human Services + Community Clinics

5 INCREASING ACCESS. IMPROVING HEALTH Solano Kids Insurance Program (SKIP) Transitional Care Program Oral Health Program Faith-Based Committee Covered California and other programs Solano Health Improvement Council

6 SOLANO KIDS INSURANCE PROGRAM Large number of uninsured children (25 percent, one of the highest in the state) SKIP was born Free or low cost health insurance for all Solano County children. Partnership with Solano County Schools Screening 51 of 51 elementary schools 8 middle schools 10 child care centers Community outreach Cost of $300,000, $1,200 per child

7 SOLANO KIDS INSURANCE PROGRAM Accomplishments in 2014 More than 3,000 children insured. All Solano public elementary schools are 100% insured All middle schools screened are 100% insured All child care centers are 100% insured Outreach to families three times a year to keep kids insured. 95 percent of Solano children are insured (one of the highest in nation)

8 COVERED CALIFORNIA AND MORE More than 100 community events to inform and educate Media outreach Assisted or signed up more than 3,000 new registrants Outreach to pregnant women to link mother and child to health coverage Preparing for October open enrollment Enrolled more than 5,000 Solano County children and adults in health coverage (all plans) in goal 6,000 children and adults

9 ELSA WIDENMANN SCHOOL-BASED CLINIC ORAL HEALTH PROGRAM Medical and dental clinic for the children of Vallejo Preventive and restorative services 700 children served in 2014 Program includes classroom based dental education Medical clinic provides preventive and treatment services. Funded by federal grant that ends in 2015.

10 TRANSITIONAL CARE PROGRAM Respite care for homeless patients post hospital discharge. Homes in Vallejo and Fairfield Provide medical care, mental health services, drug counseling, housing assistance and other services. 60 clients served annually, each site. 76% are linked to coverage, a primary care physician and housing goal is 98% in 2015

11 FAITH-BASED COMMITTEE Consortium of churches throughout Solano County Focused on improving the health of their congregations and communities Diabetes prevention and education Linkages to coverage and primary care physicians Food security Awarded $20,000 grant from Kaiser Permanente for 2015.
