2 1 The purpose of the Region VI Directory is to provide resources for agencies and individuals looking for assistance ranging from transportation to mental health resources. Many people over many years have worked on creating and updating this directory. Even while updating, services and information was changing. All attempts to keep this directory as up to date as possible are being taken. Health Tracks is a federally required program that provides screening, diagnosis and treatment services to help prevent health problems and reduce the impact of current health issues. 7 1 Community Action Region VI Community Action Region VI is a non-profit organization with years of experience in community service. Community Action Region VI provides services to low income, disabled, and elderly households in Barnes, Dickey, Foster, Griggs, LaMoure, Logan, McIntosh, Stutsman, and Wells counties. 32 1 SOCIAL SECURITY INFORMATION Jamestown Social Security Office nd Ave NE Jamestown ND Open hours are: Monday, Tuesday 9a-3p Wednesday 9a-12p Thursday 9a-3p Friday Closed Your Social Security number is used by the Social Security Administration to track what you earn while you are employed and to track your benefits once you begin receiving Social Security benefits. Your Social Security number is also used by many businesses and government agencies for record keeping purposes. If you need a Social Security number, lost your card or need to change your name on your current card, please call or visit a social security office. You will need to fill out a one-page application form and, depending on your situation, provide original or a certified copy of your birth certificate and a second form of identification. You must be a U.S. citizen or a qualified legal immigrant if you are applying for a new card. You will need a form of identification for a replacement card. Please contact a Social Security office for acceptable forms of identification. You will need a marriage certificate or divorce papers for a name change. For the nearest office please contact the Social Security Administration at or visit their website at SSI SSI pays monthly checks to people who are blind, elderly or disabled by either a mental or physical condition and who limited income and resources. Individuals who qualify may receive a set amount with couples receiving an extra amount per month. Persons applying for SSI must be low-income, have limited resources and meet the following criteria: They must be 65 years old or older or have a disability that is either physical or mental condition or combination of both, that is expected to last at least 12 months or longer or a terminal illness or be legally blind. Applicants must also have limited assets. For more information contact the Social Security Administration or your local Social Security Office. You must apply for SSI at a Social Security Office or online at OASDI Old Age Survivors and Disability Insurance or Social Security as it is better known. 37 1 Telephone Assistance Programs Qwest offers Telephone Assistance Programs for qualifying low-income customers. Lifeline Service Get a reduced rate on your monthly home phone bill. Go to to find out if you are eligible and how to apply. Tribal Lifeline Service If you live on a reservation or pueblo, you may qualify and pay as little as $1/month for your phone service. Go to to find out if you are eligible and how to apply. Link Up Assistance You may save up to 50% for installation charges and get a deferred payment schedule for any additional charges for installation. You automatically qualify for these savings if: 1. You are eligible for Lifeline or Tribal Lifeline support. AND 2. Your application for telephone assistance is received within 60 days of your phone service installation (one Link Up credit per address). Plan Details Benefits are available for one phone line per household. Telephone service must be for your primary residence. Credits cannot be applied to the cost of jacks or wiring. Security deposits may still be required. To avoid a service deposit payment at the time of installation, choose free Long Distance Restriction. If you live on a reservation, you may receive an additional $70 to cover excess construction charges.
