Please call and choose option 1 (check the status of an applications) for the status of an application. For your security, you will be required to enter your Social Security number to locate the application. Additional verification item(s) may be needed. What will be my credit limit? Your application will be reviewed by our credit department, and your credit limit will be determined after your application has been approved. Your credit limit is determined using a combination of the information that you provide in the application and information we receive from your credit bureau report. Will I receive an instant decision? Yes, in most cases you will receive a decision immediately. For applications that require additional review, you will receive a message instantly indicating additional review is

2 necessary and a final decision regarding your credit card application will arrive via the U.S. Mail within 7-10 business days. What will the instant credit limit be? Your temporary credit limit will be displayed after you have been approved for the card and it will be a portion of your approved credit limit. How long will it take to receive my card? If approved, you ll receive your Welcome Kit via the U.S. Mail within 7 10 business days, which will include your card. Where do I find privacy information? Learn more at Using the Virtual Card If I apply for a FlexPerks Visa card, do I need to get instantly approved to have access to the virtual card? Yes, you will only have access to the virtual card if you are instantly approved. If not instantly approved, you ll receive notification of the status of your application via the U.S. Mail within 7 10 business days. How do I access the virtual card? Instantly approved cardmembers will be prompted to set up the virtual card. Setup includes an opt-in to receive a text message via your phone. A one-time password will then be sent which you must use in order to set up a PIN to access the virtual card. To retrieve the virtual card, you will need to leave the U.S. Bank FlexPerks Go Mobile app, re-launch it and follow the prompts. The virtual card will then be displayed for your immediate use. Where can I use my virtual card? The virtual card can be used anywhere Visa is accepted online. Some in-store retailers may accept a virtual card, but acceptance is not guaranteed. Your virtual card will not be accepted for casino purchases or for cash advances at ATMs or for other advances as defined in the Cardmember Agreement. How do I use the virtual card? The virtual card will have all of the necessary information you need to complete a transaction, if the merchant accepts virtual cards. How long will I be able to access my virtual card? You can use your virtual card for 30 days from account opening or until you activate your FlexPerks card that arrives in the mail whichever occurs first.

3 Managing my FlexPerks Visa card Account How can I use the U.S. Bank FlexPerks Go Mobile app to manage my FlexPerks account? As a cardmember you can use the app to view your account summary, see recent transactions, and make a payment. You must be enrolled in Online Banking to access My FlexPerks Visa Account. Can I access my FlexPerks Visa card account online? Yes. You can access your card account by enrolling in U.S. Bank Online Banking. With just a few clicks, you'll be in the know about your balance, your account activity and more. You can also pay your statement online and access your account using your mobile phone. How do I enroll in Online Banking? Go to select "Full Site" at the left-hand bottom of the page. Under Login click Get Instant Access. Then simply follow the online instructions. How many of my FlexPerks transactions can I view? With the U.S. Bank FlexPerks Go Mobile app you can view a maximum of the last 25 transactions. Cardmembers will earn FlexPoints at a rate of two FlexPoints for every $1 in the one category in any given monthly billing cycle that has the highest total of net purchases charged to the account (the highest category ). In addition, FlexPerks Travel Rewards Visa Signature cardmembers will be awarded FlexPoints at the rate of two FlexPoints for every $1 in net purchases during the current month s billing cycle for any merchant location that classifies itself as having telecommunication services/products. The FlexPerks Select Rewards Card is also not eligible for the double FlexPoints on highest spend in each monthly billing cycle In all cases, U.S. Bank does not have the ability to control how a merchant chooses to classify their business and therefore reserves the right to determine which purchases qualify for additional FlexPoints. 4. Cardmembers will be awarded FlexPoints at the rate of two FlexPoints for every $1 in Net Purchases during the current month's billing cycle for any merchant location that classifies itself as having telecommunication services/products. U.S. Bank does not have the ability to control how a merchant chooses to classify their business and therefore reserves the right to determine which purchases qualify for additional FlexPoints. 5. FlexPerks Travel Rewards Visa Signature cardmembers will earn FlexPoints at a rate of three (3) FlexPoints per every $1 in Net Purchases and FlexPerks Select Rewards Visa cardmembers will earn FlexPoints at a rate of 1.5 FlexPoints per every $1 in Net Purchases during the current month's billing cycle for any merchant location that classifies itself as a Charitable and Social Service Organization. U.S Bank does not have the ability to control how a merchant chooses to classify their business and therefore reserves the right to determine which purchases qualify for additional FlexPoints. Bonus FlexPoints will be awarded within 60 days of donation. 6. You will have the ability to redeem FlexPoints for award travel on more than 150 airlines. FlexPerks Cardmembers may redeem if their accounts are open and in good standing under the terms of their Cardmember Agreement. All FlexPoints redemptions are final. FlexPoints are not transferable, are not redeemable for cash and cannot be used as payment for obligations owed to U.S. Bank or its affiliates. There are no refunds, exchanges, replacements or conversions for currency, credit, other points or rewards for no-shows or unused portions or rewards. Determination and payment of tax liability is the sole responsibility of the cardmember. We reserve the right to verify and adjust FlexPoints at any time before or after posting any redemption. Once you or we close your account for any reason, all accumulated FlexPoints may be forfeited. Cardholders must have a Frequent Flyer number with the airline if they wish to earn Frequent Flyer miles on their award ticket. Tickets are subject to the applicable airline rules and restrictions. Airlines may impose additional fees that are not included in your award ticket, including but not limited to fees for seating, luggage, and changing your ticket. For Internet reservations, if an airline does not offer e- ticketing or if you request a paper ticket, there will be a minimum shipping fee assessed of $29.95 per transaction. For phone reservations, a twenty dollar ($20) Travel Advisory Consultation fee will be charged per award ticket. Fee will appear as purchase made to Loyalty Headquarters on next billing statement. Cardmembers will also have the option to redeem 2,000 FlexPoints per award ticket to offset this $20 fee. If

6 paper tickets are requested when e-tickets are available, a minimum $50 per ticket service fee will apply. A minimum $15 shipping/handling fee will apply for all paper tickets. 7. FlexPerks Travel Rewards Visa Signature cardmembers may receive up to a $25 airline allowance with each redeemed airline award travel ticket. Cardmembers who redeem FlexPoints for award travel may be credited up to $25 on a future statement for qualified purchases made using their FlexPerks Travel Rewards Visa Signature card with the airline carrier providing award travel flight. Qualified purchases are any purchases made with your FlexPerks Travel Rewards Visa Signature card that post to your account under the airline carrier providing your award travel flight between the dates of your award travel flight. Cardmembers must call Cardmember Services at to request a statement credit within 90 days of a qualified purchase. Once the request is approved by Cardmember Services, the cardmember will receive a statement credit within 4 to 6 weeks for the amount of the qualified purchase, up to $25. U.S. Bank does not have the ability to control how a merchant chooses to classify their business and therefore reserves the right to determine which purchases qualify for allowance. You may redeem allowance for qualified purchases only if the outstanding balance of your account is below its revolve limit and if your account is open and in good standing under the terms of your Cardmember Agreement. Once you or we close your account for any reason, your allowance may be forfeited. 8. Cardholders are responsible for the cost of any goods or services purchased by the Visa Signature Concierge on cardholders behalf. 9. Up to 25% discount off each rental.
