Many servicemembers returning from Iraq and Afghanistan are going through an incredibly difficult time, as they are faced with physical, psychological, and emotional changes in their lives. Sitrin s post-traumatic growth program for servicemembers promotes resiliency and reintegration through a variety of daily treatment regimens. Additionally, Sitrin s experienced comprehensive care team uses a multi-disciplinary approach to meet the specific needs of servicemembers. Sitrin is located just 90 miles away from Fort Drum in New Hartford, NY. The convenient location allows family members to visit loved ones and/or participate in family reintegration therapy sessions as they progress through the program. 4 Who should participate in the program at Sitrin? Sitrin s program is specifically designed to treat individuals with: Post-Traumatic Stress Disorder Post-Acute Substance Abuse Orthopedic Diagnoses Traumatic Brain Injuries requiring minimal supervision Spinal Cord Injuries Multiple Trauma What does the daily treatment regimen include? Central and Peripheral Vestibular Disorders, such as Vertigo and Disequilibrium Note: Sitrin is not prepared to treat individuals with severe suicidal ideation, frequent aggressive outbursts, psychosis/schizophrenia, diagnoses that require a locked unit. Hospital and Community Patient Review Instrument (HC-PRI) and SCREEN is required for admission. The daily treatment regimen, which is structured to mirror the duty day of an active duty servicemember, will include a combination of: Individual Psychology Intervention (one-on-one sessions with an experienced psychologist) Group Therapy & Family Integration Therapy (led by trained therapists and/or a psychologist) Occupational Therapy (brain fitness, anger management, relaxation training, stress management, aquatic therapy, etc.) Physical Readiness Training (meets physical fitness standards focusing on strength and conditioning) Speech Pathology and Audiology (evaluation and treatment of cognitive communication skills, hearing evaluations and balance assessments, Tinnitus management, etc.) Planned Helpfulness (goaloriented projects tailored to specific interests of soldiers) Recreational Therapy (woodworking, fly-fishing, archery, equine-assisted therapy, etc.) Massage Therapy (promotes relaxation, stress and pain relief, etc.) Adaptive Sports (competitive and recreational sporting opportunities) 4

5 Who does the care team include? Sitrin uses a comprehensive care team, which may include a variety of disciplines depending upon the diagnosis. The team includes: Physicians and a full-time Physiatrist with extensive backgrounds in physical medicine and rehabilitation, cardiology, and internal medicine are on staff to meet your medical needs. Trained Rehabilitative Nurses and Certified Nurse Assistants provide and coordinate a plan of care, while also taking into consideration your need for physical rehabilitation. Case Managers coordinate care, participate in treatment and discharge planning, and help you understand your recovery program so you can achieve your goals. Physical Therapists help you achieve a range of recovery goals, including strength, balance, endurance, and mobility. Occupational Therapists maximize your independence in daily activities, enhance strength and coordination, and train individuals in the use of adaptive equipment if needed. Recreation Therapists offer a variety of programs, including kitchen safety skills, functional balance, horticulture therapy, and educational and leisure activities. Speech-Language Pathologists provide individualized programs to treat speech, language, memory, thought processes, swallowing, and other communicationrelated conditions. A Psychologist works with other team members to provide counseling and support whenever needed, and helps patients and families adjust to life-changing illnesses, injuries, or disabilities. A Registered Dietician plans therapeutic diets according to your specific needs and diagnoses. A Massage Therapist helps relieve stress and remove metabolic waste/toxins from the body. 6 Who will be the military support staff? Former Army Sergeant Jennifer Fitzgerald served in the Army for nine years, with two, oneyear tours in Iraq, where she received a Combat Action Badge. She currently works at Sitrin as a Military Consultant, helping military families work through issues brought on by post-traumatic stress. Rick Yarosh is a retired Army Sergeant who was injured in Iraq in He has personally participated in a variety of rehabilitation programs and can relate to injured servicemembers and the challenges they face as they recover. How will pain be managed? There are several types of medicines that are used to relieve pain, including Opioids, NASAIDS (non-steroidal antiinflammatory drugs), Aspirin, and Acetaminophen. Medicines are administered by mouth, in a pill or liquid form, or through an injection. Additionally, Sitrin offers a variety of pain management strategies through counseling, biofeedback, and massage therapy. Does Sitrin take TRICARE? Yes; Sitrin is an in-network TRICARE provider. 6

7 Does Sitrin offer any new technology or accommodations for individuals with special needs? Sitrin utilizes cutting-edge technology in various areas to assist people of different diagnoses: Optimus is an electronic medical records system, which allows Sitrin s staff to access your chart from anywhere in the building. This technology is used by the multi-disciplinary team to design a plan of care, develop interventions, and communicate with one another securely - all at the touch of a button. In two rooms of the unit, Sitrin has installed Environmental Control Units for use by individuals with neurological impairments. These assistive devices use infrared technology to allow individuals who cannot move their arms and legs the ability to control their own surroundings. With the use of the ECUs, individuals are able to control the door, lights, call system, phone, television, and window blinds. Electronic patient lift systems are available in several rooms on the unit for individuals who require extra assistance transferring in and out of bed and to the bathroom. An automated medication dispensing system, called Pyxis, allows for immediate dosing of up to 350 different medications. Pyxis gives nursing staff access to medication based upon your needs 24 hours a day. This immediate service eliminates the wait associated with obtaining medication from a pharmacy. 8 What is the setting like? Located in a serene setting on Tilden Avenue in New Hartford, Sitrin s inpatient medical rehabilitation unit was designed with patients and visitors in mind. Its comprehensive rehabilitative program is offered in a relaxing, hotel-like environment with several amenities, including: Large private rooms and bathrooms Flat screen televisions Miniature refrigerators A large spa room Beautiful sitting areas with fireplaces VIP suites A large dining area and café Wireless Internet A centrally-located patient/family lounge 8

9 Are meals provided? Yes; Sitrin s inpatient unit features a beautiful dining area with three balanced meals each day. All meals are planned by a registered dietician. If a family member or visitor would like to eat with the patient, he/she can purchase a meal ticket from the receptionist. For the convenience of visitors, the cafeteria located in the rehabilitation center is also available, featuring more food options. Are visitors allowed? Family members and friends are welcome to visit during visitation hours on weekends or on days of scheduled family reintegration therapy. Is a barber available? Yes; a barber will be available by appointment in order to maintain a military standard haircut. Additionally, the cafe located on the rehabilitation unit is open between lunch and dinner, and in the evening hours, featuring complimentary coffee and snacks for patients and families. Are there laundry services? Sitrin will launder clothing during the inpatient stay. All clothing should be properly marked with first and last names prior to admission. 10 What should participants of the military rehabilitation program bring with them? Below is a recommended list of items for participants to bring during their 6-8 week stay. All clothing should be labeled prior to admission. Laundry services will be provided. Five to seven days of comfortable clothing, to include socks and undergarments (no derogatory words, phrases, or images allowed on clothing) Three sets of Physical Training clothing (weather appropriate) Personal hygiene items (toothbrush, shower shoes, deodorant, shaving supplies) Jacket (weather appropriate) Sleepwear Eyeglasses or contacts, if applicable What outpatient services does Sitrin provide? Sitrin offers a wide-range of rehabilitation services, including: Physical Therapy Occupational Therapy Speech-Language Pathology Aquatic Therapy Pain Management Audiology Hand Therapy Fibromyalgia Therapy Lymphedema Treatment Vision Training Concussion Management Vestibular Rehabilitation Is smoking allowed? Sitrin s proposed expansion includes building a lodge/medical treatment center at Camp Sitrin, which offers a serene, relaxing setting to promote healing. The 142-acre property includes a 10-acre lake, pavilions, nature trails, sports fields, and handicapped-accessible docks. It is located just two miles from Sitrin s main campus.
