Results: Accessing the CHR and Referrals Management Network provides value in supporting improved patient care with the ability to now view community health data and have a closed-loop referral process.

Execute HIE-IAPD Contract with DHS-MQD by 6/30/ SOW 1 Hospital Interfaces: 1. build clinical data interfaces with remaining 4 hospital system s EHRs (Wahiawa, Kahuku, Kuakini and North Hawai`i) 2. allows for secure clinical data exchange between the hospital EHRs, HHIE, and MQD and supports MU attestation including transitions of care 3. build inbound interfaces to MQD 3. SOW 2 On-boarding of Medicaid Providers: 1. On-board Medicaid providers to connect to Hawai`i HIE s Health enet (CHR and Referral System) and subsequently successfully exchange data and uses Hawai`i HIE s services to meet Meaningful Use Criteria.
