3 2. Introduction It s been more than 30 years since Louis Brown was born on July 25th 1978 by Patrick Steptoe and Robert Edwards at Bourn Hall Clinic, United Kingdom. Nowadays almost 5 million babies all over the world have been successfully delivered by this technique. Improvement of knowledge and technologies brougth IVF as one of procedure which could solve the problems of many infertile couples. Instead of successful, so many issues raising in the field of IVF which was addressed to patient s safety, neonates outcome, ethico legal, and the cost of procedures. Continuing development in ovarian stimulation, laboratory procedures, cryopreservation, embryo transfer and method of embryo selection was done in term of solving these problems. Mild ovarian stimulation and in vitro maturation become promising and challenging whenever IVF facing ovarian hyperstimulation syndrome, poor responder patients, very expensive medication and ethical issue for embryo freezing. Every procedure in laboratory was done not only to get the best oocyte and the best sperm but also to get the best embryo which will be replaced into uterine cavity. So many parameter has been developed and could be done for both either invasive or non invasive assessment of oocytes, sperm and embryos. Cryopreservation of sperm, oocyte, embryo, ovarian tissue and testicular tissue has been developed with very promising result. Not only infertile couples, but also women and men who were suffering from non ovarian malignancy could have benefit on this development. Ovarian and testicular tissue bank is very promising and challenging in term of fertility preservation for women and men who were included into this group. The last challenging issue on IVF field is about the use of stem cells in reproduction. Embryonic stem cells, adult stem cells or induced pluripotency stem cells (ips) are very potential to solve our problem in premature ovarian insufficiency or azoospermia patients. Even so many ethical issues raising from this kind of treatment. Now it s the time for us working hard to answer all of the questions regarding safety and efficacy of IVF in solving infertility problem. As Patrick Steptoe said more than 30 years ago: It is the end of the beginning not the beginning of the end. 3
