2 A Message for Family and Caregivers: Most of us don t plan to take on the role of a caregiver. So when a family member finds themselves in need of assistance, the caregiver often finds themselves in a world they are not prepared for - legal issues, medical issues, financial issues and more. As the behavioral health provider for your family member, we have designed this resource guide as a way to provide you with local, state and national resources to assist you as you navigate the process of caring for someone. College Health IPA is here to assist you and your family member with their behavioral health needs. This guide offers additional resources for other types of services. 3 SECTION 1: CARE IN THE HOME It s tempting to believe that our loved one won t need assistance in their home, but for many supportive services are a way to extend the independence of a family member to live in their own home as long as possible. Meals and Nutrition Many churches, senior centers, community centers and day programs offer meals as a service to elders for no fee or a minimal fee. Skilled services refer to more intensive medical care provided by licensed health care professionals, such as nurses and therapists. Custodial care represents services that help with activities of daily living, such as dressing, cleaning and other services. 7 Medication & Pharmacy Assistance With escalating costs, the medication needs of elderly patients can have serious impact. With rising costs and limited incomes, many seniors find themselves making very difficult decisions. Pharmaceutical assistance options may include: public or private discount programs for low income individuals pharmaceutical companies have discount programs with and without eligibility requirements clinical trials - inquire with your family member s physician Resources for Assistance National Council on Aging has an online tool designed to assist individuals in identifying discount plans or free prescriptions based on question responses. ( Pharmaceutical Research and Manufacturers of America have a database of patient assistance programs through which manufacturers provide free medications to eligible patients ( National Association of Boards of Pharmacy has a list of reputable online drugstores which may offer discounts. 8 SECTION 2: TRANSPORTATION Although a loved one may be independent enough to continue to live alone, they are not always able to provide transportation when they need to leave the home - doctor appointments, grocery stores and other errands. There are a number of public and private groups that provide door-todoor transportation specifically for the disabled and elderly. 10 Section 3: Housing & Care Facilities At some point, it may not be feasible or desirable for your loved one to continue to live in their own home. Moving out of one s home and into a care facility is an enormous change in lifestyle and can cause great stress. Whenever possible, the decision should be discussed fully with the affected individual. Additionally, other family members, physician or other medical professionals and spiritual advisor may be of assistance in making sure the transition is as smooth and comfortable for all parties involved. There are expanding options available for individuals who either do not want to remain in their home or are not capable of safely caring for themselves in their home. Housing and Care Types Assisted living (Supportive Housing) - assisted living and other forms of supportive housing are specifically designed for those who need extra help in their day-to-day lives but who do not require 24 hour skilled nursing care found in traditional nursing homes. Often assisted living is located on the same campus (continuous care communities) with more intensive services so there is a seamless transition should a higher level of service be needed at a later date. Adult residential home - adult residential housing ( also known as shared or congregate housing) is often a group housing arrangement for independent seniors not requiring any medical professionals. An outside agency often arranges this type of housing. Skilled nursing home - skilled nursing facilities provide the highest level medical care that can not be managed safely in any other environment. Hospice - hospice provides special services and therapies so individuals who are terminally ill can remain at home or in a hospice facility. Hospice attempts to improve the quality of life for terminally ill patients. 9

13 SECTION 4: CAREGIVER RESOURCES Caregivers are reminded regularly that they must take care of themselves in order to be of use to the loved one in their life they care for. Caregivers are in a difficult role of taking on an additional responsibility when most already have a full plate - jobs, families, social lives. Support Options: Informal Care - caregivers should always solicit the help and assistance of friends, family, religious support, neighbors and any others who can share the responsibility. Support Groups - support groups can provide the caregiver with the opportunity to give and receive encouragement, understanding and support. It is also a great way to share ideas and exchange information on resources. Respite Care - respite care involves utilization of services to give the caregiver a break. Services such as day care, in-home care services or companions provide this necessary resource for all caregivers. A specialist will find out what kind of assistance you are looking for, and then, will direct you to agencies and programs that may be able to help. The Help Hotline service is free.
