Making a Doctor s Appointment by Phone Yang Chen is making an appointment to see her doctor. Listen to the conversation. Receptionist: Hello, City Health Center. Hello. I d like to make an appointment to see a doctor this week, please. Receptionist: I m sorry. We are fully booked this week. The earliest I can give you is next Thursday. That s fine. Receptionist: What is your last name? Chen. Receptionist: Chen? Is that C-H-E-N? Yes Receptionist: And your first name? Yang. Y-A-N-G. Receptionist: Your date of birth? May 13, 1958 Receptionist: Do you have insurance? I have GVC. Receptionist: How s 2:00 next Thursday? 2 o clock next Thursday. Okay. Thank you. Worksheet 3a-2
Stella: Stella: Stella: Hey Stella, what s up? Do you want to get together after class? I m sorry Dora, but I can t. I have a doctor s appointment at 3 o clock. Is something wrong? No, everything s fine. It s just time for my routine checkup. How about you? When was the last time you went to see a doctor? I don t need to see a doctor. I feel great! I haven t seen a doctor in ages. You should go for a routine physical exam even when you feel fine. You may have problems that you are not even aware of. Well, I don t like the idea of injections and blood. And I feel embarrassed undressing in front of people. Stella: I know what you mean, but we do need to take care of ourselves. Besides, you want to be sure that you re healthy, don t you? My doctor is really good. Here s her number. You re right. I ll call today to make an appointment. Worksheet 3b Dialogue
Dialogue 1: Arshia and Dr. Davis Dr. Davis: All right, we re all set. You can get dressed. Can I ask you a question before you go? Dr. Davis: Sure. Just to make sure I understand, what s my main problem? Dr. Davis: You have a stomach virus. It s nothing serious. And what do I need to do? Dr. Davis: Try to eat lightly for a few days, and follow a bland diet. Here is some information about the diet. Could you tell me why that is important? Dr. Davis: It will help you feel better. Dialogue 2: Muza and Dr. Davis So my main problem is that I have asthma. Is that right? Dr. Davis: Yes, you have asthma, and you are severely allergic to cats and shellfish. And what do I need to do? Dr. Davis: Don t eat any shellfish and stay away from cats. If you start to wheeze, use your inhaler. Right, I understand what I should do. But just to be sure, why is it important? Dr. Davis: Staying away from cats and not eating shellfish will help you avoid an asthma attack. Using an inhaler will help you breathe in an emergency. Thanks for taking the time to explain it. It s really clear now. Dr. Davis: You re welcome. I ll see you again in three weeks. Worksheet 4b Dialogue Practice
Donna: Hi, Lisa! How have you been? Lisa: I m fine Do I know you? Donna: I m Donna I used to live in your building, remember? Lisa: Donna? Oh, my I didn t recognize you! I haven t seen you in ages! You look absolutely wonderful! You ve lost some weight, huh? Donna: Yes, I have. Lisa: What did you do? Did you take some diet pills? Or just starve yourself? Donna: No, no! I ve just been exercising regularly, and I ve really improved my diet in the last 8 months. Lisa: I ll bet you ve had to give up a lot of your favorite foods. I remember how much you love ice cream. Donna: I still do but now I don t eat it everyday. I save it as a treat for special occasions. The key is portion control. I go easy on ice cream. I ve also cut down on red meat, sweets, fried foods and junk food. Lisa: That sounds boring. Donna: Not really! I ve discovered so many new things to eat. My diet now has much more variety, actually. I ve found so many new fruits and vegetables to choose from. I vary the foods I eat everyday. Lisa: And I can see the result. We need to get together so you can show me how you do it. Donna: I d love to! Here s my number. Lisa: Thanks, and here s mine. Talk to you soon. Worksheet 11c Dialogue
Mrs. Smith: Welcome to the Coast Company. I hope all of you will be happy working with our company. I d like to tell you about some of your benefits. As employees, you are eligible for health insurance. Only full-time employees qualify. We have two kinds of insurance. You can choose either GBC or Well Plan insurance. Listen to Mrs. Smith talking. Then, read the sentences below. Correct the mistakes you find. 1. Mrs. Smith is talking to her friend. 2. She is talking about getting a job. 3. GBC is the name of a bank. Listen to more of Mrs. Smith s conversation. Mrs. Smith: You can choose either GBC or Well Plan insurance. Both types of insurance have either family coverage or individual coverage. If you choose family coverage, you will have insurance for you, your spouse and your children. Individual is just for you. Of course, individual coverage is less expensive. GBC is an HMO. Employee: Excuse me, Mrs. Smith. What s an HMO? Mrs. Smith: An HMO is a private insurance plan that gives you a list of doctors. You must choose those doctors. If you go to another doctor, GBC will not pay for it. Employee 2: Is Well Plan an HMO? Mrs. Smith: No. For Well Plan, you can see any doctor. Employee 1: Is that the only difference? Mrs. Smith: No, there are some other differences. GBC covers prescription drugs, Well Plan does not. Employee 2: What about X-rays and blood tests? Mrs. Smith: Both plans cover X rays and blood tests. And both have a co-pay for doctor visits. Employee 2: What s a co-pay? Mrs. Smith: It s the amount you pay to visit the doctor. The copay for Well Plan is $45 per month. For GBC it is $30 per month. Here s some information about both plans. Read it over, and if you have questions, please call me. Employee 1: Thanks, Mrs. Smith Worksheet 15a-1 Mrs. Smith

Have you gone to the doctor? I can t go! It s too expensive and I don t have health insurance. Well, why don t you go to a clinic with a sliding scale? A sliding what? A sliding scale.
