2 Dorchester Substance Abuse Coalition Who we are The Dorchester Substance Abuse Coalition (DSAC) is a grassroots organization dedicated to reducing, preventing, and eliminating substance abuse among youth in our community. We bring members of the community together to address the risk factors leading to substance use and abuse, and educate parents and loved ones about prevention and treatment. Our members include youth, parents, organizations, businesses, and individuals from all sectors of the community, including the Mayor s office, the police department, the Boston Public Health Commission, local schools, and any other concerned citizens. Mission DSAC s mission is to engage all sectors of the community to prevent substance abuse, reduce harm, and promote healthy choices among Dorchester residents through a variety of prevention, education, advocacy, policy, intervention and treatment efforts. We seek to create a healthy environment for our young people to grow and flourish. Vision We envision a supportive, caring, and welcoming Greater Dorchester community where: All people feel they belong and have voices Communication, participation, respect and patience are valued All people feel pride, safe, and invested in their neighborhoods Families are empowered to develop their potential through existing and newly created opportunities A new destiny of hope is established for all residents of Dorchester History DSAC was formed in August 2006 in response to the growing problem of substance abuse among youth in Dorchester. In the wake of the deaths of two local teenagers, concerned parents and community members decided to take action to prevent such tragedies in the future. At their urging, the Neponset Health Center partnered with Caritas Carney Hospital, Health Resources in Action(formally the Medical Foundation), the Irish Immigration Center, the Dorchester Probation Court, Massachusetts Organization for Addiction Recovery, and Boston Police District C-11 to form DSAC. Service to the community DSAC mobilizes the community to eliminate substance use and abuse among youth. We focus on helping parents, family members, and other care-givers understand the harmful effects of substance use and abuse on youth, and what they can do to help prevent it. DSAC holds community and youth forums, sponsors positive youth activities, facilitates informational discussion and support groups, engages in policy advocacy, offers community resource development, and provides guidance and access to treatment programs. All of our prevention strategies rely on community participation, emphasizing parental and youth involvement, as well as collaboration with schools and youth-serving agencies. 1

A trusted choice for you and your family, has a staff made up of highly experienced providers, many of whom have trained at some of Boston s most respected medical facilities. We combine a full range of comprehensive services with access to a large network of provider organizations serving the Dorchester and South Shore communities to deliver a high standard of totally coordinated care that meets the needs of every generation. Additionally, our centralized community locations offer easy access from Boston and the South Shore, and both locations are easily accessible by public transportation. 4 Addiction What is addiction? Addiction is a chronic, but treatable, brain disorder. People who are addicted cannot control their need for alcohol or other drugs, even in the face of negative health, social, or legal consequences. This lack of control is the result of alcohol- or drug-induced changes in the brain. Those changes, in turn, cause behavior changes. Addiction grows more serious over time. The seriousness of someone s addiction can be measured by the amount, frequency, and context of a person s substance use. As their illness deepens, addicted people need more alcohol or other drugs; they may use more often, and use in situations they never imagined when they first began to drink or take drugs. The illness becomes harder to treat and the related health problems become worse and more noticeable. Symptoms of addiction include tolerance (development of resistance to the effects of alcohol or other drugs over time) and withdrawal (painful or unpleasant physical responses when the substance is withheld.) There are many warning signs that may signal that a person suffers from addiction, including but not limited to the following: They emphasize that they enjoy drinking or taking other drugs They may minimize the level of their problem They may try to hide the seriousness of their problem from those around them They may blame others for their use They may seek out desperate means to continue to feed their addiction such as stealing from those around them What is treatment? Most addiction treatments are designed to do more than simply reduce or remove alcohol or drug use; they focus on getting addicted people to change their lifestyle and even their core life values as a way of preventing relapse. Most addicted people must work hard to protect their recovery. They need to participate in therapy or support groups and call upon others, such as friends or family members, for support. The sooner a person with an addiction gets help, the better. If you re unclear about the seriousness of a drug or alcohol problem, or about what type of help might be needed, the best first step is to seek out the guidance of a health care provider. This might mean speaking with your primary care physician or seeking out other community service providers. Providers of addiction treatment span a particularly wide range of backgrounds, approaches, and methods. You are encouraged to consider the different types of care providers in relation to your problems. Here is some information that may help you find an approach that works for you. 3

5 Treatment programs Most rehabilitative care for addiction occurs in programs that are dedicated to a specific treatment specialty. There are two main programs: Inpatient Programs typically lasting about days. This is most appropriate for patients whose substance use or life situation has become unmanageable and interferes with their daily life. Outpatient Programs typically lasting days. Participation generally lasts from 2 8 hours per day for 2 5 days per week. This program is most suitable for individuals who have a reasonably supportive living situation. All programs whether inpatient or outpatient have group and individual counseling, and many programs now offer medications for addiction and psychiatric problems. Some programs offer the following as well: Social services to help people gain employment Legal assistance Improve parenting and marital relationships Assistance in obtaining drug-free housing In addition, effective programs have more structure in their curriculum, better trained and supervised therapists and counselors, individualized treatment planning, monitoring of alcohol and drug use during treatment, and some offer follow-up continuing care after discharge. Although beginning a treatment program is a stressful time for an addicted person and their family, it is an important moment to ask many questions. Try to learn as much as you can about the treatment philosophy and approach before entering a program. Here are some helpful questions you can ask and some things to think about any treatment program as you consider your options. What therapies are the counselors and therapists trained in? Is individual therapy available and how many sessions are offered? (Be wary of programs that only use group therapy) Is family involved in the treatment process and in what ways? How can the family will prepare for when the affected member returns home? Does thetreatment program involve monitoring of substance use throughout treatment? How can a providertell if a treatment is working and what do they do if it is not working? How many types of therapy, counseling, medications and other services are available for changing a treatment plan? 4

6 What continuing care options are available to patients and how long does continuing care continue? In what form is continuing care provided (home visits, telephone)? Are there any formal efforts to link patients with AA or other mutual help groups? How can the the patient be linked with their primary care physician to assure continuity of care? What is the cost of treatment? Can or will it be covered? Are there coverage limits or limitations as it applies to treatment? Payment for addiction treatment can be a complicated matter and is an honest concern as well. Treatment programs and practitioners get paid from three sources: Private health insurance Medicaid and Medicare (public insurances) Public funds that are a combination of federal and state government dollars (for patients that have neither public nor private insurance) Insist that the health plan pay for a full clinical assessment that identifies all of the physical and psychological problems that need to be addressed by treatment, as well as other services that are needed. Insist on knowing your privacy rights, how your health insurance decides what it will cover and who will work with you to coordinate care. Here are some key questions to navigating the complex maze of funding used to pay for private and public insurance coverage: Am I insured? What are my health insurance benefits? What are the deductibles, co-pays, annual and lifetime limits of my insurance benefits? What are the criteria the health insurance company will use to decide if it will allow admission to treatment and will pay for the treatment? If Medicaid is paying for my or my child s treatment services, what do I need to know about the treatment provider I want to use? Will someone coordinate the care that is being reimbursed? Who? Where are they located and how do I reach them? If I use my health insurance, who else will know about the problem? Will the record follow them? For how long?
