7 Curriculum Vitae The author of this thesis was born on the 7th of March 1976 at the Waterland Hospital, Purmerend, The Netherlands. In 1994 he graduated from high school (VWO) at the Aloysius College in The Hague and started Medical School at the Leiden University. During his medical training he followed additional lessons in philosophy, medical ethics, law and management sciences. In October 2001 he became affiliated to the Orthopaedic Department of the Leiden University Medical Center (LUMC), initially as a super-co, than as a clinical doctor, and finally as a research doctor. This Thesis about elbow prostheses was started in 2002 under the supervision of Prof. dr. P.M. Rozing. It resulted in publications and presentations at both national and international conferences. In January 2004 the author began his training with two years of General Surgery at the Reinier de Graaf Gasthuis, Delft, under the supervision of Dr. L.P.S. Stassen. In 2006 this training was continued at the HAGA Hospital, The Hague, at the department of Orthopaedic Surgery under the supervision of Dr. C.F.A. Bos. Between 2007 and 2009 he worked at the LUMC under the supervision of Prof. dr. R.G.H.H. Nelissen. His last year of training was completed at the HAGA Hospital again, under the supervision of Dr. C.F.A. Bos. In February 2010 he started as an Orthopaedic Surgeon at the HAGA hospital. In the Hague he is living together with Maria Dolores and they are expecting their first child. 171
