Commissioner nyc.gov/health Introduction This directory aims to provide New York City (NYC) hospitals, healthcare facilities, and NYC response agencies with a comprehensive resource of pediatric services offered by NYC Hospitals. The directory is divided into two sections. The first section focuses on the twenty-five NYC Hospitals with Pediatric Intensive Care Units (PICUs), providing detail on critical care services offered at these hospitals that includes information and instructions on admission and/or transfer of critically ill pediatric patients to hospitals with PICUs. The second section includes twelve NYC Hospitals that do not have PICUs, but do accept transfers of pediatric patients, either into the hospital s pediatric inpatient unit or a subspecialty unit. This guide is intended to provide information for daily referrals and inter-hospital transfers as well as to assist healthcare providers to care for pediatric patients during a disaster when their typical referral choices may not be available. Data for this project were collected by the New York City Department of Health and Mental Hygiene Healthcare Emergency Preparedness Program during as part of the Pediatric Resources and Emergency Preparedness Initiative (PREP-I) Project. This project was funded by the Department of Health and Human Services Office of the Assistant Secretary for Preparedness and Response Grant Number U3RHS The updated revision for June 2012 was completed by the New York City Pediatric Disaster Coalition staff funded by the Department of Health and Human Services Office of the Assistant Secretary for Preparedness and Response Grant Number 5 U3REP The DOHMH recognizes that the healthcare system is dynamic, undergoing frequent change. The information presented in this Directory will be updated on a yearly basis. 5 Definitions of Key Terms Interhospital Transfer: A transfer of a patient from one inpatient facility to another. May require transport to a diagnostic department, operating room or specialized care unit. Neonatal Intensive Care Unit (NICU): A high acuity inpatient unit with patients ranging from age 0 to 28 days at time of admission. Must be staffed 24 hours a day by a board certified/board eligible neonatologist either in-house or readily available by phone. Must have the ability to provide 24 hour cardiorespiratory monitoring and mechanical ventilation of neonatal patients within the neonatal intensive care unit. Neonatal Patient: A patient ranging in age from 0 to 28 days after birth. Pediatric Disaster Cart: A cart or other large container with supplies intended for use only during an emergency or disaster event. Such supplies must be specifically geared towards children in terms of their size and nature and may include medical supplies, documents for patient tracking or supplies intended for decontamination, contact isolation or airborne isolation purposes. Pediatric Emergency Department: A separate emergency department (ED) for pediatric patients ranging in age from 0 to at least 17 years old but not routinely more than 21 years at time of presentation. Pediatric Inpatient Unit: A physically separate inpatient unit for pediatric patients (including general medical/surgical beds) with patients ranging in age from 0 to at least 17 years but not routinely more than 21 years at time of admission. Each patient must be assigned to a board certified/board eligible pediatrician, family practice physician or pediatric subspecialist and must be covered 24 hours a day by one of the same who is either in-house or readily available by phone. Pediatric Intensive Care Unit (PICU): A high acuity inpatient unit with patients ranging in age from 0 to at least 17 years but not routinely more than 21 years at time of admission. Must be staffed 24 hours a day by a board certified pediatrician or board certified/board eligible pediatric intensivist either in-house or readily available by phone. Must have the ability to provide 24 hour cardiorespiratory monitoring and mechanical ventilation of pediatric patients within the pediatric intensive care unit. Should regularly accept transfers of patients from outside facilities into the unit. Pediatric Patient: A patient ranging in age from 0 days to at least 17 years but not routinely more than 21 years at time of admission. Pediatric Transport Team: A team of health care workers with some additional pediatric training consisting of a minimum of trained Emergency Medical Service (EMS) workers who travel with, stabilize, and manage pediatric patients during inter-hospital transfer. The team may also include health care workers with more extensive pediatric training (pediatric attending, pediatric intensive care attending, residents, nurse practitioners, nurses with various levels of training who are experienced in caring for pediatric patients, physician assistants). Must be under the medical direction of a pediatric provider who directly (by being physically present) or indirectly (by phone or by protocols communicated to the team before leaving for transport) oversees the transport. Must be subject to periodic quality assurance. Staffed Beds: The number of beds that a hospital staffs and utilizes for patients. Transport Team: A team of health care workers that stabilizes, manages, and travels with patients during inter-hospital transfer. The team may or may not be dedicated to a particular hospital or a particular inpatient unit within a hospital. Well Baby Nursery: An inpatient unit consisting of newborns that require only routine infant care, with the exception of phototherapy for hyperbilirubinemia, and that is utilized only post-delivery or upon transfer from the NICU.
