15 FDA SOLICITATION The objective of this requirement is to provide FDA with the resources needed to use social media to inform and evaluate FDA risk communications. Specifically, the objective is to provide FDA with: Analyses of social media that provide baselines on consumer sentiment prior to FDA communication and that depict changes in social media buzz following FDA communications In-house capability for social media monitoring; and Surveillance through social media listening for early detection of adverse events and food-borne illness. 23 MENTAL HEALTH CARE THE CHALLENGE OF IMPROVING SUICIDE RISK ASSESSMENT Suicide risk assessment in mental health usually relies on data from the patient to identify those at low, moderate or high risk. Most people who die by suicide are low risk and most high risk people do not kill themselves. It would be valuable to examine the notes made at the time of assessment and subsequent care, yet this is time consuming and is often done only during formal inquiries when a suicide has already occurred.

25 TEXT ANALYTICS IMPROVING OUTCOMES A study of over 300,000 free-text machine-readable documents in the Stanford Health Care EHR finds that text-mining tools can be used to detect unplanned care episodes documented in clinician notes or in coded encounter data. Researchers believe their methods could be used for quality improvement efforts in which events of interest occur outside of a network that allows for patient data sharing.

26 OPTIMIZATION THE HOSPITAL GAME Optimizing Surgery Schedules to Save Resources, and to Save Lives Three key steps to a successful Operations Research Optimization Problem Identify real-world challenge as relevant to mathematical optimization formulation Does one understand the real-life situation to allow for a close-enough mathematical formulation, such that the solution will be relevant to real world decision making. Translate a real-life problem formulation into mathematical language Develop preliminary mathematical statement, including control variables, objective and the constraints. Formulate complete, in-depth understanding of all known influencing factors and outcome expectations. 27 SOLVER MIXED INTEGER PROGRAMMING (MILP) PROC OPTMODEL offers the following types of solvers for different types of mathematical programming problems: Linear programming (LP) solver Mixed integer linear programming (MILP) solver Nonlinear programming solver Quadratic solver Integer programming solver The Master Surgical Schedule problem falls into the mixed integer programming problem category. The solver implements a LP-based branch-and-bound algorithm. Branch-and-bound strategy divides the feasible region in to disjoint sub-problems. The solver then incorporates pre-solving, cutting planes, and additional heuristics to increase the efficiency in finding an optimum.

28 RESULTS LENGTH OF STAY Figures represent length of stay in the nursing wards for a schedule that was generated for a single operating room in a two-week period This simulation assumes that the nursing wards were empty on the first day and gradually fill up. Optimized schedule creates a much more steady buildup in occupancy, demonstrated on day 2. At end of end of the second week, there are numerous discharges in both plans. Buildup is more stable as related to manual scheduling.

30 SUMMARY THE HOSPITAL GAME Surgical procedures and scheduling are highly variable between hospitals Successful implementation of an optimization is related to how well business requirements are formulated. Implementation must strike a balance between approximate and exact implementations. Too many business restrictions can result in an unstable solution, whereas Too few can result in an irrelevant solution that does not work. Any analyst who wants to develop a schedule optimizer for a master surgical schedule can follow the steps in this paper. Important benefits of an optimized master surgical schedule include; Data driven understanding of the maximum number of resources that are required downstream for post-operative nursing care. Achieving a more steady utilization of available resources.

37 THE ANSWER TO THE BIG QUESTION Contrary to the current belief that reduction of ALOS implies a reduction in hospital resource utilization due to improved care, the exact opposite appears to be true. Hospital administrators should seriously consider high fidelity modeling before initiating a one size fits all approach to cost containment strategies.
