3 How Does The Employee Referral Program Work? The Employee Referral Program provides employees and their families with opportunities to obtain assistance for a variety of personal problems which may affect their continued functioning as productive members West Virginia State Government or society as a whole. Confidential referrals are made for treatment with independent, off-site, professionals who are experienced in a wide range of issues including relationship, conflict, family concerns, and alcohol or drug abuse that can have a negative effect on the employee s personal and work lives. This Employee Referral Program Directory provides information regarding services for individuals who have substance-related problems. The directory is published as a service to State employees and is intended to provide contact information for substance abuse treatment programs located throughout the State. Although many service providers are represented here, the list is not intended to be allinclusive. In many communities, individual professionals such as licensed psychologists, licensed social workers, and licensed professional counselors also provide services for chemicallydependent/abusing individuals and their families. Some hospitals also provide inpatient substance abuse services as part of their psychiatric or general medical services. 4 What Can I Expect From This Program? Comprehensive Community Behavioral Health Centers Adult Residential Programs serve males and females over the age of 18. These programs provide individual and group counseling, support groups, and other services within an intensive 24-hour therapeutic environment. Contact the individual programs for information concerning admission procedures and payment requirements. Adult Transitional Living Services provide a supportive living environment for adults who are in the process of recovery from addiction and who have generally completed a more intensive treatment program (usually residential). Specialized Services for Women include a variety of programming directed to the specific needs of women who abuse alcohol or other drugs. These services include outreach services, outpatient treatment services, extended residential treatment/transitional living services, and residential treatment services which make it possible for women to continue residing with their children while receiving treatment. Adolescent Treatment Services include outpatient, intensive outpatient, and residential treatment services tailored to the specific treatment needs of adolescent substance abusers. 5 What Is My Insurance Coverage? What costs will my health benefits cover? Your benefits are dependent upon the plan you ve joined. Managed care plans cover the same types of services as described for the Public Employees Insurance Agency (PEIA) Plan. Mental Health and Chemical Dependency Benefits: PEIA PLAN Outpatient Services PEIA provides coverage for up to visits per calendar year of outpatient mental health and chemical dependency services, including evaluations, referral services, diagnostics, crisis intervention, and therapeutic services. These visits are covered at 80% after the annual deductible is met. Additional visits beyond the visit limit must be pre-certified. The phone number for pre-certifications is printed on the back of your medical ID card. Inpatient or Partial Hospitalization Services PEIA provides coverage for both inpatient and partial hospitalization treatments. These services are covered at 80% after the annual deductible is met. PEIA reimburses up to the maximum allowable fee for the semi-private room rate for inpatient care. All inpatient or partial hospitalization services must be pre-certified Again, the phone number for pre-certifications is printed on the back of your medical ID card. MANAGED CARE PLANS Each managed care plan has its own referral procedures for using mental health and chemical dependency benefits. Co-payments also vary from plan to plan. Be sure to read your benefit booklet or call your plan for instructions before getting services. Page 5

Eastridge also provides education and treatment for Driving Under the Influence (DUI) offenders under contract with the West Virginia Department of Motor Vehicles. Persons who have lost their driving privileges through DUI charges receive a substance abuse evaluation, 18 hours of education and therapy services as needed. 17 FMRS The MOTHER Program A Women s Specialized Residential Program (Long-Term) 101 South Eisenhower Drive Beckley, WV Phone: (304) or after 4:30 pm. Website: The MOTHER Program arranges intensive outpatient or residential services for women ages 18 and older who are experiencing losses (such as their job, custody of their children, finances, friends, home) due to their use of alcohol or other drugs. The individual may have tried to quit on her own or may have tried outpatient services with little success. The MOTHER Program is a six-month program divided into three phases - Early Recovery, Advanced Recovery and Extended Recovery. Treatment services include psychiatric services and medication management and counseling for co-occurring disorders, group counseling and case management. Initially women participate five days a week, then three days a week, and finally one evening a week. Women who participate in the Residential Program reside in housing provided through a cooperative agreement with Pinehaven Homeless and Rehabilitative Services. Detoxification services are provided for or arranged as needed. Priority is given first to pregnant women, women who are using IV drugs, and women with children. Childcare is arranged, and parenting groups are provided for women with children. FMRS The LEARN Program (Men s Addiction Treatment Services) 101 South Eisenhower Drive Beckley, WV Phone: (304) or after 4:30 pm. Website: The LEARN Program designs intensive outpatient services for men, ages 18 and older who are experiencing losses (such as their job, family, friends, health or finances) due to their use of alcohol or other drugs of abuse. The individual may have tried to cut back or quit on his own or may have tried outpatient services with little success. Treatment services are held Monday through Friday for five weeks and include psychiatric services (as needed), medication management and counseling for cooccurring disorders, group counseling and case management. Individuals who utilize intensive outpatient services need to have a strong support system in place to assist them in reaching their treatment goals. The LEARN Program also recommends residential services for men who are addicted to alcohol or other drugs of abuse who have a weak support system in place, and who have demonstrated the need for a structured therapeutic environment along with intensive treatment services. Treatment services, which mirror the services available in the Intensive Outpatient Program, are provided Monday through Friday for five weeks, and residential services are provided through a cooperative agreement with Pinehaven Homeless and Rehabilitative Services. Detoxification services are arranged for or provided as needed. Page 17

Good Recovery Center is a residential substance abuse treatment facility located in Terra Alta, West Virginia, comprised of a Detoxification Unit and a Recovery Community Program. The Detoxification Unit is a residential 10-day detoxification program for individuals with a chemical dependency. There are 20 available beds 10 voluntary and 10 court-ordered or involuntary. Medically monitored detoxification services are provided through a 24-hour available nursing staff and daily appointments with a physician. Treatment services are primarily supportive and group-based. Individuals will participate in group therapy, individual therapy, and a 12-step recovery program. The Recovery Community is an 18-bed, long-term residential program following a community-based peerdriven philosophy. Essentially, the residents of this week program adopt a therapeutic community concept where they rely mostly on themselves and their peers during their recovery. This method teaches self-reliance and positive social skills by reducing dependency on staff. The Recovery Community promotes personal growth and upward mobility through a three-stage process with increasing privileges accompanied by increasing responsibility and expectations. Page 18

LEGENDS offers structured, therapeutic activities in group and individual sessions. Support group meetings are provided in the context of a twelve-step approach. Individuals may be referred to LEGENDS by any source. However, first priority will be given to residents of Mercer, McDowell, and Wyoming Counties of West Virginia. Mid-Ohio Valley Fellowship Home, Inc. They offer a halfway house for people who are recovering from alcoholism and/or other chemical addictions. Prestera Center offers mental health and addiction recovery services for those with co-occurring disorders. Prestera Center also offers the West Virginia DUI Safety and Treatment Program for the West Virginia Department of Motor Vehicles and can provide assessments and services that meet the state requirements for becoming a licensed driver again Page 20

The Kanawha Valley Fellowship Home offers an environment with personal security and the opportunity to begin a new life by breaking destructive patterns of behavior associated with alcoholism and addiction. Since its inception, the Kanawha Valley Fellowship Home has helped hundreds of recovering alcoholics return to sobriety, society, and their families. Mid-Ohio Valley Fellowship Home, Inc. (Long-Term Residential) Men s, Women s, and Women with Children s Programs 1030 George Street Parkersburg, WV Phone: (304) Website: The Mid-Ohio Valley Fellowship Home, Inc., is a transitional living program for men, women, and women with children. Their mission is to offer a community where the residents are encouraged to develop good health and work habits and to live cooperatively with one another. The disease of addiction alienates the substance abuser from his/her family, neighborhood and employment. The Mid-Ohio Valley Fellowship Home eases the re-entry of the recovering person into everyday life. The following four homes are within the Mid-Ohio Valley Fellowship: 1) a home that serves fourteen male residents, 2) a home that serves six women, 3) a home that serves six women with children, and 4) a three-quarter house that serves four men. The average length of stay for a resident is 6 months. Referrals are accepted from any agency or organization serving chemically dependent people. Selfreferrals are accepted provided no intoxication has occurred in the previous 72 hours. Any many or woman expressing a sincere desire to stay sober may be eligible for placement. Page 21

Beyond providing supportive services, Prestera Center staff are advocates for chronically homeless. Trained professionals help guide homeless adults through the bureaucracy of other agencies that can seem overwhelming. 24 Adolescent Treatment Services The ADAPT Program FMRS Mental Health Council 101 S. Adolescents attend the program four days a week, gradually stepping down the number of days a week until program completion. The program lasts three hours a day, and is held during hours that do not interfere with school attendance. Program services include intensive and supportive groups designed to assist the adolescents in making changes that will enhance their willingness and ability to develop and maintain a drug free lifestyle. Co-occurring disorders are also addressed not only in individual or group counseling, but also by a board certified child psychiatrist as needed. The program allows for family involvement in the treatment process. Transportation services are provided within a one hour radius of the program site.
