Also Included Is a brief summary of each home visitation model/program in West Virginia. The summaries will provide you with the population served by each model/program as well as any criteria that needs met for enrollment. The home visitation models/programs currently providing services in the State include: Partners in Community Outreach (Healthy Families America, Maternal Infant Health Outreach Workers & Parents as Teachers); ; Program; Help Me Grow; Healthy Start/Helping Appalachian Parents & Infants Project; and Save the Children. The Resource Directory is to be used when making referrals for families. The Resource Directory will be an evolving document as agencies and programs change. The Resource Directory is not intended to replace any local directories, but serve as an enhancement for home visiting services. This list of organizations may not be complete. The program has the capacity and commitment to provide infrastructure and support to assure high quality resource-based practices that serve West Virginia families. The goal of HMG is to successfully identify chiidren at-risk and link them to the help they need. Help Me Grow offers parents and medical providers: a referral line to get expert advice; community resources to heip support early childhood development; and Ages and Stages Screening Questionnaire-3 (ASQ-3). ASQ-3 is a developmentai ^ screening tool that is endorsed by West Virginia Chapter of the American HelpMe Grow Academy of Pediatrics and avaiiabie upon request. West Virginia HMG is WestVupnia the 16'^ affiliate of the National Help Me Grow Center. 4 Healthy Families America (HFA) Healthy Families America (HFA) is an evidence-based nationaily recognized home visiting program model focused on increasing protective factors in families. The goals of the program include promoting healthy child development and the positive parent-child relationships. It is the primary home visiting model designed to work with families who may have histories of trauma, intimate partner violence, mental health and/or substance abuse Issues. Services are voluntary and offered through weekly home visits during the prenatal months and until the child turns 5 years old. HAPI works collaboratively with existing systems to provide comprehensive services to those women, infants and families at highest risk in the target area. The HAPI f^roject has developed a highly integrated service delivery system with the Program (RFTS). Services provided by the HAPI Project include prenatal risk assessments for depression and substance use, prenatal oral health education and limited services, a comprehensive postpartum/interconceptionai health education curriculum and increased access to mental health and medical services. The HAPI Project also provides postpartum/interconceptionai services for mothers up to two years following the birth of their infants. The HAPI Project's targeted service region includes eight counties in north central West Virginia. All required core services for Healthy Start are provided by local RFTS Designated Care Coordinators (registered nurses and licensed social workers) and Outreach Workers (former consumers) and are delivered in conjunction with existing RFTS services. ^ Eligibility Requirements: Counties: Prenatal & Postpartum Women Prenatal: Any current RFTS participant. Eligibility for pregnant women includes WV Residents, WV Medicaid card. Postpartum/interconceptionai: Eligibility based on Risk Screening criteria such as, smoking status, postpartum depression, low birth weight-baby, high birth score, etc. Services extend for 2 years after the birth of the baby. "Mom" is the identified participant, services focusing on Mom's well-being. 5 Maternal Infant Health Outreach Workers (MIHOW) The program was developed by the Center for Health Services at Vanderbllt University in Tennessee. The New River Health Association, Inc. has sponsored the MIHOW Program in West Virginia since MIHOW is a community-based home visiting program that focuses i,<. n o \s on improving family health, positive birth outcomes, early childhood development and positive parenting practices. Services offered in ti rttmui i K'.iiiii monthly home visits beginning in pregnancy and continuing through the child's third year. Group opportunities from pregnancy until the child enters school are also available. The PAT programs in West Virginia utilize the four-part Intervention model and accompanying curriculum known as Born to Learn and Foundational Curriculum to deliver free services through parent educators to all families with children, prenatal through age five. PAT helps parents understand their role in encouraging their child's development right from birth, which prepares children for school and life success. Parents asteachers. 6 Program (RFTS) Implemented in 1990, RFTS is a comprehensive statewide home visitation and targeted case management initiative for Medlcaid eligible pregnant women and infants up to one year of age. The ultimate goals of the RFTS Program are to reduce the risk of adverse pregnancy outcomes, improve infant outcomes and decrease infant mortality. Care coordination components include helping women engage in preventive health practices such as early comprehensive prenatal care, good nutrition and reducing the use of cigarettes, alcohol and other illegal substances. Components also include helping parents increase their knowledge of child development, offering parent education and facilitating access to community resources. RFTS follows an evidence-based curriculum with fideiity to ensure the program is precisely replicated in all West Virginia communities, leading to improved outcomes for both mothers and chiidren. Care Coordination is provided by a community based workforce of 180 registered nurses and licensed social workers established in all 55 counties. (Piease see page 8) To ensure access to healthcare for low-income pregnant women in West Virginia, the Office of Maternal Child and Family Health Perinatal Programs also offer the RFTS Maternity Services Project. This initiative provides limited funding of routine prenatal care and delivery for those women who meet the following criteria: (1) WV residency; (2) Application denied for Medicaid coverage for pregnancy; (3) Gross income up to 185% of the Federal Poverty Level; and (4) No insurance or insurance that does not cover pregnancy. This also includes pregnant teens age 19 years and under monhr regardless of family income. Women who are eligible for RFTS Maternity Services are also eligible to receive home visitation care coordination services Eligibility Pregnant women and infants up to one year of age with a current Medicaid card or Requirements: rnedical coverage through OMCFH. Save the Chiidren recently launched Early Steps to School Success (ESSS), a cost effective replicable initiative that begins with expectant parents and carries children into their early school years. ESSS provides education services to children from birth to age five, support to parents and other caregivers, and ongoing training to community educators. The programs designed to assist children with language, social and emotional development; to equip parents with the skills and knowledge that will successfully support their child's growth; and to develop strong homeschool connections. 7 West Virginia (BTT) West Virginia (BTT) is a system of services and supports for children ages birth to three who have a delay in their development or may be at risk for a delay. The early intervention services are offered at no cost to families and are provided in the child's daily natural environment (home or community setting). Families play an important role in their child's development. You know your child best. The first few years of a child's life are very important, but children grow and learn at different rates.
