1 AXS-Chubb Lucky Draw and 6-month Free Mosquito Guard Promotion Terms and Conditions Terms & Conditions of the $10,000 CASH to be given away & receive a free 6-month Mosquito Guard coverage! promotion ( Promotion ) Promotion 1. The Promotion is organised by AXS Pte Ltd ( AXS ) and Chubb Insurance Singapore Limited ( Chubb ). 2. The Promotion consists of a Lucky Draw segment and an Instant Win segment. Promotion mechanics are as stated below. 2.1 Lucky Draw promotion : $10,000 cash to be given away ( Lucky Draw ) 2.2 Instant Win promotion : Receive a free 6-month Mosquito Guard TM Coverage ( Instant Win ) Promotion Period 3. The Promotion shall run from 4 July to 31 December 2017, both dates inclusive ( Promotion Period ). Eligibility 4. This Promotion is open to all The Housing & Development Board ( HDB ) customers who are eighteen (18) years old and above, who are Singapore Citizens or Singapore Permanent Residents, and who make eligible HDB eservice payments (as defined in Clause 7) at any AXS Station during the Promotion Period ( Customer ). 5. The following person(s) are not eligible to participate in and are disqualified from participating in the Promotion: 5.1 Lucky Draw Any individual aged below eighteen (18) years old Employees of AXS Employees of Chubb or staff of vendors, advertising agencies or external auditors of AXS and any other parties who are directly involved in the organising, promotion or conducting in the Promotion; and Corporate or other entities 1

2 5.2 Instant Win Any individual aged below eighteen (18) years old Any individual aged above fifty-five (55) years old 6. AXS and Chubb may from time to time in its absolute discretion decide on the eligibility for participation in the Promotion. Mechanics 7. Customers must successfully perform a HDB eservices (as listed below) payment, regardless of amount, during the Promotion Period to be eligible for the Promotion ( Qualifying Transaction ). To participate in the Promotion, Customer has to agree to disclose his/her personal information as entered for the Promotion ( Participation Form ) to Chubb. Name as per NRIC or Passport NRIC no. The information as entered in the Participation Form will be used for enrolment in Instant Win and Chubb may use it to contact the Customers for customer services, marketing and other related activities in future via mail/electronic mail/sms/ calls. Lucky Draw 10. Lucky Draw: $10,000 cash to be given away 10.1 Allocation of chance(s) Customer will receive one (1) chance in the Lucky Draw with every unique HDB eservice payment made per calendar month on AXS Station There is no limit to the number of chances a Customer can receive during the Promotion Period Chance(s) are accumulative throughout the Promotion Period. logos, and Chubb. 3 Chance(s) accumulated are non-transferable and cannot be combined with draw chances accumulated by other Customer Lucky Draw, Prizes and Winners There will be twenty (20) winners for the Lucky Draw Each winner will win S$500 cash each ( Prize ) Each winner may only win one (1) Prize Winners will be drawn on 19 January 2018, 10.30am ( Draw Date ) at 53 Mohamed Sultan Road, #01-K1 Sultan Link Singapore (or such other venue as may be determined by AXS) On the Draw Date, twenty (20) winners and twenty (20) reserve winners will be selected at random by means of a computerized or electronic system. The reserve winners are drawn to replace any winner subsequently disqualified All winners will be contacted through phone. AXS will attempt to make at least two (2) phone calls a day for three (3) consecutive working days to the winners, from phone number (+65) to obtain their full particulars for verification purposes. Instant Win: Free six (6)-months Mosquito Guard Coverage ( Policy ) 11.1 To be eligible for the Instant Win, Customer needs to be: Aged between eighteen (18) and fifty-five (55) years old (both ages inclusive); and Not insured under any free personal accident cover underwritten by Chubb in the last twelve (12) months preceding the date of receipt of the Participation Form Upon successful enrolment, Customer ( Policyholder ) will receive the Policy from Chubb containing the policy documents (including policy schedule) within seven (7) working days by electronic mail sent to the address as provided The Policy will take effect immediately from the date ( Commencement Date ) on which the Participation Form is submitted via an AXS Station Each Policyholder is only allowed to receive one (1) Policy for the Instant Win during the Promotion Period The coverage of Policy is listed below: Benefits Accidental Death (due to mosquito-borne diseases Get Well Benefit (min. five (5) days of hospitalisation due to mosquito-borne diseases) Sum Insured S$10,000 S$100 General 11.6 The Policy covers accidental death or hospitalisation (minimum of five (5) consecutive days of confinement) due to mosquito-borne diseases as defined in the policy wording. In the event of accidental death due to mosquito-borne diseases, Chubb will pay Policyholder a lump sum of S$10,000 subject to the terms and conditions of the Policy. In the event of hospitalisation (minimum of five (5) consecutive days of confinement) due to mosquito-borne diseases, Chubb will pay Policyholder a lump sum of S$100 as a Get Well Benefit subject to terms and conditions of the Policy For the full terms, conditions and exclusions, please refer to Pursuant to Section 25(5) of the Insurance Act Chapter 142, Policyholder has to disclose in his/her Participation Form fully and faithfully, all the facts he/she knows or ought to have known to Chubb, otherwise the Policy issued here under may be void and Policyholder may receive nothing from it. Chubb has only provided Policyholder with product information. Policyholder should seek advice from a qualified advisor if in doubt Chubb reserves the right to reject Policyholder s Policy enrolment in accordance to its internal policy without any notification The Policy will terminate automatically on the date falling six (6) months after the Commencement Date. Chubb may cancel the Policy at any time by giving one (1) month s prior notice. Chubb strives to maintain Policyholder s coverage at existing terms. Chubb may effect changes to the Policy with thirty (30) days advance notice. 12. AXS reserves the right to partially or completely cancel, terminate or suspend the Promotion for any reason without any liability whatsoever. 13. AXS s decision on all matters pertaining to the Promotion is final and binding. 5 therewith and/or the results will be entertained. AXS reserves its absolute right to determine an outcome and act as it deems fit in any dispute and/or issues relating thereto. 14. AXS shall not be liable to any person in any way for any loss or damage howsoever arising from or in connection with the Promotion and/or any of the Prizes. 15. AXS reserves the right at any time to amend or delete these Terms and/or any of the instructions or explanations at its sole discretion without prior notice. Any such changes shall be binding and will take effect immediately upon such amendment, change or deletion. Participation in this Promotion shall constitute acceptance of these Terms and any amendment(s) thereof. 16. In the event of any inconsistency between these terms and conditions and any brochures, marketing or promotional materials relating to the Promotion, these terms and conditions shall prevail. 17. These terms and conditions are governed by Singapore law and the Customers agree to submit to the exclusive jurisdiction of the Singapore Courts. 18. A person who is not a party to these terms and conditions shall have no right under the Contracts (Rights of Third Parties) Act (Cap. 53B) to enforce any of these terms and conditions and notwithstanding any terms herein, the consent of any third party is not required for any variation (including any release or compromise of any liability) or termination of the Promotion. Personal Data Protection Act 19. The Customers consent under the Personal Data Protection Act (Cap 26 of 2012) ( Act ) to the collection, use and disclosure of the Customers personal data by/to the organisers, promoters or conductors of the Promotion and such other relevant third party for the purpose of the Promotion and the Customers confirm that they have read and agree to be bound by the terms of the AXS Privacy Policy, as may be amended, supplemented and/or substituted by AXS from time to time, a cop y of which can be found at Customer is deemed to have given consent and authorisation to Chubb to collect, use, disclose, and/or process his/her personal data or information supplied to Chubb without further notification to him/her, confidentially with its affiliated companies, third party service providers, business partners and/or other parties which may be sited outside of Singapore, for administering policies taken out with Chubb, customer services and to allow Chubb and/or its business partners to perform marketing and related activities, until Chubb receives Customer written instruction to the contrary. Upon Customer s written request, Chubb shall, without charge, cease to use his/her personal information for purposes other than those directly related to the Policy.
