1 A DUAL ADMISSION AGREEMENT IN PHYSICAL THERAPY BETWEEN JAMESTOWN COMMUNITY COLLEGE AND DAEMEN COLLEGE This Dual Admission Agreement (Agreement) establishes guaranteed admission for students who wish to transfer from the Associate in Science (AS) Liberal Arts and Sciences: Math & Science degree program at Jamestown Community College (JCC) to the Bachelor of Science (BS) Natural Science/Doctorate of Physical Therapy (DPT) degree programs at Daemen College (Daemen). Students selected by Jamestown Community College for admission under the terms of the Agreement will, upon completion of the AS Liberal Arts and Sciences: Math & Science degree including coursework prescribed in the Agreement, continue their studies at Daemen as pre-professional physical therapy majors with the assurance that they can complete all requirements for the BS Natural Science and Doctor of Physical Therapy degrees at Daemen in four calendar years of study. Five positions in the pre-professional DPT program will be available for JCC students each year provided they meet all departmental requisites and standards of the said program as stated in the Student Selection criteria below. In the event that more than five students apply in any given year, Daemen may, at its discretion, make available more than five positions. Upon completion of the pre-professional DPT program, students who meet academic standards specified in the Agreement concerning grade/gpa requirements, course repetitions, and completion of Comp 315, PT 101, PT 201, and PT 312 will be accepted into the professional DPT program. Student Selection Jamestown Community College will be responsible for the selection of students to be admitted under the terms of the Agreement. In general, applicants will be expected to present strong records of academic achievement in high school or equivalent indicators, including four years of English, at least three years of mathematics, and at least two years of studies in science. For applicants who have taken less than twelve (12) college credit hours, an overall unweighted cumulative average of 88 percent or better for all high school courses is expected. For applicants who have taken twelve (12) or more college credit hours, an overall cumulative quality point average (QPA) of 2.8 or better for all college courses is expected. Students must apply for acceptance under the terms of the Agreement prior to completing 30 credit hours of the coursework listed in Appendix A. No more than eight (8) of the credit hours listed in Appendix A can be transferred from another college to Jamestown Community College. Any courses completed prior to application will be verified by Daemen as appropriate for the Natural Science/DPT program.

2 While at JCC, students accepted under the terms of the Agreement will be expected to meet and adhere to the same academic standards required of native students who begin their coursework at Daemen. Those requirements are listed below. Grade/GPA Requirements 1. An overall cumulative quality point average (QPA) of 2.8 or better. 2. A minimum grade of C AND an overall quality point average (QPA) of 2.8 or better in the science courses listed below. a. BIO 1570 Principles of Biology I & either BIO 1580 Principles of Biology II or BIO 2660 Zoology (equivalent to BIO 109/109L/110/110L at Daemen) b. CHE 1550 & 1560 College Chemistry I & II (CHE 110/110L/111/111L) c. PHY 1610 & 1620 General Physics I & II (or PHY 1710 & 2710 Analytical Physics I & II or PHY 1710 & 1720 Analytical Physics I & Mechanics-Heat) (PHY 101/101L/102/102L) 3. A minimum grade of C in ENG 1530 English Composition II (CMP 101), PSY 1510 General Psychology I (PSY 103), and MAT 1600 Precalculus (MTH 134). Course Repetitions 1. Any required science course (see Item #2 above) taken at JCC may be repeated once. Students will be allowed to repeat no more than two (2) science courses at JCC. When repeating these courses a grade of C or better must be achieved. a. If a JCC course listed in Appendix A is repeated and a grade of C or better is not obtained, the applicant is immediately disqualified from continuation to the professional (DPT) program. b. Repeats of courses at JCC will be counted toward the total number of repeats allowed to native students who begin their coursework at Daemen. Applicants who are accepted under the terms of the Agreement will be expected to meet the admissions requirements for the Physical Therapy program at Daemen in order to progress from the pre-professional phase to the professional phase of the program. In addition to completion of the AS Liberal Arts and Sciences: Math & Science degree including courses listed in Appendix A from JCC, students will be required to complete Comp 315, PT 101, PT 201, and PT 312 before entering into the professional phase of the program. These courses can be completed as part of a student s four years at Daemen.

3 Student Academic Advisement JCC will assume responsibility for advising students who are accepted under the terms of the Agreement. Advisors will use the Transfer Articulation/Dual Admit Agreement guide (Appendix A) as the mechanism for course advisement for JCC students. This guide will be revised every two years in conjunction with catalog cycles or as needed due to course and/or curriculum changes. Daemen agrees to provide a training session for faculty and staff at JCC who will be involved in consulting with and advising students who plan to transfer to Daemen College. Faculty/staff members from JCC and the admissions office at Daemen College will be designated as contact persons and coordinators of this program. Dual Admission Procedure Students who anticipate enrolling at Daemen College under the auspices of the Agreement must see that the JCC Admissions Office files a completed Dual Admission Program Form with the Office of Admissions at Daemen prior to the student completing 30 credit hours of the coursework listed in Appendix A.

Humanities/Social Science designations for JCC courses are: Foreign Languages (except sign language courses), Art (ART), Anthropology (ANT), Communications ([CMM] except CMM2500), English ([ENG] except 0190, 0410, 0430, 2010), Music ([MUS] except 1650), Philosophy (PHL), Religion (REL), Theatre (THE), Economics (ECO), History (HIS), Political Science (POL), Psychology (PSY), Sociology (SOC). PLEASE NOTE: Some courses from Dance and International Education may also satisfy the Humanities/Social Science requirement; these courses will be evaluated on a case by case basis. + If student takes additional liberal arts electives based on the recommendation of their JCC s advisor.
