1 NY 511 Mission The Coalition is dedicated to improving the lives of individuals and families at risk or experiencing homelessness by advocating for and providing permanent solutions for the problems of homelessness in the Southern Tier of New York State. STRATEGIC GOAL 1: USE HOUSING AS A PLATFORM TO IMPROVE QUALITY OF LIFE Stable housing provides an ideal platform for delivering a wide variety of health and social services. Through collaborative partnerships and information sharing, NY 511 will use housing as a platform for coordinating access to a wide variety of services to lower healthcare costs, end homelessness, support community living and assist individuals in the pursuit of their goals. In addition to the moral imperative to end human suffering caused by homelessness, there are compelling economic reasons for investing in efforts to eradicate this complex social problem. To achieve this goal, NY 511 will continue to partner with local and state level organizations, to deploy evidence based interventions, such as supportive housing, housing first, and rapid rehousing, to more effectively and efficiently use our limited resources to bring an end to homelessness. 1. Strategic Objective 1A: End homelessness for veterans, people experiencing chronic homelessness, families, youth, and children, and all other forms of homelessness. 2. Strategic Objective 1B: Promote advancements in economic prosperity for residents of HUD assisted housing. 3. Strategic Objective 1C: Promote the health and housing stability of vulnerable populations. STRATEGIC OBJECTIVE 1A: END HOMELESSNESS End homelessness for veterans, people experiencing chronic homelessness, families, youth, and children, and all other forms of homelessness. NY 511 s annual Point in Time (PIT) estimates measure the scope of homelessness on a single night in January of each year PIT data identified 270 households comprised of 309 persons experiencing homelessness throughout participating counties. Recognizing that some subpopulations of youth are at particularly high risk for homelessness, including youth aging out of foster care and lesbian, gay, bisexual, transgender, queer, and questioning youth, as well as the challenges presented in counting unaccompanied youth, NY 511 implemented a Youth Engagement Strategy during the 2015 PIT timeframe, identifying an estimated 112 youth residing in doubled up situations, 5 residing in hotel/motel situations, 11 youth couchsurfing, and 64 youth who self identified as residing in an Unsheltered situation. Capturing accurate data from rural areas has also presented challenges and is worth addressing in the near future.
2 STRATEGIES Implement the Homeless Emergency Assistance and Rapid Transition to Housing (HEARTH) Act with a focus on technical assistance for prioritized access to housing and use of community data to manage performance. This includes reallocation of transitional housing, when appropriate, to permanent supportive housing, prioritizing people experiencing chronic homelessness for permanent supportive housing units made vacant through turnover, and increasing the number of rapid rehousing opportunities for families. Fully engage and leverage mainstream housing assistance, including housing choice vouchers, public housing, HOME Investment Partnerships and Community Development Block Grants (CDBG), and multifamily housing. We will build capacity among PHAs and multifamily owners to admit homeless households into their units and provide them with vouchers. Improve data and performance management through strategies to share data across systems, adoption of a common data standard for housing stability, and use of Homeless Management Information Systems (HMIS) by homeless programs funded by HUD dollars. Additionally, organizations that provide homeless services but are not yet participating in HMIS will be highly encouraged to do so. This will ensure a more accurate and consistent data management over the entire Continuum of Care. Continue to strengthen collaborations at all levels including: local and state level government and organizations, faith based organizations, educational institutions, social services, community members, formerly homeless individuals, and business owners. Implement USICH Framework to End Youth Homelessness, including integrating HMIS and Runaway and Homeless Youth Management Information systems, leveraging local Point in Time counts to improve strategies for counting youth, and developing opportunities to provide safe and stable housing through transitional and permanent housing programs. Promote implementation of coordinated assessment systems for Continuums of Care through training, technical assistance, and guidance form our local field office to better target resources. During the Fiscal year, the CoC will work to reduce the number of veterans living on the streets, experiencing homelessness to zero (as measured by the 2016 Point in Time count). Total homeless veterans temporarily living in shelters or transitional housing (key measure) This metric will be measured by data retrieved from the Homeless Management Information System (HMIS)
3 Total veterans living on the streets, experiencing homelessness (key measure) This metric will be measured by the annual Point in Time count, a count of homeless persons on a single night in January each year. Veterans placed in permanent housing This includes moves into the HUD Veterans Affairs Supportive Housing (HUD VASH) Program, permanent supportive housing, and housing through Housing Authorities (Section 8). Homeless Veterans placed in transitional housing Other Measures: Individuals experiencing chronic homelessness Number and percentage of permanent supportive housing units targeted to individuals experiencing chronic homelessness Families experiencing homelessness Admissions of new homeless families into HUD assisted housing Percentage of Emergency Solutions Grant dollars dedicated to rapid rehousing for homeless individuals and families STRATEGIC OBJECTIVE 1B: ECONOMIC PROSPERITY Promote advancements in economic prosperity for our communities Residents of HUD assisted housing often face challenges such as lack of employable skills and low educational attainment levels that limit their ability to become economically self sufficient and rise out of poverty. The CoC recognizes that, while some families and individuals will need assistance for longer periods, others are capable, with assistance, of rising out of poverty. A majority of adults receiving rental assistance who are able to work have some income from wages; however, they are most often in the lowest paying jobs. Further, increasing workplace demands for technical expertise require attention to education and training for both adults and youth, including digital literacy. NY 511 will utilize its collaborative efforts to expand access to employment and educational services. STRATEGIES Implement evidence based housing programs that link individuals with disabilities to safe housing and supportive services. NY 511 will assess models that deliver permanent supportive housing linked with employment, behavioral health services, and family unification. Utilizing the evidence based model of Housing First NY 511 promotes Housing First as the best model to ensure safe and stable housing that supports individuals with disabilities Access to Vocational/Educational Programs NY 511 programs and organizations provide Vocational and Educational training either in house or at local sites within our communities to develop the necessary skills to obtain employment
4 Networking and Outreach NY 511 participates in networking and outreach activities that promote the various organizations and programs available to our communities Workforce and CareerWorks NY 511 organizations assist clients with the resources available at local level labor departments to enhance opportunities to find training and employment During the Fiscal year, the CoC will collaborate with area resources to sustain current supportive housing programs and explore new options as funding becomes available Implementation of the Housing First evidence based model All supportive housing programs will be encouraged to adopt the Housing First model. During the annual NOFA, programs will report to the CoC if their progress in implementing this model. Vocational/Educational programs /Workforce Programs will report on Voc/Ed and Workforce opportunities during the NOFA process and during monthly CoC meetings. Networking and Outreach Outreach activities will be tracked using HMIS. Networking will occur during presentation at CoC monthly meetings and be reflected in the minutes. STRATEGIC OBJECTIVE 1C: HEALTH AND HOUSING STABILITY Promote the health and housing stability of vulnerable populations. Residents of HUD assisted housing often find themselves challenged to maintain housing stability due to the nature of their disabilities, particularly substance use disorders and mental health disorders. Maintaining overall health is a priority for the CoC (NY 511) and we understand the unique barriers posed by various disabilities. NY 511 promotes housing programs that do not penalize individuals for their struggles with their disabilities in the hopes that stable housing will lead to improved healthy behaviors and access to healthcare resources
5 STRATEGIES Promote health and financial stability of vulnerable populations by identifying opportunities to determine eligibility for Medicaid automatically or routinely. NY 511 will use income information collected in HUD funded programs and partner with state Medicaid programs and health insurance navigators. Utilize established evidence based practices on effective models for coupling services with housing and modify existing and future programs to reflect best practices. NY 511 will utilize Housing First and other evidence based models to ensure stable housing for individuals with disabilities and links to resources and services as appropriate Improve performance management by enhancing NY 511 s collection and analysis of HMIS data pertaining to health related outcomes across HUD assisted housing programs. Analyzing health outcomes will assist with CoC planning activities and program development Health and financial stability This measure will be tracked through HMIS to determine enrollment in Medicaid or other health insurance programs, enrollment in entitlement programs, and total income measures. Evidence based and best practices Programs will report on their use of evidence based and best practices during the NOFA process Health Related outcomes???? STRATEGIC OBJECTIVE 4C: DISASTER RESILIENCE Support the recovery of our communities from disasters by promoting community resilience, implementing disaster plans at the local and county levels. STRATEGIES Implementing disaster plans NY 511 will work with local and county agencies and officials to implement disaster plans Using E Finds In the event of a disaster, NY 511 agencies will implement the E Finds system to maintain accountability and client safety
6 Working with Emergency Services and Disaster Relief organizations NY 511 organizations will cooperate fully with Emergency Service personnel. Local agencies will follow directives of the Broome County Emergency Management. Organizations like the American Red Cross will be contacted and utilized to provide assistance as needed Implementing disaster plans All CoC programs will report on the status of their disaster plans at least once a year. In the case of an actual disaster, the CoC will evaluate the implementation of any plans and make adjustments as necessary. Using E Finds All CoC programs will report on the status of their E Finds program. In the case of an actual disaster, the CoC will evaluate the implementation of E Finds and make adjustments as necessary. Working with Emergency and Disaster Relief organizations In the event of an emergency or disaster, the CoC will evaluate the implementation of all planning and usage of local relief organizations. STRATEGIC OBJECTIVE 4D: COMMUNITY DEVELOPMENT Strengthen communities economic health, resilience, and access to opportunity. NY 511 is located in areas that are greatly challenged by the current economic climate and lack of employment opportunities. We are committed to developing and implementing strategies that promote sustainable economic opportunities. We work with local and legislative/governing bodies to implement ESG, CDBG, and other housing resources to develop and implement community resilience through stable housing. STRATEGIES Development and implementation of Youth partnerships NY 511 works with Catholic Charities Teen Transitional Living Program (TTLP), Berkshire Farms, the YWCA, and other youth programs to provide stable housing opportunities for youth.
7 Annual Job Fair NY 511 hosts an annual Job Fair that strategically targets populations who have the most barriers to employment (substance users, mentally ill, formerly incarcerated, etc.) and connects them with employers and vocational programs willing to engage them. Transportation NY 511 is committed to exploring reliable transportation options in order to empower communities when searching for and obtaining employment and/or vocational training Community Awareness NY 511 regularly engages in Community Awareness activities that promote education and awareness of homelessness and the issues associate with homelessness in our local communities. Youth Partnership This measure will be evaluated based on the participation of local organizations that serve Youth. Our membership database will reflect which organizations are participating in the CoC. Job Fair The Job Fair committee will report out to the CoC monthly when meeting and when the event occurs. Data captured will include how many people attended, how many vendors attended, and other relevant outcomes. This will be reflected in the minutes. Transportation The CoC will record all discussions regarding transportation in our meeting minutes. Any program or project ideas will be presented to the CoC for approval. Community Awareness The Community Awareness committee will report out to the CoC monthly and provide relevant data regarding various events throughout the NY 511 area. This will be reflected in the minutes.
