1 Building searches and templates for EMIS Practices NHS Health Check 2011/12 The Read codes used in this guidance document are the National Data Set and are a requirement for the recording of the NHS Health checks 2011/12 Attached is a EMIS document - Building and Editing a Template Target population: All patients age years without (not diagnosed with any of the listed criteria) (see below) on the 1st of April 2011 are eligible for an NHS Health Check Criteria All Patients aged years with the following pre-existing diagnosed conditions are not eligible for an NHS Health Check: Ischemic Heart Disease Diabetes Mellitus Chronic Kidney Disease Transient Ischaemic Attack Peripheral Vascular Disease History of Stroke Patients under Palliative Care/ End of Life Treatment Patients who have declined at least two invitations in the past two years by GP practices as well as Hard to Reach Provider Exclusion of Health Checks done in and : All patients who have been claimed under the Health Checks LES by the practice in and These Health Checks were recorded under specific Read Codes and can be excluded easily. However, the NHS numbers of these patients were sent to the PCT so if practices do require these details they can be sent to the practice upon request Please find attached a template guidance that will assist in the criteria and Read Code requirement for the recording of the NHS Health check. The codes used on the template guidance are from the National Data Set and their use is a requirement for recording Health checks.
