1 St. Michael the Archangel Catholic Church Baptism Registration Packet The Church teaches the parents of an infant to be baptized and those who are to undertake the function of sponsor are to be instructed properly on the meaning of this sacrament and the obligations attached to it. Canon 851, 2 o Baptism Preparation Class The Baptism Preparation classes are offered once a month on the first Tuesday at 7:30pm. You must leave a message in the Baptism preparation voic and register for the class no later than the Friday before the class at (303) ext.340 Baptism Celebration Day The Sacrament of Baptism is celebrated on the first and third Saturday of the month at 9:00am*, or on the second Sunday of the month at a designated Mass. Baptism Celebration date will be scheduled during the Baptism preparation class *December 17, 2016, February 4, 2017 & April 1, 2017 at 8:30am Requirements to have a Baptism Celebration at St. Michael the Archangel o Child must be under 7 years old o The Godparent must be a fully-initiated Catholic o You must complete the attached forms and submit at the time of the Baptism preparation class o Baptism Registration Form o Submit a copy of the Child s birth certificate o Family Information Form (If you are already a registered member) Ask for a registration form if you would like to register o Godparent Parish Affiliation Form Must be signed and sealed by the godparents parish o Both parents and godparents must attend a two hour Baptism preparation class before they can schedule a Baptism date o If the parents or godparents have already attended a class within the last two years, they must provide a letter with the date of attendance o Schedule the Baptism celebration date and Baptism presentation date on the day of the class o Call the Parish office one week before the Baptism celebration to confirm that all the paperwork has been received and that all of the steps have been completed Requirements / qualifications to become a Godparent o Must be fully initiated Catholic, meaning they have received the Sacraments of Baptism, First Communion, and Confirmation. o Must be 16 years or older. o Must participate regularly at Sunday Masses and receive Holy Communion o If married, must be married according to the teachings and laws of the Catholic Church o Those cohabitating do not meet the requirements o We are willing to work with you if you have any questions or concerns about these requirements Please contact the parish office if you have any questions
2 Baptism Preparation Class Information Form Dear Parents and Sponsors, You have asked to have your child baptized. That is a great gift. Jesus is waiting to give graces not only to the child, but also to your family and the sponsors or godparents who will assist with your child s spiritual formation. Baptism is the doorway to the graces that Christ will give to your child. It is the key that gives access to all the sacraments. It is essential for salvation and, consequently, proper preparation is required before the sacrament can be received. The Baptism Preparation team looks forward to meeting you and your sponsors. It is our great privilege to accompany you on this journey as your child receives the gift of God s life and limitless love into their soul. For this reason BOTH parents are requested to attend a Theology of Baptism class before their child's baptism. In the Archdiocese of Denver, "parishes are to provide a catechetical program of instruction for parents and godparents preceding the Baptism". -Pastoral Handbook, We encourage parents to take this class during pregnancy whenever possible. It is a good way to spiritually prepare for the life-changing event of your child's birth, and it gives the godparents time to attend the class. If it has been over two years since you took a Baptism Preparation class, you will need to attend. This is an opportunity to become reacquainted with this beautiful sacrament. To register for the class, please call the Baptism preparation class voic ext. 340 Once the class is completed, the Baptism celebration may be scheduled at St. Michael s
