The Sacrament of Baptism marks a very important point in the spiritual life of your child and of the Church. Through this first Sacrament of Initiation, your child is welcomed into the community of God s family. Because it is so significant, and to meet the requirements of Canon Law, the following Parish policy has been established for the Baptism of children who have not reached the age of reason, usually about seven years of age. Parents and godparents are required pre-baptism formation (Canon 851.2). In order to register for this formation, Please fill out the Baptismal Information form. When filling in the information, please print legibly as this information will be used for the Baptismal Record. Turn in the completed form, with a copy of your child s Birth Certificate (showing child s name, parent s names including mother s maiden name, date and place of birth), Have sponsor s forms signed and sealed. Turn in the godparent verification forms, (please have godparents answer all 4 questions on their part of the form) and a $50.00 Administrative Fee (covering pre-baptismal class costs, administrative fees, supplies, etc.; no other fees are required for Baptism) to the Parish Office by the deadline. Additionally, the Baptism date for your child will not be assigned until after parents and godparents have attended pre-baptism classes, we respectfully request that you not make plans for a particular date until then. As a reminder Canon sets as a rule that infants are to be baptized in the parish church proper to their parents (either living within the Parish boundaries or registered as members). If this is not the case, please request a letter of permission to baptize outside of the parish, particularly if it is not a parish of the Diocese of San Bernardino. Finally, Canon 872, 873, and 874 requires you to have at least one godparent (you may have two; one godmother and one godfather) who is at least sixteen years old; has received the Sacraments of Baptism, Confirmation, and Eucharist; and leads a life in harmony with the faith and the role to be under taken (i.e. a practicing Catholic: regularly attending Mass; receiving the Sacraments; if married, married in the Church). Although other Christians may stand as witnesses to the Baptism, they are only Christian Witnesses, not godparents. 2 Schedule for Baptismal Classes & Baptisms for 2016Revised It is a requirement for the parents and godparents to attend the baptismal class. The godparents can choose to take the baptismal class at their parish. Godparents attending Baptismal Class at another parish: We will need the original certification with seal, two weeks prior to the Baptism. The certification must indicate that the Baptismal Class has been taken. If parents or godparents have already taken a Baptismal Class: If the parents and/or the godparents have already taken the Baptismal class, proof (original sealed letter/certificate) must be provided with the package at time of registration. A baptismal class taken at Sacred Heart, Rancho Cucamonga is valid for 2 years.
