Under direction, successful candidates will work in providing social work services on cases involving complex social problems and social treatment plans in rural areas; performs responsible and difficult casework, directs service programs and performs related work as assigned. The deadline for application submission is August 21, 2015 Duties will primarily focus on Child Protective Services, but may also include other responsibilities, such as Foster and Day Care Licensing, Chemical Dependency and working with disabled individuals. Under general supervision, successful candidates will perform work of considerable difficulty in providing intensive social work services for the protection, rehabilitation and care of children. These positions will also potentially provide case management and social work services to individuals with developmental delays, addictions and vulnerable adults within the community. MINIMUM QUALIFICATIONS: Four Year College Degree from an accredited college/university with a major in social work, psychology or sociology (or related field) Attain a passing score on the Minnesota Merit Social Workers Exam. Also provides case management and social work services to children, families and vulnerable adults in the community. Conducts complex case investigations and evaluation for the purpose of assessing problems and determining appropriate types and methods of treatment. o Assess family dynamics in order to determine needs and level of service to be provided to the family. o Develop a concrete case plan with goals and expectations which allows others to reduce the impact of the identified problems and assists those involved to make progress towards change. o Interview client(s) and collateral sources as appropriate. o Interprets policies, regulations and programs to clients and family. o Identifies client situations, which may require intensified service and brings to the attention of supervisor for assistance or referral. o Provides direct services to emotionally, physically, or socially handicapped individuals. o Provides and assists in providing services to cases involving difficult or complex social and financial problems. o Recognize and report indicators of neglect, emotional, physical, and sexual abuse of both children and adults. o Access environmental factors within home, school, neighborhood and community. o Coordinate services with all other providers for a collaborative service delivery. o Prepares social histories with emphasis on psychological factors, adoption, or foster home placements. o Supervises foster home placements. o Interviews and screens prospective adoptive parents. o Provides preventive services. o Provides casework services for rehabilitation and care of clients. o Communicates effectively with the end result of avoiding conflict, when possible and resolving constructively when it occurs. o Testifies in court proceedings and implements judicial findings. Provided accurate documentation so that client services are efficiently delivered, records are in compliance with Agency/State/Federal regulations and reimbursements for services are maximized. o Establish and maintain requirement electronic and/or hard copy case file for eacy family. o Complete correspondence, case plans, and reports timely. o Maintain daily activity log is SSIS. o Comply with all mandatory reporting of abuse, neglect, exploitation, or fraud. Individual will be required to travel to client homes and to service provider locations on a frequent basis. May transport clients and their possessions. The job may include exposure to inclement weather, unpleasant living conditions, etc. Clients may be uncooperative or socially inappropriate. Page 3
