8 About Introduction to Obesity Obesity is when a person is carrying too much body fat for their height and sex. It has been on the increase for a number of years, as life is less physically active than it used to be. Without lifestyle changes to increase the amount of physical activity done on a daily basis, or reduce the amount of calories consumed, people can become obese. How Do You Know If You Are Obese? Most doctors calculate obesity using a formula known as the Body Mass Index (BMI) to determine whether your weight is healthy. This is a measure based on height and weight that applies to both adult men and women. Part of your initial assessment with The Hospital Group would be to measure your BMI to help you to decide whether you require a weight loss management procedure and to confirm your suitability. Not everyone who struggles with their weight should consider surgery, your own particular Body Mass Index (BMI) will determine whether you are overweight, obese, or morbidly obese. What causes Obesity? Energy Balance - To work efficiently, your body needs the energy that comes from food. When you eat the same amount of food that your body needs, your weight stays the same. If you eat more food than you need, some energy is left over. Your body stores that extra energy as fatty tissue and if your body never uses the extra fat, you will gain weight. Hereditary Issues - If others in your family are obese, then you have a higher risk of obesity Metabolic Disorders - Changes in metabolism, or how your body gets energy from food, may affect your energy balance and your weight Eating and Social Habits - Eating an unbalanced diet, snacking between meals and not getting enough exercise, may all contribute to obesity Psychological Factors - Social or emotional eating is also one of the main causes of gaining weight. Any one or a combination of these factors can lead to obesity. As science continues to search for answers, morbidly obese individuals must understand how to treat their condition in the most effective way.
9 Understanding your BMI You can visit our web site to calculate your BMI at Your BMI will also be measured when you visit for a consultation. Body Mass Index Classifications BMI less than 19 (Underweight) A BMI of less than 19 shows you to be underweight. You definitely do not need any form of weight-loss surgery BMI of 19 to 25 (Ideal weight) Congratulations! Your weight is within the normal range. At this stage, it would appear that you are not a candidate for obesity treatment. BMI of 25 to 30 (Overweight) This BMI is classed as mildly obese. You could consider a non-surgical weight-loss management programme e.g. the Gastric Balloon BMI over 30 (Obese) A BMI of over 30 is classed as moderately obese. At this BMI, you would qualify for the gastric band. BMI over 35 (Severe Obesity) With this BMI you would be classed as severely obese. The most appropriate form of weight-loss surgery for you would be the gastric band. BMI over 40 (Morbid Obesity) This BMI score is classed as morbidly obese. You are almost certainly a candidate for obesity surgery and could consider the gastric band or gastric bypass. BMI over 50 (Super Obesity) This weight is classed as super obese and is potentially life threatening. The most appropriate form of weight-loss surgery would be either the gastric band or gastric bypass. As BMI increases, so does the risk of associated health problems, including heart disease, type 2 diabetes, high blood pressure, high cholesterol, arthritis, asthma, sleep apnoea and several cancers Any surgery or weight loss procedure would be discussed at length with an obesity surgeon at the point of consultation and would be dependent upon the individual patient's co-morbidities.
10 Gastric Gastric band Key Points: 8 Feel fuller sooner and for longer, so you eat less & lose weight 8 Reversible procedure Fig 1 : The band is placed around the upper part of the stomach, creating a small pouch. The Gastric Band is an inflatable silicone plastic ring that is placed around the upper part of the stomach, creating a small pouch at the top of the stomach with a small opening to the rest of the stomach. This controls the amount of food that passes from the stomach pouch into the digestive tract, which makes you feel full faster so that you are able to eat less and therefore lose weight. The procedure generally delivers weight loss of 50-65% of excess weight over a 2 year period. It is recommended for patients whose BMI (Body Mass Index) is 35 or over but can be used from BMIs of 30. The Gastric Band is the only adjustable and reversible weight loss surgery that does not involve cutting and stapling of the stomach or gastrointestinal re-routing to bypass normal digestion. Patient benefits include reduced surgical trauma, complications, pain and scarring, as well as shorter hospitalisation and recovery time compared to other weight loss surgeries. Fig 2 : The band controls the amount of food that passes from the stomach into the digestive tract. The diagrams show the band in place around the top of the stomach. The gastric band makes the stomach into an 'hour-glass' shape. The new small upper stomach pouch is about the size of a golf ball, with the larger part of the stomach sitting below the band. This means that the pouch above the band can only store a small amount of food, which then passes through the band into the lower part of the stomach. The band controls the flow rate of food from the upper to the lower part of the stomach, which means that you feel fuller sooner and the feeling lasts longer. The drier the texture of food, the slower it will pass through the opening created by the band. You will receive full dietary advice and support from our expert team to ensure you get great results. 10

It is adjusted using a special port attached by thin tubing to the Band. This port is placed just under the skin and fluid (sterile saline - a salty solution like other fluids in your body) can then be injected or withdrawn through a very small tube to inflate or deflate the band. This will then adjust the size of the opening between the upper and lower parts of the stomach. The band can be inflated and deflated until the right level of restriction is established. It usually takes two adjustments to find the right level of restriction, but additional adjustments may be required. After your first band fill (which is carried out under x-ray usually between 4-6 weeks after surgery) these will be carried out at your clinic Women that get pregnant and have a band in place can simply have the gastric band loosened for the duration of their pregnancy. 13 Probably the biggest question facing anyone who is contemplating weight loss surgery is "how effective will it be?". We recently monitored the progress of 106 gastric band patients. The results speak for themselves: After 9 months, the total weight loss recorded was a massive 300 stone or 4,197 pounds (or 1,904kg)! The average weight loss per patient was 3 stone - that's 40lbs (or 18 kg) 99% of patients had lost weight following the surgery - only 1 patient had not lost any weight One super-slimmer had lost nearly 14 stone or 191 lbs (87 kg) The scarring from the gastric band operation is minimal, some patients struggle to find their scars after 12 months. Some surgeons also practise the pioneering belly button method, which is a single point of access to the stomach with minimal scarring. The insertion of the port is very small indeed and is placed in a central position, just under the breast bone. If you attend one of our consultations with one of our Patient Care Co-ordinators, many of these are also band patients, so you can ask to see their scar and they will be more than happy to show you - so you can see just how tiny the scarring can be When a patient achieves their target weight they should book an appointment with the nurse in order to have a slight amount of saline taken out of their band. This is so that the correct restriction is applied in proportion to your dietary needs. Yes, you can. Prospective patients sometimes think they will no longer be able to enjoy a night out with friends at a restaurant because of their gastric band. This is a particularly common question amongst patients who want to keep their band hidden from friends and relatives. Although the gastric band is the ultimate portion controller it will not prevent you from enjoying food. You will not be able to complete every course, due to the restrictions of your band, however a moderate portion of each can be enjoyed. 13 * Source:
14 Gastric Gastric band Key Points: 8Dramatic weight loss for severly obese patient 8 Non - reversible 8 Nutrient Supplements will be needed after Gastric bypass operations are a surgically proven form of weight loss when combined with a supervised diet, physical activity and behaviour modification programme. They are generally recommended for patients with higher BMI i.e. a Body Mass Index of 40 or over and according to national statistics, deliver 66-75%* reduction in excess weight in the first 2 years. There are several varieties of bypass, but all involve the creation of a small stomach pouch to restrict food intake and reduce the absorption of calories and nutrients from food which causes effective weight loss results. The small intestine is connected to the stomach pouch The gastric bypass procedure effectively reduces the size of the stomach and re-routes the small intestine. It works by making the stomach much smaller so that you feel fuller sooner. It also works by curbing the appetite as the food goes quickly into the small intestine, chemical messages are sent to the brain telling the satiety centres that food is present. The combination of restricted food intake plus malabsorption of calories, results in significant weight loss. The gastric bypass is a one off surgical procedure that should not require any further interventions. Surgery will also improve the patients sense of self control and helps many avoid sugary/fatty foods due to dumping syndrome'. Unused body of the stomach 14

Two of the most commonly performed are the Roux-en-Y gastric bypass (RGB) and Biliopancreatic Diversion (BPD). These - and their variations - are usually performed as keyhole procedures. Roux-en-Y Gastric Bypass (RGB) First a small pouch is created by stapling off the upper part of the stomach. This restricts the amount of food you can eat. Next a carefully measured length of small intestine is divided, one end is connected to the newly formed pouch, creating a channel for food to travel down, bypassing the now redundant stomach and duodenum (beginning of the small intestine). Gastric juices, (containing essential enzymes for absorption of calories and nutrients from food) are introduced to the food by rejoining the small intestine, creating a Y shaped bypass. Biliopancreatic Diversion (BPD) In this more complicated version, portions of the stomach are removed and the small pouch that remains is connected directly to the final segment of the small intestine. 18 Gastric Gastric sleeve Key Points: 8Dramatic weight loss for severly obese patient 8 Non - reversible 8 Nutrient Supplements will be needed after The small intestine is connected to the stomach pouch The sleeve gastrectomy is a lesser known procedure and suits a specific type of weight loss requirement. With an average weight loss of 30-50% excess weight in the first year, the gastric sleeve is more aggressive than a gastric band, but not such a major operation as a gastric bypass and can therefore suit those for whom the bypass may be considered inadvisable due to health complications. The gastric sleeve is a stomach reduction procedure. As such, it is a longer and more complicated procedure than the gastric band, but unlike the band, it cannot be "cheated" as it works to reduce the size of the whole stomach, not just restrict the entry to the stomach. Weight loss is usually more rapid than the band, as a sleeve gastrectomy significantly reduces appetite and ability to eat large portions. Like the bypass, the sleevectomy is irreversible. However, compared to the bypass, it is less traumatic in that it does not require any "rerouting" or "reconnecting" of the intestines, so the stomach functions as normal, but with a smaller capacity. As a result, there are fewer restrictions on the types of food which patients can consume after surgery. This is seen by many patients as being one of the great advantages of the sleeve gastrectomy - the procedure reduces appetite and therefore helps patients to control their food volume intake. Unused body of the stomach The gastric sleeve can be considered as a step towards gastric banding, in that it can later be converted if required. However, for others, the sleeve may enable them to lose enough weight with a sleeve gastrectomy alone, so that the second stage of the operation is not required. It is a very effective procedure on its own for many patients. 18

The sleeve gastrectomy reduces the capacity of your stomach by about 75% and includes the removal of the upper part of the stomach, which produces the chemical Ghrelin which stimulates appetite. This means that you will only be able to eat small meals and you will feel fuller for longer, so you will lose weight. Unlike the gastric bypass, the stomach function remains unaltered, which means a less complicated procedure and fewer side effects. The Gastric Bypass is a complex operation, sometimes taking a number of hours. If a patient is super obese or has severe obesity-related conditions, it may be too risky to proceed with a full bypass procedure. A sleeve gastrectomy is by comparison less complicated and quicker to complete, so can be done to start the weight loss process. The second stage (i.e. converting to a full bypass) would normally be done months after the first, but this does vary depending on the patient s weight loss.
