1 IOSH Managing Safely Project guidance v3.3 Download your risk assessment form at Introduction In order to gain your Managing Safely certificate, you must successfully complete a risk assessment project. You are expected to carry out a risk assessment in your own workplace. If you are not in work, or you cannot carry out the assessment in your own workplace, you may carry it out in another workplace. You must record the results on the risk assessment form provided by IOSH. This can be downloaded at The project should take about two hours to complete. This includes gathering the information and then completing the forms. The project does not have to be completed in one go, so you can fit it around your work commitments. The project must be submitted to the British Safety Council within 14 days of the end of the course. All projects are marked by independent examiners. The pass mark is 23 out of 38 (61%). Some general rules It is important that you follow the guidance contained in this document. You should address each question separately and answer it as fully as you can. You may find that you occasionally repeat yourself when answering subsequent questions. Don t worry about this. Simply provide the information requested. The examiner marks each answer as a standalone, even if you have provided the information in a previous answer. The examiner will not be familiar with your workplace, so you must make things very clear for them. Keep things simple but be as precise as possible. For example, if describing a potential fall from height, focus on the two most important things: the height of the fall and the surface on which the person lands. A same level fall on to carpet is unlikely to result in serious injury, whereas a fall of 10 metres from a ladder on to a concrete surface is likely to lead to serious injury. The examiner can only mark what s/he sees, so make sure that your answers would make sense to a third party. Read out your answer or, better still, get someone else to read it. Your answers must be typed into the PDF document provided by IOSH. In order for you to type and save text in the PDF document, you must be using the latest free version of Adobe Acrobat Reader. Instructions for downloading the latest version are at Appendix A.
2 Risk assessment form Page 1 Enter the assessor s name (your name) and the date. This will gain you 1 mark. Describe the location where the activities are taking place 1 mark is given for an accurate description. You must describe; don t just list. In order to gain the mark, the description must include: The business function (what the organisation does) The size of the area where the activities are taking place (in square metres) Whether the location is indoors, outdoors or mixed Whether the location is one or more floors Details of entrances and exits (including fire exits) Describe the activities being performed 1 mark is given for an accurate description. You must describe; don t just list. In order to gain the mark, the description must include: The nature of the routine activities (when, what, who) The nature of any non-routine activities (when, what, who) An outline of the cleaning work (when, what, who) An outline of maintenance work (when, what, who) Describe the equipment and/or substances being used 1 mark is given for an accurate description. You must describe; don t just list. In order to gain the mark, the description must include: Types and amount of equipment Types and amount of substances Note: if you do not refer to amounts, you will not gain the mark. For example "4 desks, 4 chairs, 1 filing cabinet, 1 photocopier and 2 litres of cleaning fluid. Describe the people involved in these activities (and others who might be affected) 1 mark is given for an accurate description. In order to gain the mark, the description must include reference to: The people actually undertaking the activities Other employees (e.g. those passing through the area) Contractors Visitors Members of the public (if not applicable, then state that there are no members of the public) Trespassers (if there is no chance of trespassers, state why, e.g. a secure area) State the number of people in each category. For example 3 operatives and up to 50 other employees who may pass through the area. Page 2 of 6
3 Risk assessment form page 2 There are a total of 33 boxes (11 columns, 3 rows). There is 1 mark available for each box. There is a total of 33 marks available on this page. Description of hazard and hazardous event You must identify ONE hazard for EACH of location, activities and equipment/substances. Make sure that your first hazard (top row) is specific to a location. For example, a slippery floor in the entrance hall or poor lighting at the back of the warehouse. Be specific about the location. Make sure that your second hazard (middle row) is specific to an activity. For example, carrying heavy boxes from the reception area to the first floor. Be specific about the activity. Make sure that your third hazard (bottom row) is specific to equipment or a substance. For example, use of a photocopier, or use of chemicals for cleaning. Be specific about the equipment or substance. For the hazardous event, describe what could happen. For example, someone could slip over in the entrance hall or someone could injure their back while lifting the heavy boxes. Note: The hazard in the top row must be specific to a location. If it is not, you will instantly lose all 11 marks for that row. The same applies to the other rows. The hazard in the middle row must be specific to an activity. The hazard in the bottom row must be specific to equipment or a substance. Who might be harmed? List all the people who might be harmed by that particular hazard. State the numbers involved (e.g. two cleaners). Don t just think about employees. Think about contractors, visitors and members of the public. What risk controls are currently in place? Describe the existing control measures. These might include training, PPE and signage. Comment on the existing level of management control is it sufficient? Current risk rating You must use the 5 x 5 risk rating calculator on Page 3. Provide a current risk rating for likelihood (the chance of harm occurring) and for consequence (the potential severity of that harm). Multiply the two ratings together to give a risk rating. For example: Likelihood = 2 (unlikely) Consequence = 4 (major more than seven days absence) Risk rating = 8 Are further controls needed? The examiner will only award the mark if the answer is appropriate in relation to the current controls. The idea is to reduce the risk and this normally means reducing the likelihood. Think about what could be done to reduce the risk. This might involve providing a higher standard of protection, providing further information and training, or providing enhanced levels of supervision/monitoring. Think about management controls as well as practical controls such as signage and barriers. Page 3 of 6
4 Date to be completed by State a realistic date for the implementation of the further controls. The different controls may have different timescales, for example: Signage by end of this week; refresher training within two months. Residual risk rating You must use the 5 x 5 risk rating calculator on Page 3. Provide a revised risk rating for likelihood (the chance of harm occurring) and for consequence (the potential severity of that harm). Multiply the two ratings together to give a residual risk rating. Make sure you can justify the change in ratings. Do your further controls warrant a change in rating? Be careful to differentiate between likelihood and consequence. Further controls normally lead to a reduction in likelihood, but don t normally lead to a reduction in consequence. In other words, we may have reduced the likelihood but, if an accident happens, the consequence will still be the same. Using the example from the previous page, our ratings might now be: Likelihood = 1 (very unlikely) Consequence = 4 (major more than seven days absence) Risk rating = 4 Submitting your project The project must be submitted to the British Safety Council within 14 days of the end of the course. your project to examinations.officer@britsafe.org Certificates will be despatched to successful candidates within 12 weeks.
