1 Birthday Party Rules/Reminder Checklist Please initial your choice below: Basic Party: Gym Fee - $130 (for up to 10 kids) $5 each additional child Coach s fee - $25 per coach (10 kids to 1 coach) Deluxe Party: (Includes 2 lg pizzas and birthday child t-shirt) Gym Fee - $170 (for up to 10 kids) $5 each additional child Coach s fee - $25 per coach (10 kids to 1 coach) Dance Party: 3 p.m. ONLY Gym Fee - $130 (for up to 10 kids) $5 each additional child Coach s fee - $25 per coach (10 kids to 1 coach) You may also choose to have a 2 hour party (for the 3 pm time slot only) The additional time will cost: Basic Party - $ (for up to 10 kids), $5 each additional child Deluxe Party - $ (for up to 10 kids), $5 each additional child Coach s Fee - $40.00 (10 kids to 1 coach) The coach will go over a few rules to ensure each child s safety. If a party guest is behaving in such a way that he/she is not listening to the instructions given by the coach or are endangering themselves or others, he/she will be asked to leave the floor and sit out.

2 You will have a birthday party attendant in the party room to serve the juice & to clean up after your party. The attendant serves as a hostess or host of the party. Once the party participants come off of the gym floor to go into the party room, they ARE NOT PERMITTED back on the gym floor. There will not be a coach on the floor to ensure their safety!

3 GYM SOUTH BIRTHDAY PARTIES Thank you for choosing Gym South for your child s birthday party! We have a great time planned for your child s special day! When the children are in the gym, they will play games, jump in the inflatable, have races and fun competitions! If you choose the Basic party, all party participants will receive a goodie bag. If you choose the Deluxe party, everyone gets a goodie bag and you also get 2 large pizzas and the birthday child receives a Gym South t-shirt. Parties are available on Saturdays, at 2:00 or 3:00 pm. Dance parties are at 3:00 pm ONLY. The party will require a $100 cash or c/c deposit. If you choose a c/c deposit, your card will be charged $100 immediately. This $100 will go towards the total birthday party cost. If you need to cancel the party for any reason and you do not give us at least 1 weeks notice, your c/c will charged the full amount of the party. Each coach is to be paid $25.00 cash at the time of the party. This cost is not included in your party fee. You must have 1 coach per 10 children. Please be aware of the following: No later than 5 days before your party, you must contact us to confirm the final number of kids attending your party. This allows us to contact more coaches if needed. During youth parties, only the birthday party child s parents will be allowed on the floor to take picture or video and they must fill out a waiver. During preschool parties, every child will need a parent on the floor and the parent must fill out a waiver. Please do not use flash photography.

4 There will be a $2.00 per minute charge for those who go over their appointed times. (There may be a party after yours so please be courteous). Your party will start on time and no additional time will be added to your Party if you or your participants are late. Please ask that your party participants wear appropriate clothing. (t-shirts comfortable shorts/leggings or leotard). Alcohol is not permitted at Gym South. Gym South and its employees are not responsible for children that are not picked up from the party on time. Please make sure that all parents are aware of the time your child s party will be ending to ensure all participants are picked up on time. Every party participant must have a waiver signed by their parent or legal guardian. I certify that I have read the birthday party explanation and have had all of my questions answered.
