3 About Allina Health Allina Health is a not-for-profit health system consisting of clinics, hospitals, and other health services, providing care throughout Minnesota and western Wisconsin. Serving the community 57 primary care clinics 49 rehabilitation locations 23 hospital-based clinics 13 hospitals 16 retail pharmacies 2 ambulatory care centers Home care, hospice, palliative care offerings Emergency medical services Home medical equipment

6 Involving the Voice of the Patient and Family: The WHY The right QUESTIONS Patients and their family members help us identify our BLIND SPOTS. They have unique expertise that only they can bring to the table. We tend to need them when we don t think we need them. The right PEOPLE Involvement demonstrates that they are VALUED MEMBERS OF THE CARE TEAM. They have a stake in us and care about helping us improve to enhance the care we deliver. And, would we leave any other stakeholders out? The right TIME Patients and families help increase EFFICIENCY. By partnering with them upstream in our program and process design, it saves time and resources by decreasing later rework to better meet their needs.

9 Readiness Assessment Planning Engagement Follow-up Assess needs and readiness of staff requesting patient and family advisors: What are we trying to learn? What are we ready to hear? Who are we trying to learn from? Who are the stakeholders involved? Where might we gather input? What are we currently doing to gather input? How do we want to gather input? When do we want to gain input?

10 Readiness Assessment Planning Engagement Follow-up Prepare for successful partnership: Define the patient and family participant role. Identify and include key stakeholders. Recruit patients, families and/or community members for formal screening/interview process. Coordinate meeting logistics.

12 Readiness Assessment Planning Engagement Follow-up Prepare staff and advisors to work together with the following guidelines: 12 Clearly define roles of stakeholders and PFPP advisors. Display a positive and supportive attitude toward the mission of Allina Health. Respect the perspectives of others by listening to differing opinions and different points of view. Foster a safe and supportive environment for open dialogue. Show concern for more than one issue or agenda. Work in close partnership with others. Allow the discussion to take a different direction than anticipated.

Share the input, which includes willingness to share learnings more broadly across the system. Create an action plan for using the input. Report back to participants and families on use of the input.
