5 Community Resource Guide This guide is provided for your use by the Montgomery County Drug-Free Coalition. Its purpose is to let you know about what substance abuse services and resources are available in Montgomery County. This guide does not represent a complete listing of service providers in our community. 7 Alateen, Miami Valley P.O. Box 282 Dayton, OH (937) Website: Services: Alateen is a support group to help young people, usually teenagers, recover from the effects of someone else s drinking. Hours: There are Alateen meetings all around the Miami Valley. See Website or call for a list of meeting locations. Eligibility: Anyone who is troubled by or whose life has been affected by someone else s drinking. Costs / Payment Options: None. Daybreak 605 South Patterson Boulevard Dayton, OH (937) Website: 24 Hour Crisis Hotline Toll Free: (877) Services: In addition to providing the basic necessities, such as food and shelter, Daybreak provides counseling, anger management training, violence prevention education, and life skills training that will help ensure a stable return home or a safe transition to a new home. Whenever possible, Daybreak works with the entire family. Youth enrolled in school attend their home school, and Daybreak works to enroll those who are not in school. Hours: 24 hours a day, seven days a week. Youth may reach Daybreak via the 24-hour crisis hotline. Eligibility: Any runaway, homeless, abused, or neglected youth between the ages of 10 and their 19th birthday in Montgomery County is eligible to receive emergency shelter services with the following exceptions: 1) youth with a known history of repeated or extreme violence or sexual offenses, 2) known predators of younger children, 3) youth offenders requiring a secured, locked facility, and 4) youth who have a history with Daybreak and whose files indicate that they are inappropriate for readmission. 8 Services: Children & Adolescent Outpatient Services: Provides therapeutic counseling, consultative, educational and supportive services to both children and their families who experience behavioral, mental or emotional and/or substance abuse issues related to their everyday living. Hours: Monday & Friday: 8:00 a.m.-5:00 p.m. Tuesday, Wednesday, Thursday: 8:00 a.m.- 7:00 p.m. Sojourner Program: Monday Friday: 7:30 a.m.-8:00 p.m. Eligibility: See Services. Costs / Payment Options: Costs are set for all clients except those who are eligible for free services. Clients are eligible for fee subsidies to cover the cost of services. The amount of the fee subsidy is based on family size and gross monthly income. Eastway Behavioral Healthcare: Northcutt Residential Treatment Center Eastway Behavioral Healthcare 4950 Northcutt Place 600 Wayne Ave Dayton, OH Dayton, OH (937) (800) or (937) Website: Services: Initial assessment, individual treatment plan, regular individual, family, and group therapy sessions along with individual and group chemical dependency counseling, if needed; psychiatric and nursing services; a structured program that includes recreation, outdoor activities, and socialization; and on-site educational services. Hours: 24 hours a day, seven days a week. Eligibility: Elementary School-aged boys who are victims of trauma (physical, sexual, or emotional abuse; neglect; witnessed the abuse of family members, etc.). Costs / Payment Options: Northcutt Residential Treatment Center is a Title IV-E eligible facility. Room and Board expenses are typically funded by a Children Services Board, Juvenile Court, or a Family and Children First Council. Clinical Services are funded through Medicaid. Eastway Behavioral Health Care: Webster Street Academy and Family Center 1110 Webster Street Dayton, OH (937) Website: Services: The Webster Street Academy is a school program with a mental health component. The Academy provides mental health needs assessments, treatment recommendations, and a plan that addresses these identified needs; educational curriculum; social skills training; coping skills training; community psychiatric supportive treatment; and psychiatric services. The Academy partners with area school systems. The Family Center provides outpatient individual therapy and offers children, adolescents, and - 6 -

9 families the opportunity to learn the skills to cope with mental health issues. Hours: Monday-Friday: 8:30 a.m.-3:00 p.m. Academy Morning Program: 9:00 a.m.-noon. Academy All-Day Program: 9:00 a.m.-2:30 p.m. Eligibility: Webster Street Academy: Students in 7th through 12th grade (12 17 years old) who are at-risk of being suspended or expelled from school and are experiencing emotional problems that interfere with school functioning. Family Center: Youth with mental health issues. Costs / Payment Options: Webster Street Academy and Family Center: Government insurance (Medicaid, Caresource, etc.) Montgomery County Juvenile Court: 10 & Under Program 380 West Second Street Dayton, OH (937) Website: Services: A specialized intervention/prevention program designed to keep non-adjudicated youth under the age of ten from penetrating further into the juvenile justice system. The program addresses dependency, abuse, and neglect issues; delinquent and unruly behaviors; and mental health issues. Services include: counseling, anger management, specialized diagnostic assessments, and case management. Hours: 24 hours a day, 7 days a week. Eligibility: Youth ages ten and younger who have been referred to Juvenile Court for allegedly being unruly or having committed serious felony offenses are eligible to participate. Youth without charges (walk-ins) can also be served by the program. 11 Military and Veterans Crisis Line (800) Press 1 Text: Website: Services: The Military and Veterans Crisis Line connects Service members (active duty, Reserve, and Guard) and Veterans (including their families and friends) in crisis with qualified, caring Department of Veterans Affairs responders through a confidential, toll-free hotline; online chat; or text. Support for deaf and hard of hearing individuals is available. Hours: 24 hours a day, 7 days a week. Costs / Payment Options: Free to all Service members, including members of the National Guard and Reserve, and Veterans, even if you are not registered with the U.S. Department of Veterans Affairs (VA) or enrolled in VA health care. Samaritan Behavioral Health Inc. CrisisCare Elizabeth Place 601 S. Edwin C. Moses Blvd. Dayton, OH (937) Website: Services: Emergency intervention and assessment services include: 24-hour suicide prevention and crisis counseling telephone hotline, 24-hour supportive telephone counseling service, evaluation of crisis needs, emergency walk-in care, pre-hospital screening, crisis counseling, emergency psychiatric care/medication, alcohol/drug assessments, Severely Mentally Disabled (SMD) assessments, care coordination, and Medication Assisted Treatment (MAT) services for opioid addiction. Hours: 24 hours a day / 7 days a week. Often, access for Severe Mental Health and Alcohol/Drug Diagnostic Assessments is available Monday through Friday. Eligibility: CrisisCare provides services to any adult or youth in Montgomery County who needs crisis services or evaluation for hospitalization. Costs / Payment Options: Ability to serve people without insurance / income or with limited income

13 Day-Mont Behavioral Health Care, Inc Germantown Street Dayton, OH (937) (937) (Intake and Referral) Website: Services: Works with the person to coordinate and provide mental health and substance abuse services. Hours: Monday & Friday: 8:00 a.m.-5:00 p.m. Tuesday, Wednesday, Thursday: 8:00 a.m.- 7:00 p.m. Eligibility: Persons dealing with combined psychiatric and substance abuse problems are eligible to participate. Costs / Payment Options: Costs are set for all clients except those who are eligible for free services. Clients are eligible for fee subsidies to cover the cost of services. The amount of the fee subsidy is based on family size and gross monthly income. Eastway Behavioral Health Care: Integrated Dual Disorder Treatment (IDDT) 600 Wayne Ave Dayton, OH (800) (937) Website: Services: Integrated Dual Disorder Treatment (IDDT) for adults battling both a severe and persistent mental illness and a significant issue with alcohol and/or drugs. Hours: Monday Friday: 8:00 a.m.-5:00 p.m. Eligibility: Adults with no way to pay for services must be Montgomery County residents. Adults with Medicaid do not need to be Montgomery County residents. NOVA Behavioral Health, Inc. Integrated Dual Disorder Treatment 732 Beckman Street Dayton, OH (800) (937) (800) TTY/TDD Website: Services: Multiple therapeutic interventions that promote increased mental health stabilization, harm reduction, alcohol and other drug use, and individual responsibility for self and actions

14 Hours: Monday Friday: 8:00 a.m.-4:30 p.m. or by appointment. Eligibility: Men and women, 18 years of age and older who have a primary mental health disorder and secondary addiction issues are eligible to participate. Must be psychiatrically stabilized and must have a case manager and linkage with a mental health facility. Costs / Payment Options: Sliding fee scale or private health insurance. South Community Behavioral Healthcare Integrated Dual Disorder Treatment 3095 Kettering Blvd. Moraine, OH (937) Website: Services: South Community Behavioral Healthcare works with the individual on sobriety, understanding his/her mental illness and substance abuse disorder, with an emphasis on community treatment. Services include pharmacological, psychological, educational, and social interventions. Hours: Monday, Wednesday, Friday: 8:00 a.m.-5:00 p.m. Tuesday, Thursday: 8:00 a.m.- 7:00 p.m. Eligibility: Individuals with co-occurring substance abuse and severe mental illness. Costs / Payment Options: Self-pay, Medicaid, Medicare, private health insurance, payment assistance: sliding fee scale. Wright State University Boonshoft School of Medicine Substance Abuse Resources & Disability Issues (SARDI) Consumer Advocacy Model (CAM) Integrated Dual Disorder Treatment 6 South Patterson Blvd. Dayton, OH (937) Phone/TTY Website: Services: Integrated treatment includes a continuum of services, all of which address mental health and substance use as interacting, concurrent disorders. The same care provider delivers treatment for both the mental illness and substance use disorders at the same time. This treatment provider assumes responsibility for integrating mental health and substance abuse treatments so that the interventions are selected, modified, combined and tailored for the specific consumer. Box 282 Dayton, OH (937) Website: Services: A support group to help family, friends, and young people, usually teenagers, recover from the effects of someone else s drinking. Hours: There are Al-Anon and Alateen meetings all around the Miami Valley. Meetings are held every day of the week with daytime and evening choices. Eligibility: Anyone who is troubled by someone s drinking, grew up with a problem drinker, or whose life has been affected by someone else s drinking. Costs / Payment Options: None. Families of Addicts Support Group Lutheran Church of Our Savior 155 Thurston Blvd. Dayton, OH (937) (Lori Erion-Organizer) Website: Services: A support group for families and friends of addicts, which includes the addicts themselves if they so desire. Hours: Meetings are every Wednesday from 7:30 p.m.-9:00 p.m. Eligibility: Anyone whose life has been affected by the disease of addiction. Costs / Payment Options: None. 19 Drug Free Action Alliance (DFAA) 6155 Huntley Road #H Columbus, OH (614) Website: Services: Advocates for alcohol, tobacco, and other drug issues. Some of their programs include: The GAP Network of family engagement groups seeks to end the crisis of substance abuse, specifically prescription drug and opiate addiction in all Ohio communities. Know! empowers parents of 11- to 13-year-olds to raise children who are alcohol, tobacco and drug free. Ohio Youth Led Prevention Network consists of youth-led substance abuse prevention providers and youth who are committed to the cornerstones of youth-led and peer prevention, positive youth development, and community service. Buzzkill is a program to help colleges educate their students on the responsibilities of social hosting. Parents Who Host, Lose the Most is a public awareness campaign to educate parents about the health and safety risks of serving alcohol at teen parties and to increase awareness of and compliance with the Ohio Underage Drinking Laws. Drug Free 24/7 is a social marketing campaign of prevention messages for communities, schools, and organizations that promote drug-free lifestyles and facilitate the expectation of living drug free 24/7. Partnership for Drug-Free Kids 405 Lexington Avenue New York, NY (212) Website: Services: (Web-based) The Partnership for Drug-Free Kids is a drug abuse prevention, intervention, treatment, and recovery resource, existing to help parents and caregivers effectively address alcohol and drug abuse with their teens and young adults. The Parents Toll-Free Helpline is a nationwide support service that offers assistance to parents and other primary caregivers of children who want to talk to someone about their child s drug use and drinking. The Helpline is not a crisis line. Requires Veterans to remain in the Dayton VAMC Domiciliary residential care facility. Dual Diagnosis Treatment Program (SARRTP): A domiciliary residential program (up to 2 months) designed to assist Veterans who are experiencing substance use disorders difficulties along with other diagnosed mental health problems. Opiate Treatment Program (OTP): An outpatient methadone maintenance/treatment program. Outpatient Care: Includes an assessment of medical/psychosocial needs, individual/group counseling, and educational sessions. Aftercare: A support service (after completion of residential / outpatient care) for individuals pursuing a sober lifestyle. Ambulatory Detox Treatment: Provides uncomplicated detoxification treatment from alcohol or other substances. 23 Wright Patterson Air Force Base Alcohol and Drug Abuse Prevention and Treatment Program (ADAPT) 307 Red Bud Lane, Building 830 T1 Dayton, OH (937) Website: Services: Level One outpatient treatment program. Services include: substance abuse assessments; individual, group and family treatment; and substance abuse prevention and education. Substance Abuse Treatment (Group/Individual) services are provided to individuals (TRICARE beneficiaries) who are addressing substance abuse or dependency issues. Referrals for those needing a higher level of substance abuse care will also be provided. Hours: Monday Friday: 7:30 a.m.-4:30 p.m. (closed all federal holidays and down days) Eligibility: Active Duty all branches, guard/reserve (on active orders), dependents of deployed Active Duty members, retirees and dependents (space available), and DoD civilians (space available) are eligible to participate. Heroin Helpline (937) Services: Concerned family members, friends, co-workers and people combating substance abuse can call to talk to an expert from the Montgomery County Alcohol, Drug Addiction, and Mental Health Services (ADAMHS) about the next steps toward recovery. Hours: 24 hours a day, seven days a week. Eligibility: Determined at the provider level. Costs / Payment Options: Treatment options are available for private, Medicaid/Medicare, and indigent consumers. Samaritan Behavioral Health Inc.: Project DAWN (Death Avoidance With Naloxone) Elizabeth Place 601 S. Edwin C. Moses Blvd. Dayton, OH (937) (CrisisCare) Website: Services: Project DAWN is a community-based drug overdose education and naloxone distribution program. DAWN kits are available to friends and family members of anyone

26 in Montgomery County who either regularly takes Opioid-based medication such as Hydrocodone, Fentanyl, Tramadol, Morphine or uses heroin. Hours: 24 hours a day / 7 days a week. Eligibility: Must be a Montgomery County resident. Some training is required. Call CrisisCare to sign-up for a DAWN training class. South Suburban Coalition-United for Alcohol and Drug-Free Youth C/O Dr. David Roer 5450 Far Hills Avenue Kettering, OH (937) (937) Website: Services: An eight-community organization advocating for our youth by promoting healthy choices and positive alternatives to reduce underage drinking and other drug use. The Coalition is made up of youth, parents, businesses, professionals, and other community/ school volunteers from Centerville, Kettering, Miami Township, Miamisburg, Moraine, Oakwood, Washington Township, and West Carrollton. United Way of the Greater Dayton Area HelpLink West First Street, Suite 500 Dayton, OH (937) (800) Website: Services: Provides assistance with navigating United Way s local nonprofit network. HelpLink employees listen to your needs, research all available service options, connect you with help, and follow-up as needed. Must attend doctor appointments and therapy sessions and pass screening tests for other drugs as part of the treatment. Hours: Varies. Call for information. Day-Mont Behavioral Health Care, Inc Germantown Street Dayton, OH (937) (937) (Intake and Referral) Website: Sojourner Program 904 Wilberforce Place Dayton, OH (937) Services: Adult Outpatient Services: For adults with mental health and/or substance abuse concerns. Provides individual/group, case management, couple/family counseling, and coping skills education. Dual Treatment Needs: For individuals dealing with combined psychiatric and substance abuse problems. May include Cognitive Behavioral Therapy and Motivational Enhancement Therapy. Children & Adolescent Outpatient Services: For children & their families experiencing behavioral, mental, emotional, and/or substance abuse issues. Provides therapeutic counseling; consultative, educational, & supportive services. Sojourner Program for Women: Clinicians provide a variety of services to assist women with developing a life free from substance abuse and regaining custody of their children. The average length of stay for the program is months. Children & Adolescent Services: The Webster Street Academy is a school program with a mental health component. The Academy provides mental health needs assessment, treatment recommendations and plan; educational curriculum; social and coping skills training; community psychiatric supportive treatment; and psychiatric services. The Family Center provides outpatient individual therapy and offers children, adolescents, and families the opportunity to learn the skills to cope with mental health issues.
