Interested firms may obtain a copy of the Request for Statement of Qualifications by contacting Dr. Michael F. Quatrini at the phone number or address listed below. Sealed Statements for Qualification will be received by the School District until 4:00 p.m. on March 31, All Statements of Qualification must be delivered by hand, or mail to the School District at the following address: Tornillo Independent School District P.O. Box Cobb St. Tornillo, Texas Phone: (915) The School District reserves the right to waive any formalities, technicalities or deadlines, and to reject any or all Statements of Qualifications. The School District also reserves the right to verify the accuracy and completeness of all responses by utilizing any information available to the School District without regard to whether such information appears in the Statement of Qualifications. By submitting a Statement of Qualifications, each architect agrees to waive any claim it has or may have against the School District, its trustees, agents and employees, and any persons or entities contacted as references, arising out of or in connection with the review, evaluation or recommendation of any Statement of Qualifications, the acceptance or rejection of any Statement of Qualifications, the waiver of any requirements in this Request for Qualifications, and any complaints or concerns regarding the award of contracts. 3. Contract Documents The School District will use the Standard Form of Agreement Between Owner and Architect AIA Document B Edition plus the School District s required supplemental provisions. Please note any exceptions or issues you would raise relative to this document. A copy of the School District s Supplemental Provision will be provided upon request. 4. Professional Liability Insurance. The School District will require all architects selected to maintain professional liability insurance with limits of not less than $1,000, Evaluation Criteria. The School District will use the following criteria with respect to evaluating statements for qualifications: Experience of Staff/Ability to Perform 30% Previous Experience with School Districts 25% Previous Experience with Similar Projects 25% References 15% Local Presence 5% 3

4 6. Special Conditions. a. No Lobbying. No firm submitting a Statement of Qualifications shall lobby, contact or solicit School District staff members or members of the Board of Trustees during the selection process. Any questions or concerns about the RFQ or requests for clarification shall be directed to the School District contact persons listed in the RFQ. b. Criminal History. The School District reserves the right to require any of the firm s representatives or employees who will be performing services at the School District be subject to a criminal history check and shall not have a criminal history which is objectionable to the School District. The firm selected shall submit appropriate information to the School District in compliance with Section et seq., Texas Education Code. RFQ. c. Deviations. List any deviations or clarifications from the terms and conditions of this d. Right to Reject. The School District reserves the right to reject any and all statements of qualifications, to negotiate portions thereof, and to waive any informality or mistake in a Statement of Qualification. e. Negotiations/Interviews. The School District reserves the right to enter into negotiations with any of the firms submitting Statements of Qualification and to schedule interviews with administrative staff and/or Board of Trustees as the School District deems appropriate. 7. Procedure for Submitting Statement of Qualifications. All Statements of Qualifications shall be submitted with an original and four counterparts by mail, , or hand delivery addressed to: Tornillo Independent School District P.O. 6 STATEMENTS OF QUALIFICATIONS STATEMENTS SHOULD CONTAIN THE FOLLOWING INFORMATION: A. General information such as point of contact, address, state registration and type of organization. B. A statement of qualifications of the firm s ability to complete this type of project. Include availability of personnel who will perform the work, current and former clients, and evidence that the firm is capable of performing the duties as described above including any unique qualifications. Indicate and list previous experience and results of similar projects built within budget and schedule constraints. Provide name of client(s), name of project(s), cost of construction for each, overall square footage of finished project(s) and client contact with current phone number. C. Provide the firm s background, demonstrating current workload and available personnel. List currently available staff for each of the following: Firm Principal, Design Architect(s), Contract Administrator(s), and Drafter(s). D. Identify the professional services and disciplines your firm can provide. Provide a list of the engineering and special services consultant firms which you would retain for the project. The School District reserves the right to accept or reject any or all consultants to the architects. E. Provide proposed timeline for programming and design services. F. Indicate ability to secure Professional Liability Insurance, indicating maximum amounts of insurance available. G. List references for similar projects that represent your firm s work and services provided. H. Provide signature, date and title by principal of your firm. I. The notice must include a general description of the conduct resulting in the conviction of a felony. Subsection (b) states a school district may terminate a contract with a person or business entity if the district determines that the person or business entity failed to give notice as required by Subsection (a) or misrepresented the conduct resulting in conviction. The district must compensate the person or business entity for services performed before the termination of the contract. This does not include any attendance at any meeting scheduled by the School District such as a pre-bid conference, or any contact with the Interim Superintendent or General Counsel for the purpose of obtaining clarification regarding any of the procedures or requirements. The District reserves the right to disqualify any firm that violates this policy.
