2 Description of our Program Welcome to Donor Egg Bank USA. For many individuals, using eggs from a donor is a positive and effective means to moving down the path to parenthood. Our goal is to help guide you along the way and provide you the cost of your treatment in advance. Following are the steps in our process Consultation and pre-screening with a Reproductive Endocrinologist. Create an account on our website at to review our donors. Determine both clinically and financially if frozen donor eggs is the best option for you. Choose a financial plan: Single Cycle or Assured Refund. Contact us to request the necessary contracts (240) Return signed copies of our contracts to initiate the selection process. Select an anonymous egg bank donor. Satisfy all outstanding balances due. Start cycle with your Fertility Practice and Doctor. Benefits of Frozen Donor Egg Treatment Convenient Our donor eggs are frozen for your immediate use so you can cycle at a time that works best for you. Certainty Frozen donor eggs give you certainty that your cycle will not be canceled because of a donor who fails pre-screening, has a low ovarian response or unexpectedly has a life event. Successful Our success rates are nearly as good as traditional cycles and with our 100% Assured Refund Plan you can have peace of mind, that if you do not deliver a baby you will receive a full refund. Affordable In most cases, frozen donor egg is more cost effective than a traditional fresh cycle because the fixed costs associated with a donor egg retrieval is shared across more than one lot of eggs. Biological Siblings If available you may obtain additional egg lots for future attempts at having full biological siblings. 2

There is no refund if you are unsuccessful in delivering a baby. If unsuccessful, expenses spent in a single cycle plan may not be applied towards an Assured Refund Plan. Assured Refund Plan The Assured Refund Plan provides a 100% refund if you do not deliver a live born baby within 6 donor egg IVF cycles. There is no obligation to complete all 6 cycles and at any time you may choose to end treatment and still receive an 80% refund. We estimate that over 85% of our patients will qualify for the Assured Refund Plan. To qualify, the primary factor assessed is normal uterine function. The Assured Refund Plan provides excellent financial 3

4 protection if you do not have a successful delivery. If a baby is delivered on the first cycle, you will have paid approximately twice the amount that a Single Cycle Plan would have cost. If a successful pregnancy occurs on the second cycle the costs are nearly equal to the Single Cycle Plan. For our patients who require 3 cycles of treatment or more, the Assured Refund Plan will be less expensive than if they had paid for each cycle individually. Patients choosing the Assured Refund Plan have peace of mind, that should they not deliver a baby, they will receive a 100% refund. The Assured Refund Plan offers our patients 6 frozen donor egg bank treatment cycles. If you have cryopreserved embryos from any of your 6 attempts, you will be required to use the frozen embryos before moving on to another treatment cycle. However, the frozen embryo cycle will not count against one of your six attempts. Each cycle is subject to a cycle review and on-going participation is not guaranteed. If during your treatment, your physician or Donor Egg Bank USA deems you ineligible to continue or perhaps a different course of treatment is recommended, we will issue you a 100% refund. If you choose to stop treatment on your own accord, we will issue you an 80% refund. Our Assured Refund Plan is unlike other Egg Bank programs, which may only give you a portion of your money back if you are unsuccessful. We know that your dream of having a baby isn t complete, until your baby is tucked safely in your waiting arms. If you do not realize your dream, we have not met our commitment to you. Assured Refund Contingent The great thing about a frozen egg cycle is often times a mock or pilot cycle is not needed since the eggs are not thawed if the female s uterine lining is not sufficient. Individuals who meet the requirements of Assured Refund and who have not had a cycle on exogenous estrogen (FET, Mock or Pilot) cycle may be accepted as contingent on a good lining during the actual cycle. Assured Refund Exclusions Individuals who intend to use PGD, select embryos for family balancing, and/or not thaw all of the eggs in a cycle would not be acceptable candidates for this program. Cryopreservation Cryopreservation expenses vary and we encourage you to request information from your fertility center regarding both the cost of cryopreservation and subsequent storage fees. Cryopreservation is excluded in the single cycle plan. Cryopreservation is included in the Assured Refund plan. Individuals who elect to participate in Assured Refund are required to utilize the cryo-preserved embryos before beginning another egg bank cycle. Medication Recipient medications are excluded in both the Single Cycle Plan and the Assured Refund Plan. Recipient medication is estimated to be between $400 and $900 per cycle. 5 Program Fee Due Dates The full cycle fee and applicable supplemental agency fees are due within 2 business days of selecting your donor or being accepted into the Assured Refund Plan. Please contact our Financial Department at (240) to make a payment. Supplemental Agency Fees In our effort to offer you a diverse database we have worked with several reputable agencies to identify, cycle and freeze eggs from a diverse portfolio of donor choices. If we used an agency to cycle a donor we have passed those costs on to you in the form of a supplemental agency fee. If an additional charge applies, it can be found on the donor s profile listed as supplemental fee. The supplemental fee is charged to you, each time that donor s eggs are chosen regardless of the financial plan you choose. Cancellation of Donor Selection Once you make a donor selection, we hold the match for 3 business days prior to processing the eggs for shipment. Should you change your mind regarding your choice, please contact us immediately at (240) within the 3 business day timeframe. Once the eggs are shipped, cancellations for non medical reasons will incur a $4,500 re-stocking fee and payment to the practice for services rendered. Shipping After we have received your payment and documents, please allow two weeks for shipping to occur. Eggs are shipped UPS priority overnight delivery and will be sent directly to your lab. Cycle Guarantee We guarantee you mature eggs available for sperm insemination. Using quality sperm, you will have at least one normally cleaving embryo available for transfer on day 3 or we will provide you a new treatment cycle. If you are enrolled in the Assured Refund Program, the cycle will not count against one of your 6 tries. Eggs for Future Use A full egg lot is $12, USD plus an annual storage fee. Should it apply, supplemental agency fees would be in addition. Should you decide to relinquish your reservation, we will consider a 50% refund of fees paid within 3 years as long as the eggs have been maintained at a network fertility practice and all payments are current. 5
