1 3D Hybrid Operating Room After refurbishing our hosting and office area in 2012 and investing in a larger and improved animal facility (still on-going), we decided to invest in the most advanced 3D hybrid operating room in the market. Being one of the most experienced labs in hybrid procedures for structural heart disease made it clear that we needed imaging that could address the legitimate questions of our customers in terms of anatomy and functionality. We have made significant progress in our understanding of how to best approach valvular or vascular trans-catheter and hybrid therapies but also how to best validate orthopedic or neurologic implants. We try to keep up with the most recent advances in imaging because we believe this imaging is part of the equation for good research. There is no doubt that it also contributes to convincing fellow researchers, physicians and last but not least, investors. 2 Recent development of trans-catheter treatment of valvular disease has brought cardiac surgeons and interventional cardiologists to perform hybrid approaches in a single theater, so called "hybrid room", being both a high-end catheterization laboratory and a state of the art operating room (OR). The contribution o IMM in the development of stented percutaneous or minimally invasive treatment of structural heart disease over the past 15 years has fuelled the need for a hybrid room for preclinical research. International teams and research groups, start-ups and large players alike, come to our laboratory in this high tech environment in order to validate novel hybrid strategies for aortic and mitral valve disease. For thi major step forward R decided to partner up with Philips Healthcare. Truth be told, the choice with the two other large players in the world of hybrid rooms could have been interesting options but beyond technical aspects (optimal spacing within our walls, quicker results for 3D, high end imaging) the partnership we established with Philips several years ago with the IE33 ultrasound machine was strengthened for the acquisition of the hybrid OR. Once we decided upon whom to work with, a 5-month working plan was established and the works were completed in September The room has been actively used ever since. This hybrid OR comes with everything common for surgery - great LED light source, state of the art radiotransparent operating table, air renewal, electric supply compatible with electrocautery, etc., and a floormounted rotational angiography system called the Allura FD 20 (large flatpanel). It really has the best of both worlds. It has 3D reconstruction capabilities. But the rotational view, without reconstruction, is already very useful. Imaging quality is, at any rate, remarkable. Also, there is a live broadcast capability with a direct feed in the meeting room with interactive microphone/audio. In that meeting room, a split screen can be viewed with live and reference images of the Allura, 3D reconstruction or ultrasound images, general OR camera and specific surgical camera. Everything can be recorded and used for a live-case or live-in-the-box. It is a remarkable teaching and marketing tool. Newsletter #15 of IMMR_MAY 2013_Hybrid OR Page 2 of 8

3 Hybrid procedures are very much a recent concept in the cardiovascular arena. The first hybrid percutaneous coronary angioplasties were performed along a conventional cardiac surgery in the 1990's. But the idea flourished in the early 2000's when transcatheter Aortic Valve Implantation (TAVI) became one of the hottest fields in interventional cardiology. This is where our room comes naturally as a wonderful research piece of equipment. But there are other medical specialties where this hybrid OR can be an insightful adjunct. Most and foremost, vascular surgery, interventional radiology, orthopedics and neurology. This room is simply one of the best a preclinical research lab could have and very few of us have such a high-end tool to explore and validate medical devices in clinically relevant animal models. We asked her a few questions to get an insider's standpoint on the potential of vascular/surgical operating theaters. Ms Crochet Hijazi, could you tell us what you do with Philips Healthcare, in a word, what is a "Modality Specialist ixr"? Philips Healthcare ixr Modality Specialist means specialist product line interventionnal X-Ray within Philips Healthcare. I m responsible for promoting the interventional X-Ray products & systems and maximizing customer satisfaction. What is your definition of a hybrid OR? The cathlab environment, with its high end X-ray system, trained interventional staff, and specialized consumables provides a superb infrastructure for endovascular treatment. The OR, with its high sterility, trained surgical staff, anesthesia and surgical equipment provides an excellent infrastucture for surgical treatment. When you combine both in a single room, the Hybrid Suite is born. What are, in your opinion, the key aspects that make the Philips hybrid room department the leader in its field? Knowledge of the Hybrid Surgery area and network of key opinion leaders in vascular, cardiac and neuro therapy Unique capabilities in advanced imaging (2D X-ray imaging, 3D X-ray imaging, Live 3D ultrasound, image fusion,...) Experience in building Hybrid ORs (>500 so far) The right solutions for the Hybrid OR (FlexMove, integrations,...) Can you share any insight into what the future of a hybrid OR could be? What are the innovations we will see in the up-coming five years? Further technology developments in advanced imaging capabilities. New image fusion and image guidance tools. Further improvements with regard to user interface and workflow. Simplifying the access to the advanced functionality in the Hybrid OR. Adaptations to new clinical areas. Neuro-, spine and orthopedic surgery. New applications in urology, oncology. Why did you decide to partner up with IMMR and what are the potential benefits for Philips? We decided with IMMR to implement the most effective Hybrid Operating Room in a preclinical research laboratory. The aim is to imagine, think and build together the future of interventional hybrid rooms in terms of ergonomics, evolution of multi-modality fusion tools, integrations, new interventional tools.
