2 Introduction The debate on how technological innovation fosters collaboration and interdependence in healthcare sector is gaining increasing attention in the scientific literature and in policy arena. Several works have emphasized how the introduction of technological innovations in healthcare sector impacts and disrupts existing boundaries and routines between professionals Starting from Barley s pioneer work, many scholars have stressed the importance of organizational adaptation to the new technology during its implementation process and the role of interaction and multidisciplinarity as determinant to reap the benefits of new technology implementation. To date, the attention of scholars into the identification of organizational patterns that can foster technological innovation s users towards collaboration and multidisciplinarity has been mainly focused on human resource. 2

3 Theoretical framework Since multidisciplinarity requires a deep change of production processes and organizational models, it is necessary to expand the lenses and use a system-wide approach: ORGANIZATION Production processes Operations management TECHNOLOGICAL INNOVATION Organizational structure and support systems MULTI- DISCIPLINARITY Human resources Governance structure Objective: to test the theoretical framework of system wide approach in investigating the multidisciplinarity triggered by technological innovation in health care 3

4 Methods Technological innovation- Transcathetic Aortic Valve Implantation (TAVI) - arguably the fastest-growing technology in cardiology ( alternative to traditional surgical aortic valve replacement in carefully selected patients) 1. a technology that challenges routine and calls for a multidisciplinary approach to be successfully implemented in the clinical practice 2. relevant for its potential impact on organizations 3. innovative but sufficiently diffused in order to have a higher number of case studies to choose from. TAVI poses both a technological challenge for the cardiologists and cardiac surgeons that must learn the new procedure, and an organizational challenge for the operating room team which must learn a to work in a multidisciplinary team (disruptive technology). Multiple case study: 4 Italian hospitals among high adopters, selected by theoretical sampling strategy in which we sought variance of organizational dimensions (size, ownership, level of specialty). 4

6 Results (2) 3) & 4) Organizational structure & Human Resources Given a strong multidisciplinary connotation, TAVI appears to be an example of technology that provides incentives for departmental structure of the hospitals (Cardio Thoracic Department that includes Cardiology and Cardiac Surgery) Some of the hospitals analyzed are also considering the creation of a functional Organizational Unit for TAVI with the goal to identify and formalize standard clinical pathway and guidelines for TAVI Procedures (standardization) and promote knowledge sharing among professionals involved in the procedure Although all case studies adopted similar HR management tools to foster multidsciplinarity by creating ad hoc teams, the degree of collaboration and thus success of those tool svaried across hospitals. An effective collaboration between two specialists is of pivotal importance for fostering successful multidisciplinarity, also between the other key operators involved in TAVI implementation 6

7 Conclusions All 5 dimensions identified in the framework were involved in the implementation of the new technology TAVI, although the degree of involvement varied extensively across four hospitals It seems that, in all the four cases analyzed, the physical lay-out of hospital production units did represent an obstacle for multidisciplinarity. In the hospital in which model dimensions were actively managed, the outcome was tangible in terms of positive working environment and overall organizational efficency/effectiveness In the hospitals in which some dimensions were left unmanaged, conflicts and tensions between the two specialties were evident at various levels ( turf war ) The proposed theoretical framework of system wide approach appears suitable for investigating multidisciplinarity triggered by technological innovation 7
