2 Rockaway Volunteer Ambulance Search and Rescue Corps Mission Statement The Rockaway Volunteer Ambulance Corps (R.V.A.C) mission is to render first aid and pre-hospital care to those that are sick and injured and deliver the most efficient ambulance response times and provide excellent pre-hospital patient care thorough excellence in training. With the latest equipment and advance technology in conjunction with strong leadership, team building, professionalism and continuous quality improvement. Vision Statement To render care to anyone within our operational territory with professional high level of care and transport them to the hospital. List of all Existing Services in the proposed area; FDNY EMS ( 911 Service) First Response Ambulance Hunter EMS; RCA EMS Transcare EMS Midwood EMS North Shore EMS Chevra Hatzalah EMS Primary Care EMS Currently in the Rockaway-Peninsula area there is FDNY who provide solely 911 BLS and ALS emergency services. The high response by FDNY EMS are hurting the community by delaying the care that the sick and the injured need. The proposed service will help in a positive way by providing extra 911 emergency care to the community and help reduce the high response times. It will also help by training the community in different areas of health such as first aid, CPR, and community events that will increase community involvement to help build trust within the community. All other listed services mainly provide 24/7 nonemergency transport and are licensed to operate in all of NYC. The proposed service will not negatively affect or financially affect any of these services because the proposed service will only operate in the Rockaway- Peninsula area. 3 Certificate of Need within the Rockaway Peninsula Area Rockaway Peninsula has over 210,000 residents. During the summer season the Rockaway Peninsula has over 7.5 million visitors enjoying the attractions that the Rockaway-Peninsula has to offer. Rockaway VAC has reached out to local businesses in the area and they will welcome a volunteer ambulance service to the community. This proposed area is currently covered by the Fire Department of New York EMS, The Fire Department, as great as they are is unable to cover in a timely matter our large diversity of tourist and resident. Rockaway had suffered greatly with medical needs in the community. In 2012 the Peninsula Hospital Center (PHC), formerly known as Peninsula General Hospital, had closed its doors to the community leaving our only hospital to be St. John s Episcopal Hospital Center located on Beach 20 th street to cover our large population. In the same year the Rockaway Peninsula had another tragedy the destroyed the homes and businesses of many in the community this was Hurricane Sandy. Hurricane Sandy (unofficially known as "Super storm Sandy") was the deadliest and most destructive hurricane of the 2012 Atlantic hurricane season, and the second-costliest hurricane in United States history. This along with the closing of one of our two hospitals that served our huge population brought the response times of a city ambulance to an all-time high. In 2014 the average response times for a city ambulance was 6-8 minutes citywide but minutes in the Rockaway-Peninsula according to community residents and also providing that weather and other condition was alright. The Rockaway Volunteer Ambulance Corps (R.V.A.C) plans on helping alongside the NYC Fire Department EMS with reducing the average time it takes an ambulance to get to a sick or injured person in our community. Area of Operation R.V.A.C will operate from the Rockaway/Nassau Boarder line to the Marine Parkway Bridge and From Beach to Bay in the Rockaway Peninsula Area. Community Board #14 ( Queens) City Council Districts # 31 & th & 101 st Precincts State Senate Districts # 10 & 15 ROCKAWAY VOLUNTEER AMBULANCE SEARCH AND RESCUE CORPS 3
4 New Service Description Staffing and Level of Care R.V.A.C will staff members in the following levels: NYS Certified First Responders (CFR) NYS Certified Emergency Medical Technicians- Basic (EMT-B) R.V.A.C BLS crew will only render appropriate treatment, which includes First Aid, CPR, Epi Pen, Albuterol- Nebulizers, Oxygen, Nasal Narcan (naloxone) and AED. RVAC will stock as required as per Article 30 part RVAC will operate (1) ambulance during the startup of this project and operate more as the project grows. Although the Rockaway-Peninsula is a large area for one ambulance, Rockaway VAC ultimate goal is to operate 4 or more ambulances in the community. We will operate these ambulances in the Rockaway- Peninsula section of Queens in the city of New York in the State of New York. RVAC has the writing consent of experienced CFR s, EMT s and Paramedics who has interest in volunteering their time to staff the new volunteer ambulance corps. RVAC will staff each ambulance with the minimum of (1) Driver & (1) NYS Certified EMT/Paramedic. The squads will follow strictly NYC REMSCO protocols. Each member before volunteering and able to ride on the ambulance will need to complete and pass a NYS CPR & First Responder course. Each member who has interest in driving our fleet will need to go to training and get certified in EVOC/CEVO. RVAC will operate 24 hours a day, 7 days a week, 365 days a year. RVAC will operate at a Certified First Responder (CFR) and Emergency Medical Technician- Basic (BLS) levels only. Each EMT-B would be certified by NYS. Each Ambulance will have at least one EMT-B with 2 years or more experience in charge of the crew and maintaining emergency care to the patient. The Rockaway- Peninsula has over 210,000 resident and in summer of 2015 had over 7.2 Million Visitors & Tourist according to The Parks Dept. & MTA statistics which proves that Rockaway-Peninsula has a large area to support a new ambulance service. The proposed area is large and tends to get more than 40 calls per day, depending on the season which proves that the area is large enough to maintain a full service. RVAC projects to respond to per year. Residents, tourist, and visitors can activate RVAC emergency system by simply dialing calling our hotline that we will have connected directly to a trained dispatcher. When RVAC gets a mutual aid agreement with FDNY EMS that is currently being worked on, Rockaway VAC can also be activated by dialing 911. RVAC is currently in the process of obtaining a MARS number from FDNY and assisting in call request. Our goal is to ultimately reduce the current response time by the city from minutes in Rockaway down to 4-7 mins from time of call to patient contact. RVAC will be an improvement to public convenience and help cover the medical needs that is needed in our community. ROCKAWAY VOLUNTEER AMBULANCE SEARCH AND RESCUE CORPS 4
5 Equipment Storage Rockaway VAC is working with multiple sources to obtain an official EMS base station. Until RVAC finds an official base equipment such as 4x4, glucose, ETC will be stored in a 24 Hour secured storage facility located on Beach 90 th street in Rockaway Beach. RVAC ambulance(s) will be temporary stored in a secured locked lot when not being operated. Mutual Aid RVAC is working to set up mutual aid and work along FDNY if needed and request ALS in time of need. RVAC is also seeking to have a mutual aid agreement with another EMS agency that s license to operate in the current territory. List of Hospitals to receive our patients St. John s Episcopal Hospital Jamaica Hospital North Shore LIJ Brookdale Hospital Coney Island Hospital ** Also we will transport patients to special hospitals according to protocols** ROCKAWAY VOLUNTEER AMBULANCE SEARCH AND RESCUE CORPS 5
6 Training and Continuing Education RVAC members will have the chance to complete NYS CME training and skills practice. Members will have the chance to complete NYS certified EMT course which has been approved by NYS if been active volunteer. When members are not on calls they will maintain they level of skills and training but preforming scenarios as if they was on real calls with a NYS EMT with experience and/or NYS CIC s. Member will also get the chance to renew their certification by attending CME s or a NYS refresher course sponsored by the state. Communications RVAC will have a general office telephone number that the public can contact RVAC in case of an emergency. RVAC will work with NYC FDNY-EMS and receive a unit number and radio and respond to calls. Each ambulance and fly car will be equipped with on board radios and portable radios to communicate with dispatch. Each unit will have Nextel radios to communicate with OLMC. RVAC will be a positive addition to the current communications in place. On-line Medical Control RVAC Online Medical Control will be: Dr. Joseph Zito Located at: 5 Beaver Drive, Locust Valley, NY Written approval of medical control for participation in this proposed service is attached. As per PHL Article 30, Section 3006, Rockaway VAC will participate in the QA/QI program. Physician: Dr. Joseph Zito QI Coordinator: EMT-P Stephen Davis As per NYS EMS policy Rockaway VAC will document every patient contact on the NYC REMSCO approved EPCR and submit the EPCR to NYC FDNY as required by each agency monthly. 7 Epcr &Storage of Medical Records Rockaway VAC will use a service to provide us with Epcr s. Rockaway VAC will notify REMSCO of service and retain their approval before use. All medical records will be stored within a secured online service and members are not allowed to access these records. Only the President, Vice President and on duty supervisor will have access to stored and locked medical records. Access to the medical records can be obtained by written notice of patient or patients proxy unless subpoena by the court of law. EPCR s will be viewed quarterly for QA/QI and for training and education of members. Fiscal Stability Rockaway VAC will bill the insurance of the patient being treated/transported to maintain operations. Rockaway VAC will use a professional billing service and will NOT seek payment for those patients who does not have insurance. Rockaway EMS will also run on the support of local elected officials who vowed to support this project. Rockaway VAC will also apply for capital, state, city, and private grants. We will also solicit donations from community resident to keep operational. (Please see budget attached) ROCKAWAY VOLUNTEER AMBULANCE SEARCH AND RESCUE CORPS 7
8 Budget Details Start Up Budget Rockaway VAC has received a grant from Williams Rockaway community grant in the amount of $15,000. Williams grant gives Rockaway based nonprofits up to $15,000 twice a year. Rockaway VAC will apply for these grants and according to Williams there is a 99% chance Rockaway VAC will receive the grant. Link to website: Rockaway VAC received $15,000 from community supporters from one part of the community and plans to reach out to the full community. Rockaway VAC has been approved for a credit line of at least $20,000. Budget for First Year Rockaway VAC and its professional billing company project that if Rockaway VAC does at least 1100 transported calls per year. Its projected revenue is $470,000- $500,000 per year. (Letter Attached from Billing Company) Rockaway VAC also has attached letters of support from local politicians that vowed to assist in financial need of Rockaway VAC through reimbursement. Rockaway VAC has received donations of $15,000 from one part of our community that has 4-5 areas. If RVAC fundraise in all location we project to get at least $50,000 from community residents. Rockaway VAC will apply for federal, state, city, and private grants. These grants each range from $20,000-$100,000. Rockaway VAC is currently applying for and receiving grants. 10 Rockaway Volunteer Ambulance Search & Rescue Corps Date: November 28, 2015 To: Regional Council of EMS of New York City Re: Rockaway Volunteer Ambulance Search and Rescue Corps Application Response 1) Rockaway-Peninsula is a large area for (1) Ambulance. Rockaway VAS has written In the application that it plans on adding more ambulances as the project grows. Rockaway VAS is a community ambulance services and shouldn't focus on one part of the community. RVAC should be approved for the territory that it applied for. So when it gets more ambulances it can just get inspected and start operating instead of reapplying again for more territory AND waiting longer and doing anothercon. 2) The call volume was corrected and incorrectly inputed maybe typo. We estimate the call volume in the community being applied for by the city is more than 1500 a year, which is equivalent to 4 calls a day. 3) Donations and grants are explained in the attached budget narrative attached to the updated CON APPLICATION.
11 4) ROCKAWAY EMS is working with local officials and the community hospital to establish a official base for ROCKAWAY EMS. More details added to the application. 5) The Directors on the Certificate of Corporation was not active and involved in the corporation and therefore been replaced by the newly listed directors on the IRS form. The directors that was removed has no stock,bonds, or ties with Rockaway VAC. 6) ROCKAWAY EMS is currently in talks with FDNY in establishing mutual aid and receiving a MARS NUMBER. REMSCO will be notified as soon as this agreement have been made. 7) Our medical director address is in Locust Valley. This is located about 25 Minutes from our territory. Our Medical Director is the Vice- Chairman of St. John's Hospital in Far Rockaway. He mostly spends his time at the hospital or at a location closely to the hospital which is in our operational territory. Dr. Zito has agreed to be available to the agency whenever needed. 2
