Meetings of the Regional EMS Council are recorded to comply with the Governor s Executive Order #3 to web cast meetings. A roll call attendance was conducted; a quorum was present. Committee members present were asked if they had comments, questions or corrections regarding the Minutes of the June 24, 2014 meeting. Hearing none, the Minutes were approved as written. Public Notices for the following Transfer of Operating Authority applications: Superior Ambulance transferred to Maimonides Medical Center (1) Competency and Fitness approval from NYSDOH received. Bi-County to Emergency Ambulance Service, Inc. (1) Competency and Fitness approval from NYSDOH received. (a) All applications listed above were posted on the NYC REMSCO webpage ( or ed directly to members. STAFF REPORT Nancy Benedetto, Executive Director Administration, reported that all contracts (Program Agency & REMSCO) have been renewed. She stated that she had a meeting with the new FDNY Commissioner Daniel Nigro and discussed the roles of the Regional Council and REMAC. Commissioner Nigro is a past member of the Regional Council. Emergency Preparedness Report: Joseph Raneri, Disaster Preparedness Coordinator Planning REMSCO continues to participate in ESF 8 workgroup and the DOHMH Pan Flu exercise planning meetings. Will be participating next week at RUMCSI at an MCI drill. Agencies may hear chatter over REMSCO radio during this time. Will also be doing a limited test with EMS agencies participating in the drill. REMSCO is working with NYC DOHMH to renew its regional POD plan. Page 4 of 9

5 The POD plan allows for distribution of prophylaxis medications to Volunteer and Proprietary EMS providers. Agencies will need to submit updated MOU s to REMSCO as part of the plan renewal. Agencies will be the new MOU s to complete. It s of utmost importance that those be returned. Will be setting up a training date to allow the Volunteer Districts use of the REMSCO Dialogics notification system. Will grant district heads the ability to send out notices to volunteer agencies within their districts. Will be working with District 4 to update agency contacts. REMSCO Radios REMSCO C2 Channel radio FCC inspections have been ongoing with most of Staten Island and Brooklyn completed. REMSCO will be coordinating with agencies and the vendor (AllTech) to schedule dates & times for these inspections to occur. During this process, any necessary repairs to malfunctioning radios will be done. Agencies with Radio issues must contact REMSCO and submit a Radio Service Request so work can be completed. Replacements of damaged radios from Sandy are complete except one site (Roxbury FD) which is still completing repairs of quarters. EPCRs REMSCO staff has been meeting with multiple E-PCR vendors and is seeking to develop a regional E-PCR system to help transitions into using E-PCR s. This regional system will be open to all NYC agencies who wish to participate. AMBULANCE COMMITTEE REPORT, Chair Joseph Marcellino, MPH The Ambulance Committee met immediately prior to REMSCO. No Applications were received for Determination of Need for a New or Expanding Ambulance Service. Two (2) new applications were received Transfer of Operating Authority Bi County Ambulance & Ambulette Transport Service Corp. (8147) dba Lifestar to Comprehensive Ambulance Service Bi County LLC dba Emergency Ambulance Service This is the transfer of a private ambulance corps to a private entity. The application has been made public via publishing in the NY Times, Public Notice flyers, and posting on the NYC REMSCO webpage. This comes in the form of a seconded motion from the Ambulance Committee: The Ambulance Committee of the Regional Emergency Medical Services Council of New York City finds, based upon the receipt of a complete application for Transfer of Operating Authority of Bi County Ambulance & Ambulette Transport Service Corp. (8147) dba Lifestar, to Comprehensive Ambulance Service Bi County LLC dba Emergency Ambulance Service, and a review of Competency and Fitness having been conducted by the Regional Emergency Medical Services Council of New York City and the New York State Department of Health; and that review having revealed no issues challenging Competency and Fitness, recommends that approval of the Transfer of Operating Authority of Bi County Ambulance & Ambulette Transport Service Corp. (8147) dba Lifestar, to Comprehensive Ambulance Service Bi County LLC dba Emergency Ambulance Service, operating authority within the County of Queens, be granted. Page 5 of 9

6 The REMSCO conducted a written vote as follows: 17 in-favor, 0 opposed, 1 abstention. The motion carried and will be submitted to NYS DOH. Superior Ambulance transferred to Maimonides Medical Center This is the transfer of a private ambulance corps to a hospital. The application has been made public via publishing in the NY Times, Public Notice flyers, and posting on the NYC REMSCO webpage. This comes in the form of a seconded motion from the Ambulance Committee: The Ambulance Committee of the Regional Emergency Medical Services Council of New York City finds, based upon the receipt of a complete application for Transfer of Operating Authority of Superior Medical Transportation, LTD, #0736, to Maimonides Medical Center, and a review of Competency and Fitness having been conducted by the Regional Emergency Medical Services Council of New York City and the New York State Department of Health; and that review having revealed no issues challenging Competency and Fitness, recommends that approval of the Transfer of Operating Authority of Superior Medical Transportation, LTD, #0736, to Maimonides Medical Center, operating authority within the Counties of Bronx, Kings, New York, Richmond, and Queens, be granted. The REMSCO conducted a written vote as follows: 15 in-favor, 0 opposed, 3 abstention. The motion carried and will be submitted to NYS DOH. Public Access Defibrillation Programs 75 New application received for June September Renewal applications received for June September 2014 Technical Advisory Group (TAG) Reports Mutual Aid TAG: The Committee asked that the Chevra Hatzalah MAA be sent out again. They also asked for an updated listing of MAA status for all volunteer agencies. The TAG Chair (Scott Orlanski) will be reaching out to agencies in order to obtain MAAs. The TAG will communicate via conference call and . Photo ID TAG: The ID printer is having software updates. The status of this equipment will be reported at the next meeting. NYC REMSCO is committed to providing security IDs. Security SPECIALTY Sticker TAG: Joseph Raneri When putting a new vehicle in service or replacing the windshield on a vehicle, agencies should be submitting for new and/or replacement security stickers. Security stickers identify vehicles that are part of the NYC Mutual Aid Mobilization System, and allows vehicles access to restricted roadways during citywide emergencies. The fee for regular ambulance sticker has been reduced to $ Received Special Sticker request from Volunteer Heart Ambulance Page 6 of 9

7 Mr. Raneri asked that the specialty sticker TAG membership be reassigned to review and approve the application. Yedidyah Langsam asked that all EMS agencies be sent reminders about obtaining security stickers. Mapping TAG: Materials are being retrieved from the REMSCO archives. Letters have been sent to all EMS agencies asking for their old paperwork to identify original boundaries. Over use of the spine board was discussed. SEMSCO recommends to continue to use the neck collar, but a patient can have safe spinal protection on a stretcher. The backboard would continue to be used as an extrication device. This will be further discussed at SEMSCO. The Joint Policy Statement for Ground Ambulances a federal document, was discussed. This identifies pediatric equipment recommended. The major deficiency found is how we transport pediatric patients without appropriate car seats. The next meeting of the Ambulance Committee is scheduled for Tuesday, October 28, TRAINING AND EDUCATION REPORT The Training and Education Committee of the Regional EMS Council of New York City will meet on October 7, REGIONAL EMERGENCY MEDICAL ADVISORY COMMITTEE REPORT REMAC met on September 9, The following is a meeting summary: Membership: Members were advised that they would have to sign Conflict of Interest forms to comply with new Not-for-Profit regulations. The following individuals were appointed to REMAC: o o Celia Quinn, MD, MPH, Medical Director of Healthcare System Readiness Bureau (Office of OEPR) to fill the seat vacated by Louis Soloff, MD. William Fisher, MD, has been appointed by the NYC DOHMH to fill the Psychiatric Physician seat vacated by Patricia Schwartz, MD. REMSCO voted unanimously to approve the new REMAC members. Multiple Protocols were reviewed and approved: 1. Pediatric Age Advisory: It states adults for ALL protocols are 15 years and older. Pediatrics are less than 15 years old. The GOP and Protocols will be reviewed and changed to be consistent. 2. Appendix H: Specialty Care Centers: This appendix has been revised to identify only NYS DOH designated centers. 3. Medical Control at the Scene: This section of the GOP has been revised to delete mention of unsolicited medical intervention. Only REMAC credentialed OLMC physicians can provide Page 7 of 9

8 direct medical control at the scene of a call. Other, un-credentialed physicians, cannot. Additionally, REMAC OLMC physicians can provide direct medical control to all agencies in the NYC system, except during MCI s when that responsibility belongs to FDNY OMA physicians. 4. Epinephrine Auto-injector BLS revisions: BLS Protocols 407 Wheezing and 410 Anaphylactic Reaction have been changed to remove the mandatory availability of the second dose of epinephrine auto-injector. 5. A REMAC Advisory was developed and distributed regarding the Lifevest, for educational purposes. 6. Etomidate administration in GOP for Prehospital sedation was revised. 7. Hemostatic Dressings: an advisory will be created allowing the use of hemostatic dressings at the discretion of the agency medical director. Further discussion regarding current regional paramedic credentialing exams is postponed until completion of a legal review. A request for support for a Mount Sinai Study using EMS providers to provide non-emergent patient follow-up was discussed. Although the concept was supported, REMAC did not believe it could provide waivers to credential involved physicians as OLMC without meeting all credentialing requirements. The next meeting of the REMAC will be on Tuesday, October 21, 2014 QUALITY IMPROVEMENT COMMITTEE REPORT The Joint Regional Emergency Medical Services Council/Regional Emergency Medical Advisory Committee (REMAC) Quality Improvement Committee will meet on October 7, STATE EMS COUNCIL REPORT The State EMS Council/SEMAC met on September 9 & 10, Yedidyah Langsam, PhD, representative to SEMSCO reported the following: CPAP training for EMT-Basics was approved. This is at the discretion of each region. Hypothermia has been suspended in all regions except NYC. NYS DOH would prefer notification first. Spinal Immobilization is being over-utilized. Use of spine board, except in extrication, is being eliminated. Patients can be safely immobilized on a stretcher with a collar. A TAG was formed to move this forward. The draft statement was approved. The ALS formulary will be finalized at the January 2015 meeting. The state epcr vendor has been mandated to upload data to the state bridge. There is a large amount of data still not available on the bridge. Agencies were reminded that a PCR must be left at the ED with each patient. Community Paramedicine TAG asked SEMAC for a statement of support to modify Article 30. Active Shooter Response Recommendations: supported by law enforcement. Approved for BEMS to develop guidelines. 9 CONs: Regional Councils must notify BEMS immediately upon receiving a CON/TOA application. Votes cannot take place until Competency & Fitness is approved by BEMS. CON TAG is continuing its work. The next meetings of the SEMSCO/SEMAC will be held on January 13 & 14, UNFINISHED BUSINESS No report. NEW BUSINESS Yedidyah Langsam stated the REMSCO should investigate how to receive funding for pediatric car seats in ambulances. At this time, unless pediatric patients are in MVA s, there are no car seats available for transport in the ambulance. It was suggested that there may be a State program for car-seats. Alison Burke, announced that the Greater NY Hospital Association will be hosting a NYS DOH Stroke Review on September 22 at 2pm. This will be held at the offices of the GNYHA. Announcements have been ed. This is an information session focusing on documentation requirements for patients presenting with Stroke. Marie Diglio announced that JFK BLS Ambulance has submitted its IN Naloxone Program notification. Nancy Benedetto stated that all members on all committees will be asked to complete and submit Conflict of Interest forms in order to comply with new regulations. Joseph Marcellino announced that Coney Island Hospital would host an Active Shooter course on Wednesday, September 24, from 1pm 3 pm. Vincent Barranco asked that in order to avoid confusion, at future Council meetings, only Council members sit at the table. Visitors should use seating provided away from the main table. No further business remaining, the meeting of the Regional EMS Council was adjourned. The next meeting will be held October 28, Page 9 of 9
