1 China Tourist Visa Requirements Here is a checklist of what you will need to provide: Passport: Applicant must submit their current passport. Passport must have at least two blank visa pages and must not expire within the next six months of entry to China. Passport Photos: Applicant must submit one (1) passport photograph. Non- US Citizens: If applicant is not a US citizen, applicant must submit original valid Alien Registration Card (Green Card), US visa, or other proof of legal status in the United States, with permission for re- entry. In addition, non- US citizens must submit one completed China Supplementary Form. The document is a fillable PDF and all fields with no answer must be answered "N/A." The completed form should be printed single- sided and signed in blue or black ink only. The application is a "fillable PDF" meaning that you must click in each answer field and type your answer. Once completed, print and sign the document. Applications should be printed single- sided and signed in blue or black ink only. Each traveler will need to provide a completed and signed Chinese visa application. The application itself can be found at the link below. The application is a fillable PDF meaning that you MUST type your answers into the blank fields. Once all the fields are completed, you will print the application and sign in the middle of the fourth page (signature line is indicated with a red arrow). We have provided a guide on the following pages which tells you how to answer each questions on the application. PLEASE FOLLOW THE GUIDE PRECISELY WHEN COMPLETING YOUR APPLICATION(S). You may be prompted by Adobe to "install simplified Chinese language pack." You must accept AND reboot computer in order for update to take affect before being able to complete the application.

It is recommended you print the Application Guide to use as a reference when completing the application. The application is a fillable PDF and all answers must be typed hand written applications will not be accepted by the embassy or consulates. All questions which are left blank MUST be completed with the answer "N/A." Incomplete or inaccurate answers will result in rejection of application and delayed processing. 1.1 Full English Name as in Passport Last name, middle name and first name as they appear in passport. 1.2 Name in Chinese Must be completed by anyone of Chinese, Taiwanese, Hong Kong or Macau decent.
