Doors open away from gym area. Emergency exits accessible and usable? Gym floors: Clear of objects which may cause tripping / slipping. Floor sockets covered and flush with floor. Floor plates secure in floor, hooks and plates in good condition and flush with floor? Gym lighting: Light bulbs not burned out? Page 9 of 16

Vendor inspections and maintenance: Verify that the schedule for vendor inspections and maintenance for all major gym facility and equipment is being conducted appropriately? Wall climbers and peg boards: Secure attachment to wall, peg holes and pegs in good condition, no cracks or splinters in pegs, pegs fit properly into holes? Workplace Organization and Housekeeping: Gym space should be free of, "stored" furniture/boxes/equipment along the perimeter walls and corners.
