2 COMING TO OUR NEIGHBORHOOD. When? On June 15, 2015 What? New Emergency Codes Why the change? This will allow us to align ourselves with our sister hospitals, and will provide continuity for those employees who work at multiple sites 2

4 Know the new codes STAFF RESPONSIBILITY Understand your personal role Keep yourself, patients and colleagues safe Keep yourself informed of changes with policies and procedures Be an active participant in creating a culture of safety 4

5 CALLING A CODE We will be using a Call Center for calling all codes, effective the same time we are changing to new emergency codes. We will no longer personally page codes overhead. Procedure: Upon discovery of any kind of code situation, you will notify the Call Center by dialing 911 from any hospital phone The Call Center will page the code through a paging system. Do NOT dial 9 to get an outside line first If your area uses a Code Blue Button, you will still use it PLUS someone will call 911 to have the Code Blue paged overhead. 5

6 CALLING A CODE/GETTING ASSISTANCE OUTSIDE OF THE MAIN HOSPITAL All clinics and facilities outside of the main hospital will notify the Call Center of any emergency, who in turn will obtain assistance, i.e., fire department, ambulance Hospital employees will NOT be responsible for obtaining assistance 6

7 CODE RED: FIRE/SMOKE EMERGENCY Used to notify everyone for small or large fires. Procedure: Follow RACE Pull the nearest fire pull Notify the Call Center, by dialing 911 (they will call the fire department) Use a fire extinguisher if appropriate 7

8 R = Rescue everyone in the area A = Alert everyone i.e., use the intercom, yelling WHAT IS RACE? C = Confine the fire i.e., close the door or smoke compartment E = Extinguish if you can or Evacuate the area 8

9 OPERATION OF A FIRE EXTINGUISHER P = Pull the pin at the top of the fire extinguisher A = Aim at the base of the fire S = Squeeze the handle S = Sweep from side to side Note: Most fire extinguishers at SJMC are ABC, which are designed for general use. 9

10 FIRE RESPONSE Key Points: If the fire is not in your area, close the doors to prevent the spread of fire/smoke. It is important to evacuate persons horizontally through the nearest fire/smoke doors. If vertical evacuation is required, move down the stairways to the outside. Know the location of fire extinguishers, pull stations and exits in your unit. If you become engulfed in flames, you should stop, drop and roll to extinguish the fire. 10

11 CODE BLUE: CARDIAC/RESPIRATORY ARREST Used to respond to anyone who is unresponsive or has had a cardiac or respiratory arrest. Procedure: Notify the Call Center, by dialing 911 from any hospital phone Begin CPR, if trained. Stay with the person until the Code Team arrives. Upon arrival of the Code Team, stay with their companions or take direction from the Code Team. Off campus offices will also notify the Call Center 11

12 CODE BLUE: CARDIAC/RESPIRATORY ARREST The Code Team is comprised of specific healthcare providers and Security. Roles noted in the Code Blue policy 12

13 WORKPLACE VIOLENCE Used for workplace violence, which may be threats or acts of violence. Procedure: If a threat: o Attempt to de-escalate the situation, if trained o Notify the Call Center, by dialing 911 If violence is occurring: o Notify the Call Center, by dialing 911 o Move those in danger to a safe area if possible 13

14 CODE PINK: INFANT/CHILD ABDUCTION Used if an infant or child is missing or suspected of being abducted (patient or visitor), Procedure: Notify the Call Center by dialing 911 Observe halls and exits for any suspicious persons or activity Secure exits as necessary 14

15 CODE YELLOW: DISASTER Used for any internal or external event resulting in casualties. i.e., major accident on I-94 or a snowstorm or tornado occurred and victims may arrive. Procedure: Activate your department s disaster response based on the event. Administration will determine if the Incident Command Center (ICC) will be set up. 15

16 INCIDENT COMMAND CENTER Definition: An organized system response to a disaster involving the hospital and local and potentially county and federal organizations as needed. Will typically be set up in Atrium Conference Room C, but may be moved at the discretion of Administration. Will be maintained throughout the disaster. Utilizes NIMS command and management components to respond to the incident or disaster. 16

17 CODE GREEN: MEDICAL EMERGENCY Used for any type of injury or illness involving staff, visitors or outpatients that require care i.e., falls, seizures, chest pain Does NOT include respiratory or cardiac arrest Procedure: Notify the Call Center by dialing 911 Remain with the person until responders arrive. Do not let the victim get up. If the person is unconscious, call a Code Blue The first responder should complete a VOICE report Response is the same as Code Blue Roles noted in Code Blue Policy 17

18 CODE TEAL: ELOPEMENT Used in the event of a patient elopement. Definition: The patient has disappeared from the unit and/or has left the hospital. The patient may not have the capacity to make a decision regarding leaving the hospital due to confusion, disorientation and/or mental health illness. 18

19 CODE TEAL: PATIENT ELOPEMENT Procedure: The unit RN will notify their Director and/or charge nurse Search the unit for the patient If unable to find the patient, an with a description of the patient will be sent to all SJMC employees Notify the Call Center, by dialing

20 ACTIVE SHOOTER Announced as an Active Shooter through a page. No specific code is used. If an active shooter were to arrive in our hospital, a page would be made through the intercom system announcing the event and the location to alert everyone in the building. As the shooter moves through the organization, an additional page will be made to keep staff informed. The ALICE response will be implemented. ALICE = Alert, Lockdown, Inform, Counter, Evacuate 20

21 DR. HELOISE Dr Heloise is an internal phrase used at SJMC to alert a colleague that you feel threatened by a perpetrator. * Dr. Heloise is NOT paged. Procedure: You would use Dr Heloise in a normal sentence when speaking with a colleague. The colleague will notify Security. Security will evaluate the situation upon arrival. 21

22 STAT STAFF Stat Staff is used by direct patient care providers to indicate a need for additional staff urgently. Procedure: A page is announced through the intercom system identifying the location and/or department. Available staff from other units will report to the location. 22

23 Please note the elimination of: Code C Code Black NEED TO KNOW. We will use Code White for Workplace Violence situations. TACT trained personnel will report to a Code White. Reference cards for the new emergency codes will be distributed soon to accompany name badges 23

24 SEVERE WEATHER In the event of severe weather i.e., tornado warning or tornado watch, all staff will be notified through an overhead page. Definitions: Tornado Warning: A confirmed tornado has been sighted. Tornado Watch: Weather conditions are right for a tornado to occur. 24

25 SEVERE WEATHER Procedure: Tornado Watch: Prepare your unit to mobilize if directed. Tornado Warning: Move patients in the Atrium Building to the center hallways Move patients in the Woodland areas to the lower level Wait until an all clear has been issued to return to your departments and normal procedures. 25

26 RAPID RESPONSE Used if a patient s condition is deteriorating and a healthcare worker needs assistance to analyze or take additional steps with the goal to eliminate a Code Blue. May be called by a patient, family or healthcare worker. Is not used for outpatients. Procedure: Call ICU for assistance at ext

27 OTHER INFORMATION If a disaster were to occur involving contagious diseases, infection control resources will also be involved for management i.e., Ebola, Smallpox, Measles, Influenza, etc. 27
