2 As the leading experts in peer support group development, Self Help Connect UK has been actively creating supported self-care approaches and resources for over 30 years. We are the leaders in developing systemised approaches to supported self-care for people living with long term conditions, providing commissioning support to the NHS since Utilising the collective experience of Self Help groups over the last 3 decades, Self Help Connect UK has developed a unique insight into the needs of people to take control of their own health and wellbeing, and to join with others to create active partnerships between individuals, their carers and health and social care professionals. At Self Help Connect UK we aim to support the commissioning of evidence based solutions that fit with the local experience of patients and carers. Long term conditions now take 70% of the health service budget. At the same time many people wish to be more informed and involved with their own care, challenging the traditional divide between patients and professionals and offering opportunities for better health through increased prevention and supported self-care. Simon Stevens Working with this model, Self Help Connect UK will support you to utilise existing assets to support self-care. We can support you to build a house that works for your patients and carers, and which builds on local community assets. Supported self-care is best described as support within a framework, offering patients and carers access to a range of services that provide equipment, knowledge, skills and encouragement to self-care and self-manage. Supported self-care provides information and signposting to local community based activities that support people to live well and build their own resilience and capacity to cope with a long term condition. Self Help groups bring people together who share a common problem. They offer practical and emotional support, giving members hope and encouragement to manage better and to know that they are not alone. Supportive professionals may signpost to groups, offer practical support and, in some cases, run groups. Being a member of a Self Help group can help people to better understand their long term condition, learn new coping skills and reduce feelings of isolation, fear and anxiety. Membership helps build confidence, selfesteem, sharing hope and inspiration with others in the same situation; helping people be more in control. Self Help directories are a valuable resource for patients, carers and health and social care professionals linking to wider directories of service they can stand alone or be integrated into multi-agency information resources for public and professional access. 4 COMMISSIONING SUPPORT: SELF CARE Our self-care business intelligence services identify the key providers and gaps in service within the self-care framework locally. Using our local analysis, we can assist in the development of local self-care strategies and implementation plans. This is done by co-designing models of engagement with local patients, carers and partner organisations to design self-care pathways that integrate with existing, new or emerging care planning systems. [CCGs should be] encouraging independent peer-to-peer communities to emerge Simon Stevens 2014 Self Help Connect Connect to Health Self Help Connect offers a range of commissioning support to create systemised self-care support through peer led Self Help groups. Working to deliver cost effective local support for new and emerging Self Help groups, we can guide you and your partners to develop a best practice Self Help group infrastructure that delivers quality support for your patients and carers. In addition, you will have a unique information resource which will enable effective signposting to groups. As part of a self-care review, strategy creation or as a stand-alone service we can help you build a vibrant Self Help group sector which can be fully integrated into your existing or emerging self-care offer. Our unique training for commissioners, health and social care professionals and third sector providers is delivered by experienced trainers who have first-hand experience of working with groups and delivering support and information services. Connect to Health is a new way of supporting patients and carers to connect to supported self-care activity in their local community. Fully integrated into Primary, Secondary health services and Community Services, Connect to Health offers a holistic approach to supporting self-care and care planning. Delivered as part of an integrated care system, Connect to Health has contributed to the reduction of unplanned hospital admissions and improvements in the quality of life for patients and their carers, Connect to Health systemises supported self-care and provides a bridge between professionals, patients and carers to access local support to manage long term conditions and improve health outcomes within a risk stratification framework. Dr Peter Ilves MBBS MRCGP is a GP principal and CCG/HWB commissioner, in Wandsworth There are four elements to what Self Help Connect achieved for and with us. Firstly they consolidated and further raised our awareness and understanding of the impacts that Self Help groups can have on individuals and communities. Secondly they helped us to realise our baseline assets that sat within Wandsworth. Although we thought we knew, we didn't. Thirdly they started to engage with communities, key contacts, join events and gently fortify existing groups and increase the number of functional groups. Fourthly they collaborated with an existing third sector organisation, imparting training, expertise and supported their conversion to Self Help Wandsworth. Due to the training, skill and expertise of Self Help Connect, Self Help Wandsworth has become a very successful organisation. We now have a local provider delivering the Self Help service but are doubly confident as we know that connections with Self Help Connect have been maintained. There has been a palpable increase in the number of Self Help groups and we are aiming to evaluate the impacts that these are having. From a community point of view we know that this element of what we have been trying to achieve has been very positively supported and received by the population we serve. Elaine Garrett is a Community Action, Health and Wellbeing Development Manager on the Isle of Wight I approached Self Help Connect with a view to mentoring me so I had exposure to colleagues working in the field who were in touch with national policy and research developments. Their support has given me the opportunity for some reflective practice. Testing my own ideas and plans with colleagues who are experts in the field and who bring me into contact with a strong and embedded organisational culture of Self Help within a wider third sector has been very helpful. I have also been signposted to resources including newly emerging research. Being mentored by Self Help Connect has tested and validated my own approach. More recently having access to their practical resources, both for groups and health care professionals has been enormously helpful. Although they had a very clear process and approach it always felt tailored and that we had ownership. There were several episodes and situations where we found Self Help Connect to be both adaptable, flexible and always inclusive of our needs Peter Ilves, GP
5 Self Help Connect have worked with us to develop our strategy and practice. Providing initial business intelligence to build the case for supported self care locally, they have supported us to develop a systemised approach to sign posing patients and carers to self-care support in their community. As an early adopter of the Connect to Health model within our integrated adult care teams, we have seen increased engagement with local VCS support services and better access to a broader range of support for our patients and carers. Their involvement from the start has been invaluable and we continue to benefit from their expertise as we grow our ambition to fully integrate supported self-care across Mid Nottinghamshire.
