12 The data and knowledge gateway PHE provides many high quality data and analysis tools and resources for public health professionals. The PHE data and knowledge gateway provides direct access to these resources. The resources help local government and health service professionals make decisions and plans to improve people's health and reduce inequalities in their area. 19 The Needs Assessment underpinning your local Health & Wellbeing Strategy. THE JOINT STRATEGIC NEEDS ASSESSMENT (JSNA) Presentation title - edit in Header and Footer 19

Purpose: To identify the current and future health and social care needs of the local population, including: addressing inequalities; recommendations for planning and commissioning; informing the H&Wb Strategy. Key points: A statutory requirement since 2008 Should be unique to the local area Should consider what local communities can offer in terms of assets and resources Should be part of a process of continuous strategic assessment and planning informing LA, CCG and NHS commissioning plans. Responsibility of: Local Authorities and Clinical Commissioning Groups (CCGs) have an equal and joint responsibility to prepare JSNAs and HWbSs, the duty of which is discharged through Health & Wellbeing Boards. 20 Public Health Intelligence
21 It is only the combination of the JSNA, joint health and wellbeing strategy and aligned commissioning plans that have the potential to be transformational in improving health, care and wider services for people in our communities. 22 What to expect Format: Historically large, paper-based documents Now largely web-based, interactive, and more widely accessible. Key points: Broadly data, facts and figures as well as narrative and story Links to nationally produced reports and tools as well as local reports and needs assessments Evidence of best practice and what works. Content: Health and care information across the life-course: maternity, infancy and preschool; school-aged children and young adults; working age adults; older people For each: who is at risk and why; level of need in the population; current services in relation to need; unmet need, and service/ information gaps. 22 Public Health Intelligence

24 Local initiatives Information integration: Recognising that people have ongoing and complex health needs requiring support from multiple agencies and various professionals Moving away from a traditional focus on isolated episodes of care Supporting the transformation of health and care services specifically the Better Care Fund (BCF). Population segmentation: Using private sector products to help conduct robust, dynamic population grouping exercises to help better target interventions, and evaluate programmes of work. Needs Assessments, Health Equity Audits and local profiling: Many and varied depending on local priorities, commissioning intentions, and information gaps Neighbourhood (Ward); Children s Centre; School Cluster. 24 Public Health Intelligence
