1 REQUEST FOR PROPOSAL (RFP) FOR ATHLETIC TRAINER SERVICES SCHENECTADY CITY SCHOOL DISTRICT This RFP is divided into three sections: I. Procedures for Responding to RFP School year RFP # C II. Overview of the Schenectady City School District III. Elements of Proposals. a. Professional Credentials and Experience b. Arrangements for Provision of Services c. Litigation Management Plan d. References e. Costs/Fees f. Insurance I. Procedures for Responding to RFP All completed proposals must be received by the District Clerk, David Weiser, no later than 9:00 a.m. on June 27, The address for submission is: Mr. David Weiser, District Clerk Schenectady City School District 108 Education Drive Schenectady, NY Please be certain that the firm name is identified on each page of the proposal. Please indicate the name and telephone number of the firm s contact person for your proposal. Each proposal should list all rates, fees and charges on a separate page(s). Selected firms responding to this RFP will be interviewed by a committee. It is anticipated that such interviews will take place during July Appointment to be made no later than on the July 20 th Board of Education Meeting. The District reserves the right to award all services to one firm or to award certain services or groups of services to different firms based on the proposal or proposals that best meet the 1

2 needs of the District. The decision will be based on all available information, and the firm(s) selected will not necessarily be the lowest bidder(s). The District also reserves the right to reject all of the proposals submitted in response to this RFP and/or to hire a firm not responding to this RFP. This RFP will be renewable for up to four (4) years at a mutually agreed upon rate annually. The District reserves the right to cancel the agreement with a forty-five (45) day notice to the provider of service(s) during the fiscal year or cancel or renew with a thirty (30) day notice prior to June 30 th of any given year. An annual engagement letter will be generated by the district. If there are any questions regarding this RFP, please contact David Weiser, District Clerk, at , extension II. Overview of Schenectady City School District The Schenectady City School District provides a full range of educational programs and services for over 10,000 students. The District operates two (2) Early Childhood Centers, nine (9) elementary schools, four (4) magnet schools, two (2) middle schools, a high school composed of five unique houses and an Adult/Community Education Center located throughout the city in twenty-one (21) separate facilities. A seven (7) member Board of Education establishes district policies and employs a Superintendent of Schools to manage the day-to-day affairs of the District and advise the board. III. Elements of Proposals A. Professional Credentials and Experience Please include in the proposals the following information in the order requested: 1. A brief description and history of the firm including the date established, the number of staff, and the areas of specialties in which services are provided. 2. A list of the members of the firm who are available to be assigned to the District, along with their professional credentials and a summary of their resumes. Please indicate the name of the primary contact for this account. 3. A listing of current school district clients, including dates of service, and references and telephone numbers. A separate list of non-school district clients with dates of service, references, and telephone numbers. -continued on the next page- 2

3 In connection with the client listing above, please describe your experience in representing small city and urban school districts. 4. The firm s professional memberships in New York State and national organizations. B. Arrangements for Provisions of Services 1. Please describe the steps the firm would take to keep the District informed of changes in the decisions concerning state and federal laws, rules and regulations that affect the operation of school districts. 2. Please describe the manner in which the interests of the District are protected from mistakes, errors or omissions by the firm. In this connection, please describe the position of the firm on indemnification of a client for the financial harm caused by action(s) taken and/or not taken by the firm or your employees. 3. Please see Appendices A and B for requested services. C. Litigation Management Plan Each proposal should include an outline and explanation of the firm s Litigation Management Plan. The Litigation Management Plan pertains to any litigation that arises from performance of duties. D. Request for References Please provide three names, business addresses and telephone numbers of contacts of current clients, preferably school districts. E. Costs/Fees 1. Please describe the services included as well as those excluded from the scope of the work covered by the retainer. Please state whether the annual fee includes any or all reimbursable expenses, such as but not limited to, telephone charges, mileage fees, copies, faxes, etc.. 2. Hourly rates for services not covered in the proposal or considered to be extra services. 3. Services for which another form of fee arrangement is proposed. 4. Any additional fees/costs proposed by the firm. Please list all forms of reimbursable expenses you will charge the District in addition to the above fees for services. 3

4 F. Insurance Requirements Include an insurance certificate to verify that you will be able to provide the following limits of liability for insurance: 1. Workers Compensation: Proof of Statutory Workers compensation and Employers Liability Insurance required for all employees. Proof of Coverage must be on the approved specified form, as required by the New York State Workers Compensation Board. ACORD certificate are acceptable. 2. Commercial General Liability Insurance: Limits of at least $1,000,000 per occurrence / $2,000,000 aggregate. The District must be named as an additional insured on a primary and in a noncontributory basis. 3. Professional Liability Insurance: Limits of at least $1,000,000 per claim. The District must be named as an additional insured. 4. Automobile Liability: $1,000,000 combined single limit for owned, hired and borrowed and nonowned motor vehicles. The Provider will agree to indemnify the District for any applicable deductibles. The policy naming the District as additional insured shall (a) be an insurance policy from an A.M. Best rated secured (A minus or better), New York State admitted insurer; (b) provide for 30 days notice of cancellation; and (c) state that the organization s coverage shall be primary coverage for the District, its Board, employees and volunteers. The District shall be listed as an additional insured by using endorsement CG or equivalent. If another endorsement is used, a copy shall be included with the Certificate of Insurance for District review and acceptance. In closing, the District reserves the right to change the above requirements before or during the engagement with thirty (30) day notice. 4

5 NON-COLLUSION CERTIFICATION By submission of this RFP/bid, each bidder and each person submitting on behalf of any bidder, certifies and in the case of a joint bid, each party thereto certifies as to its own organization, under penalty of perjury, that to the best of his knowledge and behalf: 1. The prices in this RFP/bid have been arrived at independently without collusion, consultation, communication, or agreement, for the purpose of restricting competition as to any matter relating to such prices with any other bidder or with any competitor. 2. Unless otherwise required by law, the prices which have been quoted in this bid have not been knowingly disclosed by the bidder and will not knowingly be disclosed by the bidder prior to opening, directly or indirectly, to any other bidder or to any competitor, and 3. No attempt has been made or will be made by the bidder to induce any other person, partnership or corporation to submit or not to submit a bid for the purpose of restricting competition. 6 Appendix A Athletic Trainer Services Request for Proposal Specifications Proposals Due: June 27, 9am RFP # c Athletic Training Services: 1. Service all athletes involved in interscholastic athletics, including tryouts, conditioning, reconditioning, practice, and pre-game activities or competitions. 2. The term reconditioning shall include services identical to those conditioning services provided for purpose of developing and maintaining the initial requisite of cardiovascular and neuromuscular capacities necessary to compete in their respective sports after having met the standards of competition established by the school district through its Chief School Physician, or other official medical designee, that any previously existing physical injury or limitation has been appropriately treated, and as necessary rehabilitated. 3. Through an Athletic Trainer and as requested by the School District, the contractor will provide athletic training services for the benefit of the above-described students. Services may include one or more of the following components: a. Education of all coaches regarding sports first aid and injury protection b. Ordering of athletic training supplies and inventory c. Keep injury records of all injured student athletes d. Assisting coaches in preparation of accident reports e. Making appropriate recommendations for the care and reconditioning of injured athletes f. Designing weight training and conditioning programs for all teams g. Preparation of all first aid kits for use by coaches and teams in the trainer s absence h. Daily communication with coaches regarding player status for practice or game per physician recommendation 6

7 i. Assisting the Chief School Nurse in arranging for the examination of athletes by a physician prior to participating in sports j. Instruction of coaches and assisting in the proper fitting of athletic equipment. k. Assisting the School District as requested in confirming that athletes have the appropriate medical clearance before returning to sports subsequent to athletic injury or illness l. Advising student athletes regarding appropriate training techniques m. Preparing athletes for practice or game participation n. Conferring with School Health officials on matters relating to injured athletes o. Advising, as requested by the Athletic Director, on the safety of all athletic facilities p. The Athletic Trainer will provide nutritional counseling to all athletes that request it, informing them on safe weight reduction and weight gain 7

Coverage of all boys and girls home basketball games 6. Coverage of all home wrestling matches 7. Coverage of all home boys lacrosse games 8. Midnight Madness Cheer competition 9. All home hockey games 10. Coaches first aid and CPR recertification 11. Schenectady Invitational Wrestling Championships 12. Cheerleading competition 13.
