3 1 1.0 Key words Application, form, guide, electrical, assist, connection, applicant. 2.0 Introduction This guide is designed to assist all applicants in completing Customer Services related forms. The content has been developed on a non-technical level and easy to understand for any person without an electrical background. This guide will assist applicants but not replace Customer Service staff and their knowledge. The guide shall speed up the applicant process, reduce incorrect information and ensure all correct documentation is submitted timeously. The form will be available online or at any customer service centre. 3.0 Reference Documents The latest revision of the following documents shall apply: 3.1 Application for new/changes to electrical connection. 3.2 Meter card. 3.3 Constitution of the Electrical Contractors' Association South Africa 3.4 Certificate of Compliance 3.5 EThekwini Electricity Tariff Brochure 3.6 EThekwini Electricity Bylaws 4.0 Definitions and Abbreviations Applicant Any person whom completes and submits a form. This person maybe but not limited to an electrical contractor, developer, business owner, property owner or any person duly authorised and mandated to act on behalf of represented persons. Form A form may refer to but is not limited to an application form, meter card, CoC and other supporting documents. 5 3 5.0 General Information 5.1 Read all instructions and requirements carefully. 5.2 Use a black pen only. Do not use a pencil, red or green pen. 5.3 Complete all required fields in block letters (capital letters). 5.4 All supporting documentation must be certified as original and not older than three months. 5.5 Any incorrect information can lead to your application being delayed. 5.6 Any mistakes must be struck out neatly and initialled or complete a new form. 5.7 Fields with options shall be marked with an 'X'. 5.8 If you are unsure of any fields or require advise, contact any Customer Services branch for further assistance. 6.0 Application for New / Changes to Electrical Connection 6.1 WHEN SHOULD I COMPLETE THIS FORM? For a completely new development i.e. no existing electrical infrastructure is at the property. Building supplies for construction sites shall also use this application (The site will be metered) Application for new temporary supply (e.g. show, festival, religious, sport events). This connection is only permissible for a maximum of fourteen (14) days only and is unmetered. You will be charged for estimated consumption before the event Changes to existing electrical connection e.g. changing from credit to prepaid, changing meter position, pole, or connection type (e.g. overhead to underground vice versa) Power enquiry (quotation for new or changes to electrical connection) Reinstatement of electrical connection e.g. connection may have been stolen, connection removed due to non- payment For Lighting applications, the applicant should advise Customer Services on quantity and wattage of lamps used. Where low wattage consumption is required, consumer to provide actual current requirement.

6 6.2 WHAT SUPPORTING DOCUMENTS ARE REQUIRED? We have six different types of applications and supporting documents vary per type of application. 7 5 6.3 HOW DO I COMPLETE THE FORM? Section 1: Particulars of Owner Only the property owner is authorised to make changes to their electrical connection. If land is being developed and has no owner, the developer must write their details. 1. 8 For an informal address: There might not be defined road names, hence use landmarks such as shops, buildings, schools to describe and uniquely identify the site affected. Further locality shall be added such as the nearest transformer (TRF) and pole (P). You may be required to meet our technical staff at site should they be unable to locate your premises. 9 7 How to get GPS Co-Ordinates using Google Maps: 1. Go to maps.google.co.za 2. Search for your address in the search bar (e.g. 1 Jelf Taylor Crescent, Durban) 3. Zoom in/pan to the correct location 4. Right click Whats here? 5. The co-ordinates will appear in the search bar and the location will appear with a green arrow Section 3: Electrical Representative This refers to an Electrical Contractor or Consultant. An Electrical Contractor as defined by the ECASA constitution: a person who is currently registered with the Electrical Contracting Board of South Africa as an electrical contractor and who undertakes to perform electrical installation work and/or the verification and certification of the construction, testing and inspection of electrical installations on behalf of any other person, but excluding an employee of such first-mentioned person. It's up to you the customer (applicant) to ensure the electrical contractor you use is registered with the relevant authorities and adheres to SANS and other safety regulations. Their registration with the authorities shall be valid at the time of installation. EThekwini Electricity does not screen, regulate or suggest any panel of electrical contractors.

This choice might not always be available as it's dependant on the existing electrical supply in the area. If you are unsure, leave it blank and a Customer Service staff will advise you. 11 Section 5: Meter Type Field Name Description Example Credit Prepaid (Wireless) Prepaid (Comms) Bidirectional A meter that continuously records electricity consumption and is read every three months. The consumption is paid after (post usage) a bill is generated based on the consumption. Electricity units must be purchased before consumption and no bill or meter readings are conducted. Once all units are consumed there will be no electricity. (Same as prepaid cellphone airtime) Prepaid meter that does not use any cables between the meter and keypad but requires the keypad to be connected to any plug point in the house to communicate. Prepaid meter that uses communication cables between the meter and keypad Meter allows energy to be exported back on the grid, designed for Embedded Generation Select the appropriate meter option with an 'X' Section 6: Reason for Change Complete this for change to electrical connection Completed section with sample data 6 Reason for Change: Existing Meter No.: Overhead to Underground Boundary Metering Existing Connection No.: E Service Cable/ Meter position Demand change Tariff change Section 7: Date Supply Required This is the date you require the electrical connection to be installed. For installations that are required as soon as possible, write the current date of application. Use the date format YYYYMMDD (e.g. 11 August 2014 will be ). 7. Date supply required:

12 6.3.8 Section 8: Type of Supply All electrical connections shall be metered except for temporary supplies that are less than fourteen (14) days. 8. Type of supply: Metered Unmetered temporary Period (<14 days): Section 9: Requested Demand Residential customers shall ignore this field. This field is for large power users such as businesses. A kva is 1,000 Volt-Amperes referred to as apparent power. This field is best suited for an electrical consultant/ contractor to calculate and advise a customer. 9. Requested Demand (kva): 30 kva Section 10: Requested Power This is for pure Lighting Applications where the quantity and wattage of lamps used shall be stated. Where low wattage consumption is required, consumer to provide actual current requirement. 10. Requested Energy (kw): Section 11/13: Tariff / Scale Our tariff book explains in detail the diferent types of tariffs dependant on the type of electrical user you are. 13 Section 12: No. of Phases The electrical supply can either have single or three phase. Your electrical contractor or Customer Services staff will be best suited to advise you on the number of phases you require. Single phase will only have one live wire, neutral and earth. Three phase has three individual live wires, neutral and earth (total of five wires). Regular residential houses will use single phase. Three phase will be better suited for larger houses with swimming pools, central air-conditioning and multiple rooms etc. Businesses with large loads such as motors, heaters, lighting, cooling should use three phase. The cost for three phase is significantly more and thus a thorough analysis should be conducted before selecting three phase. 12. Number of Phases: Single Three Section 14: Details of Main Switch This section is best suited for an electrical contractor to complete and advise a customer. Voltage: Example, single phase V or three phase V Current: maximum load intended for the supply Fault Rating: found on the circuit breaker Protective device: refers to a circuit breaker or isolator that shall be present. If none is found on site, an appropriate protective device shall be installed. 14. 14 Section 15: Terms and Conditions These terms and conditions need to be accepted and signed irrespective of the type of application. It must also be dated using YYYYMMDD format (e.g. 11 August 2014 will be ). I accept that: (a) I am bound to conform with the Legislation and policies in force from time to time relating to Electricity Supply and undertake to pay any fees levied in terms of such Legislation, and (b) I authorise ethekwini Electricity to use drilling equipment on the building structure to secure the service connection and equipment referred to in (c) and (d) (c) The cost of the connection will allow for a maximum length of 10 meters of cable from the connection point on the building to the electricity dispenser, which must be placed inside the building on an outside wall. (d) I indemnify ethekwini Electricity against any damage, structural or otherwise that might be caused whils installing or maintaining the service cable, service connection, metering and associated equipment on the propoerty described above. Signature: Date: (YYYY/MM/DD) 7.0 Meter Card 7.1 What is a meter card? A meter card is proof and pre-compliance (some electrical tests done) that the affected property is ready for EE staff to perform changes or install a new electrical connection. 7.2 When do I complete and submit the meter card? The meter card shall be submitted when the premises is ready for the connection / changes to be completed by our technical staff. Premature submission (i.e. site does not meet EE standards) of the meter card will result in additional charges and delay the completion of the connection. Failure to submit the card and incorrect information can lead to the job being delayed. 7.3 Who completes the meter card? The meter card may only be completed by an electrical contractor. Any person (applicant or electrical contractor) may physically submit the card to Customer Services. 8.0 Certificate of Compliance 8.1 What is a Certificate of Compliance (CoC)? An Electrical Certificate of Compliance is a certificate issued by a registered person whom certifies and deems your electrical connection safe according to minimum standards set out in legislation. 8.2 When shall a CoC be submitted? The CoC shall be submitted no later than 48 hours (two working days) after EE staff have completed the installation (new, changes or reinstatement to connection). 8.3 Who completes the CoC? Only a registered person that is in possession of a valid licence may issue a CoC. The person must be registered with the Department of Labour.
